Sir/Madam:

I am respectfully submitting this letter to your good Office to formally
request for the waiver and/or cancellation of my phone bill with final warning
for disconnection per Account No. 0713880272 in the amount of PhP
1,308.54 as received via mobile phone on December 23, 2013. Said
amount pertains to the alleged internet fees from the period August 14,
2013 to September 7, 2013.

It is within this Offices knowledge and record that I have availed of the
Samsung Galaxy S4 plan with unlimited internet services among others,
aside from my current SMART 599 mobile plan. Bearing in mind the S4s
WiFi sharing features, I requested assistance from your Offices Service
Personnel to facilitate the transfer of my previous 599 mobile plan SIM card
to my mobile Android tablet so that I can maximize the use of the said WiFi
features with said tablet device. No further instruction or advice was given to
me by your Service Personnel on how to properly utilize the WiFi sharing
features of said devices without incurring any additional internet charges.

In so doing, I relied with utmost good faith that I was not incurring any
additional charges in sharing my S4s WiFi with my Android tablet within the
period that I was alleged to have incurred the above-stated internet fees. It
is for this reason that I was deeply surprised that sometime on September
2013, I was given notice that I had internet charges regarding my Smart
599 mobile plan. Under protest, I paid the amount that I believe should be
due to me at that time and requested this Office to reconsider the imposition
of said internet fees. However, I was later informed that my request for
reconsideration was not approved.

From the foregoing, I am now humbly requesting your good Office to waive
and/or cancel the imposition of said internet fees in the spirit of fairness and
human consideration. I have always been a fair and prompt subscriber of
your services and I do believe that granting this request is only but
reasonable and just under the circumstances.
