Activity Intolerance

The patient presented with elevated vital signs, weakness, and low hemoglobin and hematocrit levels. This indicated an intolerance to activity due to an imbalance in oxygen supply and demand. The nurse's goals were to decrease the patient's physiological signs of intolerance within 8 hours. Interventions included monitoring vitals, assisting with limited activity, bed rest, deep breathing exercises, oxygen supplementation, and a quiet environment. After 8 hours the goal was met as the patient's physiological signs of intolerance decreased.
