El abordaje percutáneo se sigue utilizando como técnica quirúrgica en el tratamiento de la litias... more El abordaje percutáneo se sigue utilizando como técnica quirúrgica en el tratamiento de la litiasis renal y en casos en que se requiere drenaje de una vía urinaria obstruida. Presentamos una serie de pacientes tratados con un acceso percutáneo dorsal, el cual tiene la ventaja teórica de permitir acceso simultáneo al riñón por vía anterógrada y retrógrada simultáneos. La serie consta de 23 pacientes, 17 tratados por litiasis y 6 en los cuales se colocó una nefrostomía percutáneo temporal. Los resultados fueron excelentes, logrando acceso a la pelvis renal en todos los casos. Hubo 2 complicaciones no significativas. El acceso percutáneo en decúbito dorsal es una técnica reproducible, que podría ser de elección en algunos casos, como pacientes con deformaciones músculo-esqueléticas o trastornos respiratorios, que impidan su colocación clásica en decúbito ventral.

Una de las variantes más importantes la constituye el adenoma de la corteza suprarrenal productor de aldosterona (síndrome de Conn).El tratamiento de elección en este subgrupo de pacientes es la extirpación de la lesión. Se presenta una serie inicial de pacientes portadores de aldosteronoma sometidos a adrenalectomía parcial por vía laparoscópica.se revisó la casuística de pacientes hipertensos sometidos a adrenalectomía parcial laparoscópica entre noviembre del 2001 y marzo del 2004 debido a hiperaldosteronismo primario. Todos presentaron estudio de imagen (tomografía computarizada) compatible con tumor de la corteza suprarrenal, y en 2 pacientes la lesión fue bilateral. Un paciente tenía antecedente de adrenalectomía incidental durante colecistectomía abierta efectuada años antes.se operaron 16 pacientes, 13 mujeres y 3 hombres, con una edad media de 55,4 años. Se efectuaron 18 adrenalectomías laparoscópicas: 17 cirugías conservadoras y una adrenalectomía total en un tumor de 4,3 cm en una paciente con lesión bilateral. El tiempo operatorio medio fue 70,9 minutos, con un sangrado promedio de 30 ml. No hubo complicaciones ni necesidad de conversión a cirugía abierta. La estancia hospitalaria postoperatoria fue de 2,8 días. En todos los casos hubo mejoría total o parcial de la hipertensión.la serie, aunque pequeña, confirma que la suprarrenalectomía parcial por vía laparoscópica es reproducible, con buenos resultados y con los beneficios de la cirugía mínimamente invasiva.primary hyperaldosteronism is one of the few potentially curable causes of secondary arterial hypertension. One of the most important variants is the adenoma of the adrenal cortex that produces aldosterona (Conn's Syndrome). The treatment of choice in this subgroup of patients was the removal of the lesion. An initial series of patients with aldosteronoma subjected to partial laparoscopic adrenalectomy is presented.We examined the case selection and methods applied to hypertensive patients subjected to partial laparoscopic adrenalectomy between November 2001 and March 2004 due to primary hyperaldosteronism. They all presented an imaging study (CT scan) compatible with a tumour of the adrenal cortex and, in two patients the lesion was bilateral. One patient had a history of incidental adrenalectomy during and open colecistectomy performed some years previously.we operated on 16 patients, 13 of them women and 3 men, with a mean age of 55.4 years. We performed 18 laparoscopic adrenalectomies: 17 conservative operations and one total adrenalectomy of a 4.3 cm tumour in a patient with bilateral lesion. The mean duration of the operations was 70.9 minutes, with a mean bleeding rate of 30 ml. There were no complications or the need to resort to open surgery. Postoperative hospital stay was 2.8 days. In all the cases, the hypertension improved totally or partially.although small, the series confirmed that partial laparoscopic suprarenalectomy can be performed with good results and with the advantages of minimally invasive surgery.

To describe a case of renal angiomyolipoma treated by robotic assisted surgery. We report the cas... more To describe a case of renal angiomyolipoma treated by robotic assisted surgery. We report the case of a 26 year old females patient, in the context of third month pregnancy, who was diagnosed of spontaneous self-limited retroperitoneal hemorrhage due to renal angiomyolipoma. The patient was treated conservatively until normal delivery. At the 3rd month postpartum a robot-assisted (Da Vinci S) nephron sparing surgery (partial nephrectomy) was performed. Despite being a benign tumor, there are cases in which the renal angiomyolipoma requires surgical treatment. To our knowledge, after a thorough review of the literature, this would be the first reported case of angiomyolipoma treated with conservative surgery with robotic assistance (Da Vinci S-HD).

