The document outlines procedures for pilots to follow during abnormal/emergency situations on the aircraft, including task sharing between the PF and PNF as well as how to use the ECAM and QRH summaries during failures.

For all abnormal/emergency procedures the task sharing is as follows: PF is responsible for thrust levers, flight path/airspeed control, aircraft configuration and navigation/communications. The ECAM should be applied first to perform the procedure and review the STATUS section. IMPORTANT
SCOPE
The QRH contains some specific procedures which are not displayed on ECAM.
As a general rule, procedures displayed on the ECAM are not provided in the QRH
(refer to FCOM 3.02).
TASK SHARING FOR ABN/EMER PROC
For all abnormal/emergency procedures the task sharing is as follows:-
PF - Pilot Flying - responsible for:-
Thrust levers
Flight path and airspeed control
Aircraft configuration (Request configuration change)
Navigation
Communications
PNF - Pilot Non-Flying - responsible for:-
ECAM or checklist reading
Execution of required actions on PF request
Engine master switches, ENG FIRE pb (with consent of PF)
ECAM CLEAR
DO NOT CLEAR ECAM WITHOUT CROSS-CONFIRMATION OF BOTH PILOTS.
ABN/EMER PROC INITIATION
Procedures are initiated on pilot flying command.
No action will be taken (apart from audio warning cancel through MASTER WARN
light) until:-
The appropriate flight path is established and,
The aircraft is at least 400 feet above the runway, if a failure occurs during takeoff,
approach, or go-around.
(In some emergency cases, provided the appropriate flight path is established, the
pilot flying may initiate actions before this height).
NORMAL CHECKLIST
The Before Start, After Start, Parking and Securing Aircraft checklists will be
initiated by the CM1 and read by the CM2. Other Normal checklists are initiated
by the PF and read by the PNF.
The Before T.O. checks, below the line, and after landing checks, when requested,
should be actioned silently by the PNF as listed in the FCOM 3, SOPs. After
completion of the actions, the Table or QRH checklist should be read aloud
confirming the actions and the appropriate "........ checklist complete" announced by
the PNF.
The appropriate crew member shall respond after having checked the existing
configuration. When both pilots have to respond, "BOTH" is indicated.
The sign or indicates the specific VIRGIN ATLANTIC AIRWAYS requirements.

USE OF SUMMARIES
GENERAL
In case of an electrical emergency configuration, or a dual hydraulic failure:
The ECAM should be applied first.
This includes both the procedure, and the STATUS section.
Only after announcing "ECAM ACTIONS COMPLETED", should the PNF refer to the
corresponding QRH summary.
When the failure occurs, and after performing the ECAM actions, the PNF should
refer to the "CRUISE" portion of the summary, in order to determine the landing
distance coefficient.
Since normal landing distances are also given on this page, the PNF will be able to
compute the landing distance taking failure(s) into account, in order for the pilot to
decide whether to divert or not.
APPROACH PREPARATION
As always, approach preparation includes a review of the ECAM STATUS.
After reviewing the STATUS, the PNF should refer to the "CRUISE" portion of the
summary, to determine the VREF correction, and compute the VAPP.
A VREF table is provided in the summary, for failure cases leading to the loss of the
MCDU.
The LANDING and GO-AROUND portions of the summary should be used for the
approach briefing.
APPROACH
The APPR PROC actions should be performed by reading the APPROACH portion
of the summary.
The PNF should then review the ECAM STATUS, and check that all the APPR
PROC actions have been completed.

NB: This is a summary of the A340 AWOPS procedures to be used when Low Visibility
Operations are announced - for the full text please refer to the the FCOM and Ops Manual.
LOW VISIBILITY TAKE-OFF
1. Return Alternate required within 2 hours, Cat 1 or above.
2. Check H.I. edge and C/L lighting available.
3. Check threshold and R/W end lighting available.
4. Check T/O minima.
5. All relevant RVR's at or above minima.
6. Maximum crosswind 10 kts.
7. TOGA thrust to be used.
8. Verify ILS tuned on RAD NAV page (LS pb ON).
9. Check lights visible 9/5 for spacing 15 m / 30 m with RVR 125 m.
RTO
- Max autobrake.
- Maintain C/L.
- Call ATC.
- 900 m to go : C/L lights to alternating red/white.
- 300 m to go : C/L lights on red.
LANDING IN CAT 2/3
1. A/C autoland operative. Check Tech Log and QRH page.
2. Airfield equipment to Cat 2/3 standard (FCOM 2 + NOTAMS/ATIS).
3. (a) Both pilots to hold valid LVO certificates.
(b) Capt. as PF.
4. Confirm runway approved for autoland (No Cat 3B on L status runways).
5. Minima checked.
6. Two alternates required Cat 1 and above.
7. Extra fuel for holding/approach delays (normally > 45 mins).
8. Autoland wind limits (30 kt HWC, 10 kt TWC, 20 kt XWC).
9. Use config "full" (config 3 - one engine inoperative).
10. Approach briefing to include:
(a) Callouts on approach.
(b) Different PF/PNF duties.
(c) R/W and approach lighting expected.
(d) Visual cues at DH.
(e) Procedures for Rollout, R/W clearance and taxi.
(f ) GA procedure.
(g) Alternates and fuel state.
(h) Seat position.
(j) Aircraft external lighting.
(k) Autobrake 3 unless runway length or surface conditions warrant 4.

