    dated December 12, 2012;
    2. I have caused the preparation of the foregoing petition and I have read and
    understood the contents thereof, and I hereby verify that its contents are
    true and correct of my own personal knowledge and based on authentic
    records;
    3. I hereby certify that I have not commenced any similar petition involving
    the same issues in any court, tribunal or quasi-judicial agency, and to the
    best of my knowledge, no such other action or claim is pending; and
    4. I hereby undertake that should I hereafter learn of any other such action or