Objective: Congenital adrenal hyperplasia (CAH) is an uncommon syndrome which represents a therap... more Objective: Congenital adrenal hyperplasia (CAH) is an uncommon syndrome which represents a therapeutic challenge. We analyzed the role of bilateral simultaneous laparoscopic adrenalectomy in the management of CAH. Material and methods:: Between October 2004 and September 2006, three female patients underwent bilateral simultaneous laparoscopic adrenalectomy for CAH. Data were retrospectively collected. Variables analyzed were persistence of CAH clinical signs, variations in 17 OH progesterone level and corticoid medication, operative time, median blood loss, postoperative pain, hospital stay, and body image perception after surgery. Results: Median age was 16.3 years. Complete regression of virilization signs, acne and hyperpigmentation was achieved in one case. The other two cases showed partial regression of signs. Levels of 17 OH progesterone reached normal parameters in all cases. Steroids doses were lowered and given only for replacement purposes. Mean operative time was 125, 65 and 60 min for whole, right and left procedure, respectively. Median blood loss remained under 50 ml in all cases and there were no complications. Median postoperative pain level was 5 according to visual analog pain scale. Median hospital stay was 4 days. Conclusion: Bilateral simultaneous laparoscopic adrenalectomy shows all the advantages of minimally invasive surgery, and appears a viable alternative to medical management, which is not exempt from complications.

There are no pathognomonic clinical signs and management is controversial due to the bleeding risk. We report a bladder cavernous hemangioma resolved using bipolar transurethral resection.METHODS: We review the case of a female patient who presented with asymptomatic hematuria. On cystoscopy we discovered a reddish sessile lesion compatible with bladder hemangioma. We describe the diagnostic work up, surgical management and review other therapeutic alternatives for these lesions.RESULTS: Fifty five year old healthy female patient consulting for total painless hematuria. Cystoscopic evaluation revealed a 1 cm diameter sessile reddish elevated lesion near the bladder neck. We performed a transurethral endoscopic resection using the Gyrus Bipolar resectoscope®. Pathologic report concluded cavernous angioma.CONCLUSION: Bladder hemangiomas are benign and rare lesions. Clinical presentation has no pathognomonic signs although gross painless hematuria is the most frequent complain. Management is controversial due to the bleeding risk of this highly vascularized lesion. However, it appears that small lesions could be treated using transurethral resection. Although they have a benign course, follow up is mandatory to detect recurrence or residual disease.

OBJECTIVES: Vesicovaginal fistulas are uncommon and remain a surgical challenge. Abdominal hyster... more OBJECTIVES: Vesicovaginal fistulas are uncommon and remain a surgical challenge. Abdominal hysterectomy remains their main cause. We present our experience in laparoscopic repair of vesicovaginal fistula.METHODS: Between January 2006 and January 2008, 8 laparoscopic transvesical procedures for VVF repair were performed at our institution (O'Connor technique). For each case we analyzed demographic and surgical variables as well as the outcome. Surgical technique is also described.RESULTS: Average patient age was 45 years. All patients had past history of hysterectomy and subsequent history of urine leak through vagina, with a mean evolution time of 22 months. The average operative time was 150 minutes and the mean hospital stay 4.7 days. All patients evolved without complications except one who had urinary tract infection (Pseudomonas aeruginosa) that was treated with antibiotics. After a mean follow up of 32 months, there were no recurrences.CONCLUSION: In experienced hands, transvesical transabdominal laparoscopic vesicovaginal fistula repair is a feasible and safe procedure, preserving all the advantages of minimally invasive surgery.

The basaloid carcinoma of the prostate (BC) is a rare malignant neoplasm arising from the basal c... more The basaloid carcinoma of the prostate (BC) is a rare malignant neoplasm arising from the basal cells of prostatic ducts and acini. We report a case and review the literature. A 76-year-old man presented with symptoms of lower obstructive uropathy, the IPSS score was 29 and prostate specific antigen (PSA)of 0,924 ng /ml. Transurethral resection of prostate (TURP) was performed in September 2008, histopathological diagnosis was BC. In February 2009 laparoscopic radical prostatectomy was performed. Histopathological examination revealed a BC with adenoid cystic growth pattern, perineural infiltration and focal involvement of the left seminal vesicle. Immunohistochemically, the cells were negative for PSA, stained and were strongly positive for specific monoclonal antibodies anti-cytokeratin 34βE12, p63 and BCL-2. The patient has 23 months of follow-up, with complete continence and no evidence of tumor recurrence. The BC is an extremely rare subtype of malignant tumors of the prostate, where immunohistochemistry plays a fundamental role in diagnosis.

Teratomas confined to the adrenal gland are exceptional; only 3 cases have been reported in the English-language literature. We report 2 cases of mature teratomas of the adrenal gland that were laparoscopically excised.

L a cirugía endoscópica a través de orificios naturales es una tecnología emergente que combina l... more L a cirugía endoscópica a través de orificios naturales es una tecnología emergente que combina los principios de la endoscopía y la laparoscopía para realizar cirugía tradicional sin el uso de incisiones abdominales 1,2 . A diferencia de la cirugía abierta y laparoscópica tradicional, el acceso a la cavidad abdominal es primero establecido por el paso de los equipos endoscópicos a través de los orificios naturales. Se crea una incisión transvisceral controlada y se pasa el equipamiento endoscópico en la cavidad peritoneal permitiendo la visualización de los órganos intraabdominales. Gettman et al 3 .
