    OUTLINE OF Ph.D. PROGRAM (Plan of Study)


    DIRECTIONS
    Student completes Parts I, II, III, and submits the form to the Department Chair or Graduate Director for
    signature. Department forwards completed and signed form to the Graduate School.
    A minimum of 48 hours of graduate coursework must be listed in Part III, including transfer credit if
    applicable. A request for transfer of graduate credit must be submitted to the Graduate School.

    A minimum 24 hours of continuous dissertation research (699) is required.
