OPERATIONS CIRCULAR NO. 2 OF 2001

Subject: Operational Training and Procedures : Pilots

In a recent fatal aircraft accident, the Court of Inquiry investigating the said
accident has warranted review of Pilot Training with special emphasis on
discipline in the air, Cockpit Resource Management (CRM), adherence to
Standard Operating Procedures (SOPs) and procedures such as recovery from
approach to stall and clean stall.

2. In light of above, all Operators are required to implement the following with
immediate effect :-

(i) Approach to Stall and Recovery for clean and landing


configuration be included as an exercise in Simulator
Sessions performae of at least of the total sessions for
each stage viz. Initial, Conversion, and Refresher Training
(e.g. Total sessions of the stage are 8. At least 4 sessions
must show approach to stall and recovery, clean and
approach configuration as an exercise).

(ii) Cover CRM in Initial, Conversion, and Refresher Training


and include the subject in their exercises, on the respective
performae, as Recap of CRM. This inclusion should exist in
at least 1/3 rd of the simulator sessions performae or during
class-rooms programs if simulator is not applicable for a
phase. Check of CRM be included in the LOFT Training and
the format for LR, IR, Route Checks as well as CA40 Tests.

(iii) Exercises on Wind Shear/ Microbursts/ CFIT also to be


covered as in (ii) above.

(iv) Review of the approach procedure be done before Top of


Descent and the review must also include the specific
actions and phraseology associated with normal go-
around/wind shear go-around/approach to stall and
recovery.

(v) 360 degree turns in the Final Approach phase be prohibited


and a missed approach be executed whenever the aircraft is
not stabilised during this phase. This is to be specifically
emphasised in the Operations and Training Manuals.

(vi) Increase their surveillance (through CVR monitoring in line


operations) for adherence to SOPs and high standard of
CRM and call the crew for counseling/ corrective measures
as required.

All Operators are required to make suitable amendments to the performae


in use and Operations and Training Manuals and forward the same to this office
within a period of 30 days from the date of issue of this Operations Circular.
