Licensee Lists

This tool allows for downloads (to open in Excel ®) of active Rhode Island licensee contact information by profession. The license data include: Name (last, first, middle or facility name), Owner/Manager Name, Identification Number, Profession, Specific Type, Status, Specialty (where applicable), Issuance and Expiration Dates, Business Address (3 lines), City, State, ZIP, Email, Business Phone, and Fax Numbers. The following beds counts are also included for applicable license types: Total Capacity, Alzheimer/Special Care Unit, Skilled Nursing Facility, Long-Term Care, Skilled/Long-Term Care, and Private Pay. This information gets updated daily.
