If you want to remain anonymous, please do not provide your name on the form. You can provide as little or as much information as you want. If you want feedback from the BBE DEI committee and/or the BBE DEI committee chairs, please provide an email address (can be an anonymous address) or a phone number and request feedback on the form you send.
