Reporting

The National Science Foundation (NSF) requires KY NSF EPSCoR to report annually on (1) the demographic information of project participants; (2) measurable outcomes such as publications, outreach, and new proposals; and (3) degrees awarded. Collection of this information is authorized by the NSF Act of 1950, (42 U.S.C. 1861, et seq.)

KY NSF EPSCoR TEAM MEMBER FORM

WHO: All financially supported KY NSF EPSCoR participants (faculty, students, technical staff, subcontractors, etc.) are asked to complete this survey each grant year.

TIME COMMITMENT: Approximately 3-5 minutes

Providing participant demographic data is voluntary, but strongly encouraged by the NSF sponsor.  The demographic data is aggregated by role on the project (e.g., faculty, undergraduate students, graduate students, etc.) and is reported to NSF in aggregate, not by individual.

Demographic details can be reported by keeping the “About You” section in NSF’s EDOCS, https://edocs.epscor.nsf.gov, updated for each grant year. If the CLIMBS project does not appear in your EDOCS account, please request access from Jeff Mossey, jeff.mossey@uky.edu.

EXTERNAL ENGAGEMENT + OUTREACH ACTIVITIES FORM

WHO: All financially supported KY NSF EPSCoR participants are asked to complete this survey each grant year.

TIME COMMITMENT: Approximately 5 minutes per entry. 

Outreach is an essential component of NSF EPSCoR’s investment strategies which seeks to strengthen the academic research competitiveness in Kentucky. Our researchers, students, and staff engage with external audiences including K-12 educators, community colleges, informal education facilities, and industry. 

To track participant productivity for annual reports, use the following form to report Outreach Activities directed toward:

o   Scientists and students within or beyond the universities involved in the CLIMBS project;

o   K-12 institutions and teachers that provide instruction in science or engineering; 

o   The general public through lectures, social media, museums, or other informal learning settings; 

There is no retained username or password; instead, simply provide a new record for each new activity and event. Collected information will be aggregated without any personally identifying information for reporting to NSF.  Participation is voluntary and all answers and comments you provide will remain 100% confidential.

KY NSF EPSCOR Report Guidelines for EOC, URE, and RA Awards

WHO: PI and supported students of KY NSF EPSCoR Annual Seed Funded and External Awards

TIME COMMITMENT (STUDENTS): Approximately 5 minutes. 

A KY NSF EPSCoR award final report is a brief document intended to describe your use of EPSCoR support to further your research/outreach and to provide performance information for annual reporting to the NSF. The report should not be a full explication of the project, however, if you have produced a lengthier summary (e.g., a draft of an academic paper) please attach it as a separate document. PIs should direct supported student participants to http://kynsfepscor.uky.edu/reporting to complete a short survey on their experience.

The report should not exceed 3 pages (excluding illustrations/figures/images), submitted as a Word or PDF document to crofcheck@uky.edu no later than 90 calendar days after the period of performance end date. All final reports are required by the terms and conditions of the award.

The final report should contain the following elements found in this .pdf:
