Endocarditis

Vegetation around the Valves

Sub Acute and Acute Stages

R.Jonas, MSN, RN RevisedSpring2008 1

Anatomy and Physiology
• Recall the layers of the heart
• What are the functions of the valves in the
heart?
• Why do we care about the valves when
talking about endocarditis?
• Review

R.Jonas, MSN, RN RevisedSpring2008 2

Infective Endocarditis
• Defined as bacterial or fungal infection of the
endocardium that includes the valves-infection
of the heart chambers or valves

R.Jonas, MSN, RN RevisedSpring2008 3

Infectious Cardiovascular Disorders

• Infectious Cardiac disorders can alter the

structure and function of the heart
• The endocardium is contiguous with the
valves of the heart….
• Etiology- viruses, bacteria, or inflammation
• Acute and sub-acute

R.Jonas, MSN, RN RevisedSpring2008 4

How does Infective Endocarditis
Happen?
• What are some causes?
– Endothelial lining?
– Vegetation?

R.Jonas, MSN, RN RevisedSpring2008 5

Pathophysiology
• Turbulent blood flow resulting from
valvular diseases
• Invasive procedures
• Dental Procedures
• Causes vegetation on the valves

R.Jonas, MSN, RN RevisedSpring2008 6

Infective Endocarditis

The more virulent bacteria causing the acute

bacterial form of infective endocarditis can lead to
Infective Endocarditis-caused by serious destruction, as shown here in the aortic
Staph Aureus; note tan, reddish valve.
vegetation Irregular reddish tan vegetations overlie valve
cusps that are being destroyed. Portions of the
vegetation can break off and become septic
emboli.

Predisposing Factors

• What kind of clients are at risk for this?

R.Jonas, MSN, RN RevisedSpring2008 10

Managing Endocarditis
• Nursing Care
– Name some interventions

• Teach client about what?

R.Jonas, MSN, RN RevisedSpring2008 11

Diagnostics for Infective
Endocarditis
• Name and explain them…

R.Jonas, MSN, RN RevisedSpring2008 12

• Infective Endocarditis

R.Jonas, MSN, RN RevisedSpring2008 13

Nursing Diagnoses R/T Endocarditis

• Risk for decreased c/o r/t mechanical

changes in heart valves
• Risk for altered cardiopulmonary, cerebral,
and peripheral perfusion r/t
thromboembolus
• Name another one……

R.Jonas, MSN, RN RevisedSpring2008 14

Summarizing
• What is it?
• How do you recognize it?
• What nursing care do you give
• How do you diagnose it?
• Nursing Diagnoses
• Medications?
• Linked criteria or diagnoses…
