The NMDA Dairy Bureau offers Certificates of Origin and Free Sale. It also offers Sanitation Certificates as a service to the plants that we inspect. These certificates assist these companies in exporting their products out of the United States.
If a plant or distributor needs a certificate please do the following:
- Download and fill out the sample Certificate of Origin (PDF) and Sanitation Certificate (PDF) as completely as possible.
- Print out the completed form, or you may email it by clicking on the “Submit by email” button (after downloading and filling out).
- Fax or mail the completed certificates to the Dairy Bureau office:

- Include any special instructions
- Include a Federal Express Account Number, or preferably a completed FedEx label. To charge the cost of shipping the Certificates to the required location
- Please allow as much time as possible to process these certificates
Once received turn around time is typically three to five days.
