Member Form Submission Guidance
Pre-Retirement Beneficiary Designation Form Submission Reminder
- Pre-Retirement Beneficiary Designation Forms received by fax will be returned to the employer. Do not submit with Employee Data Form, these will be returned.
- Active and Inactive NMERB Members must complete a Pre-Retirement Beneficiary Designation Form to designate a beneficiary for their benefits. Members should review their pre-retirement beneficiary designation when significant life events occur and update if necessary.

- Only current forms will be accepted. Current forms are listed on the this website. Please update your organization’s intranet and any other resources that contain NMERB Member Forms.
- Submit original documents via mail or electronically through DocuSign when applicable.