Initial Actions
PF
1) Disengage A/P
2) Looking through F/D, turn immediatly towards cleared heading then
initiate climb/descent if or as required.
PNF
1) Acknowledge
2) Select cleared heading/ALT and pull.
Subsequent actions
When appropriate, clean up as required. eg a standard G/A procedure can
be initiated by PF selecting TOGA then back to CL, then calling G/A Flaps.
CAUTIONS
1) Do not overbank. The necessity to immediately respond is ad
dressed by disconnecting A/P then smoothly applying bank, looking
through F/D.
2) Beware of high ROD beyond 1000 fpm in event of descending
breakout. Consider that ROD will already be approx 800 fpm, if
established on G/S.
3) Do not descend below cleared altitude.
Notes
1) It is recommended to initially target the following pitch altitudes
Climbing +12 deg.
Descending +2 deg. (or current attitude if stablised on G/S.)
Then adjust as required.
2) A/THR will maintain SPEED mode. brief accordingly if A/THR U/S.
3) If simultaneous TCAS RA, turn with ATC, climb/descend with TCAS.

This procdure applies when engines are running. If engines are not running, refer to the QRH
"ALL ENGINE FLAME OUT" (with or without fuel remaining) procedures, which has been
amended to include the ditching procedure, when the engines are running.
PREPARATION
ATC/TRANSPONDER (if available) . . . . . . . . . . .NOTIFY/AS RQRD
Notify ATC of the nature of the emergency encountered, and state intentions. If not in contact
with ATC, select transponder code A7700, or transmit the distress message on :
(VHF) 121.5 MHZ or (HF) 2182 KHZ or 8364 KHZ.

Turn the HDG selector knob and pull.
Adjust target SPD/MACH.

executed below boundary given in the figure.

COCKPIT DOOR FAULT
This procedure should be applied, if the Cockpit Door Locking System (CDLS) fails. This Failure
is indicated when the FAULT light on the center pedestal's CKPT DOOR panel comes on.
CKPT DOOR CONT PANEL . . . . . . . . . . . . . . . . . . . . . . . . . . . . CHECK
This panel is located on the overhead panel. It is used to identify the faulty CDLS item, and to
verify the status of the pressure sensors and the three electrical latches (referred to as strikes).
If two or more electrical latches (strikes) are faulty :
The cockpit door is not intrusion-proof.
If two pressure sensors are faulty :
Automatic latch release is unavailable, in case of cockpit decompression.
If no LED on the CKPT DOOR CONT panel is on :
The CDLS control unit is faulty; therefore, the cockpit door might unlock automatically. If
it does not, consider using the mechanical override system to unlock the door.
NOTE: In case of a DC BUS 2 fault, no FAULT indication appears on the center pedestal's CKPT
DOOR panel. The CDLS is not electrically-supplied, and is inoperative.

F / CTL RUDDER JAM/RUDDER PEDAL JAM
Rudder jam :
In case of a rudder jam, the rudder pedals are not jammed.
If the rudder is jameed in the 0 position :
The RUDDER FAULT ECAM warning will automatically be triggered. Refer to this
procedure.
If the rudder is jammed in a position other than 0 :
Unusual pedal movement or position, which corresponds to the computed rudder trim
value, may indicate a rudder jam.
AP can be used. However, automatic landing is not authorized.
When engaged, the autopilot introduces rudder trim, and so, pedal deflection.
When the rudder trim reaches 25, the indication is dashed on the RUDDER TRIM panel.
At autopilot disengagement, this pedal deflection remains.
Use the ECAM F/CTL page for a visual check of the rudder position.

stabilized heading sideslip, but ensure that all changes between rudder centralized
and steady heading sideslip are made smoothly. As speed is reduced, the Rudder
Travel Limit may gradually open to allow more rudder to be applied by the trim runaway.

FUEL JETTISON
WARNING
If the jettison procedure is applied following the "FUEL LEAK" or "ENG FAIL"
with tank damage procedure, the flight crew must comply with the following
requirements:
If a fuel leak is present, and weight reduction is essential for landing, once
close to the landing airfield, adopt the following procedure for fuel jettison.
It is possible to jettison in any convenient configuration and at any speed.
When practicable, the height should be sufficient to avoid contamination on
the ground (5,000 feet is considered adequate).
Do not jettison in a thunderstorm.
When jettisoning is performed, avoid flying into the jettisoned flow (which is
descending at about 500 ft/min).
Average weight reduction (by jettison only) is 1600 kg/min.

