Cranial Nerve Assessment

The cranial nerve assessment showed normal function of CN II, III, IV, VI, VII, IX, X, and XII. CN I and VIII were not assessed due to the patient's inability to speak. Examination of CN V revealed intact sensation and blink reflex. CN XI showed right-sided paralysis of the extremities. Motor function was diminished on the right compared to normal function on the left.

CRANIAL NERVE ASSESSMENT CN I (Olfactory Nerve): Not Assessed(Patient cannot speak) CN II (Optic Nerve): uses eyeglasses, Visual Acuity

and Confrontation were not assessed CN III (Oculomotor Nerve): 3mm in diameter normal pupil, and failure to follow the object with both eyes, PERRLA CN IV (Trochlear Nerve): normal eye position and intact extra ocular muscle movements CN V (Trigeminal Nerve): can clench teeth, blink reflex is present CN VI (Abducens Nerve): can follow object with both eyes CN VII (Facial Nerve): symmetric facial movements CN VIII (Acoustic Nerve): Not assessed CN IX (Glossopharyngeal Nerve): palates are intact, intact gag reflex CN X (Vagus Nerve): tongue is placed midline with no lesions, papillae and with light white coating, able to speak CN XI (Spinal Accessory Nerve): there is presence of right sided paralysis of the extremities CN XII (Hypoglossal Nerve): Tongue in midline, able to stick out tongue and move it from side to side, up and down
