Please note that certain regulations prohibit providing information deemed confidential, including the Health Insurance Portability and Accountability Act (HIPAA), the Family Educational Rights and Privacy Act (FERPA), the Clery Act, and other information that may be protected by state and/or federal law(s).

Tennessee residents can request records pursuant to law. Requests can be made verbally, through electronic mail, via fax or through the U.S. Postal Service or other means of physical delivery. Requests should be as specific as possible and can be made during normal business hours (8 a.m. to 5 p.m., Monday through Friday). Our office reserves the right to confirm Tennessee residency.