FUEL LOSS REDUCTION
Apply this procedure only if suggested by the FUEL LEAK procedure, and if
the flight crew needs to minimize the fuel loss. It covers two type of leak :
- A leak from wing : a manual Trim tank transfer (via the Center tank) and/or
manual Center tank transfer can be done, to symmetrically transfer the
fuel to all inner tanks, and avoid automatic transfer to only the least full
inner tank.
- A leak from the Center tank, or the Trim tank : a manual Trim and/or
Center tank transfer to the inner tanks can be done to save some Trim or
Center tank fuel.

Engines should be shut down sufficiently early to ensure fuel is shut off before the
nacelles impact, but sufficiently late to ensure adequate hydraulic supplies for the flight
controls. Hydraulic pressure for flight controls remains available for 30 seconds after
the shut down of engines 1 and 4.

A stall may be indicated by varying degrees of abnormal engine noises,
accompanied by flame from the engine exhaust and possibly from engine inlet in
severe cases, fluctuating parameters, sliggish or no thrust lever response, high EGT
and/or a rapid EGT rise when thrust lever is advanced. Engine stalls must be
reported for maintenance action.
Stall is detected by the FADEC :

HIGH ENGINE VIBRATION
The ECAM's VIB advisory mainly serves as a guideline to induce the crew to closely
monitor and crosscheck engine parameters.
VIB detection alone does not require engine shutdown.
NOTE : High engine vibrations may be accompanied by cockpit and cabin
smoke, and /or burning smells. This may be due only to compressor
blade tip contact with associated abradable seals.
High N1 vibrations are generally accompanied by perceivable airframe
vibrations.
High N2/N3 vibrations can exist without perceivable airframe vibrations.
IF NO ICING CONDITIONS EXIST :
ENG PARAMETERS . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . CHECK
Check engine parameters and especially EGT ; crosscheck with the other
engines.
Report in maintenance log.
If rapid increase above the advisory :

Flight conditions permitting reduce N1 to maintain the vibration level below
the advisory threshold.
NOTE : If the VIB indication does not decrease following thrust reduction,
this may indicate other engine problems. Apply the applicable
procedure.
IF NO ICING CONDITIONS EXIST :
An increase of N1 vibration in icing conditions, with or without engine anti-ice,
may be due to fan blades and /or spinner icing.

Increase thrust with the power setting compatible with the flight phase. The VIB
level will return to normal after ice shedding, despite a light increase during
acceleration.
Resume normal operation.
NOTE : If possible, shut down the engine after landing, for taxiing, when
vibrations above the advisory level have been experienced during
the flight.

ENG TAILPIPE FIRE
Internal engine fire may be encountered during engine start, or engine shutdown.
It may be seen by the ground crew, or the EGT may fail to decrease after the MASTER
switch is turned off.
CAUTION
External fire agents can cause severe corrosive damage and should,
therefore, only be considered after having applied the following procedure :

Note : Do not press the engine fire pushbutton, since this would cut off the
FADEC power supply, which would prevent motoring sequence.

There are two initial thrust reductions, De-rate 1 and De-rate 2.
De-rate 1 is a 10% reduction of full climb thrust and De-rate 2 is a 15%
reduction. At altitudes (above 30,000 ft) the de-rate climb is identical
to the max climb rating. For temperatures below ISA +10 there is a
progressive transition from De-rate climb to max climb between
26,000 ft and 30,000 ft for DCLB 1 and between 24,000 ft and 30,000
ft for DCLB2. With Ambient temps >ISA +10 the altitudes are lower.
De-rate climb can be selected on the PERF CLB Page, by inserting
'D1' or 'D2' in position (1R) on the CLB Page.
After analysis of the data the following is a guide to the use of De-rate
Climb.

Reduced Take-Off Thrust may be used at the Captain's discretion
and is referred to as Flexible Take-Off Thrust. Flexible Thrust may be
used when the actual take-off weight is lower than the RTOW, except
when prohibited by the exceptions listed below. Thrust reduction must
not exceed 40% of the full rated take-off thrust. Use of reduced thrust,
where possible, will assist engine condition and make a significant
contribution to reducing the rate of engine failure. The engines may be
opened up to full 'Go-around thrust at any time after V1 is attained
should the need arise.
Reduced Take-Off Thrust (Flexible Trust) must not be used when:
(a) The corrected 'FLEX' temperature (CT) is higher than ISA
+60C or lower than 'TREF' (Reference Temperature) or lower
than ambient temperature.
(b) The runway is affected by snow, slush, standing water or ice, or
suspected of having a marginal braking capability.
(c) Significant windshear conditions are anticipated.
(d) Operating with landing gear extended.
(e) One or more brake(s) inoperative.
(f) One or more thrust reversers are inoperative.
(g) Ground spoilers inoperative.
(h) One tachometer inoperative.
(i) Generalised or non specific RTOW tables are used.
A calculated RTOW of more than maximum structural can be used for
Flexible Thrust calculations.
