Term Definition

Request Type Choose Appropriate Request type to mention whether its a request for new access or
request to modify / delete existing access. E.g. New / Update/ Renew/Add/Delete

Choose Appropriate Employee type:

Employee Type Regular Employee of TCC


Trainee under training
Contractor Working with TCC
Client /Consultant
Guest of TCC
Other (If Any Other Mention the type)

Select the Required Service and Service type.

E.g. 1.: Requesting A New Email Account


Select Service as Email Account and Service Type as New
Services Requested

E.g. 2: Requesting for renewal of existing software license Select Service as


SOFTWARE and service type as Renew

List of Services: Network ID/Email Account/Distribution Group / Internet


Access/Access to Email Groups/Software License/Shared Folder Access etc.

Choose the appropriate application to which Access is required. Maximo /
