    AFFIDAVIT OF SELF-ADJUDICATION BY SOLE HEIR


    I, MIRA LOADA ALFANTE, Filipino, of legal age, widow, with residence
    address at Block 1, Lot 2, Narra Street Woodsgate Subdivision, Camp 7, Baguio
    City, Philippines, under oath, depose and say:
    1. That I am the surviving wife and only sole heir of the late ROBERTO D.
    ALFANTE there being no other ascendants, descendants or collateral
    relatives, surviving, who died intestate in the Baguio City last June 27,
    2011;
    2. That at the time of his death, he left the followings, to wit;
    a. Parcel of land and all existing improvements thereon, covered by
    Transfer Certificate of Title No. T-83124, issued by Register of Deeds for
    City of Baguio, with an area of TWO HUNDRED FORTY FIVE (245) SQUARE
    METERS, more or less, located in the City of Baguio, Philippines,
    particularly described as follows;

    A parcel of land ( Lot 2, Blk. 1, Pcs-131102-000284, being a portion of
    consolidated Lots A-3-A (LRC), Psd-244362, Lot 1, (LRC) Psd-70552, Lots 4-A4-H & 4-C (LRC) Psd-80889, Lot 42-A, 42-B, (LRC) Psd-12541, Lot 4& 2 (LRC)
    Pcs-3201, Lot 3-C-3, Psd-CAR-000031; Lot 8, (LRC) Psd-30671, Lots 3-C-1-A &
    3-C-1-B, Psd-CAR-000435, Lot 3-C, Psd-1-008349, Lot 3-C-4, Psd-CAR-000051
    & Lot 3-C-2-C, Psd-CAR-000434), situated at Res. Sec. J, City of Baguio,
    Island of Luzon. Bounded on the SE., along line 1-2 by Lot 3, Blk. 1 of the
    conso-subd. plan; on the SW., along line 2-3 by Lot A-4-D, Psd-1-007756; on
    the NW., along line 3-4 by Lot 1, Blk. 1; on the NE., along line 4-1 by Road
    Lot 3, all of the conso-subd. plan. Beginning at a point marked 1 on plan
    being N. 22 deg. 12W., 1233.98 m. from triangulation sta. BOYD, B.T.S.,
    thence
    S. 64 deg. 46W., 18.93 m. to point 2;
    thence
    N. 25 deg. 15W., 12.96 m. to point 3;
    thence
    N. 64 deg. 46E., 18.93 m. to point 4;
    thence
    S. 25 deg. 15E., 12.95 m. to point of beginning;
    containing an area of TWO HUNDRED FORTY FIVE (245) SQUARE METERS,
    more or less. All points referred to are indicated on the plan and are
    marked on the ground by PS cycl. Conc. mons. 15x40 cms. Bearings true;
    date of original; survey on ; date of consolidation-subd. survey on June
    30, 1994 executed byb Engr. Rodolfo S. Maningo Sr. and was approved on
    April 11, 1996.

    b. Dollar Savings Account No. 001830071821, Banco de Oro SM Branch,


    Baguio City;
    c. Savings Account No. 1830318630, Banco de Oro SM Branch, Baguio
    City;
    3. That the deceased left no debts or obligation;
    4. That the above-.mentioned deceased died intestate, without leaving a
    will;
    5. That pursuant to Rule 74, Sec. 1 of the Rules of Court, I hereby adjudicate
    unto myself the above-described parcel of land including all
    improvements built thereon and Accounts by means of this affidavit and
    hereby files the same with the Register of Deeds for the City of Baguio,
    Banco de Oro SM Branch, Baguio City and other concern agencies with
    the request that said adjudication be made effective without judicial
    proceedings as prescribed by the aforementioned Rules of Court;
    6. I am executing this instrument, to attest to the foregoing facts and
    circumstances and for all legal intents and purposes it may serve.

    IN WITNESS WHEREOF, I have set my hand this 8th day of May, 2013 in
    the City of Baguio, Philippines.
