Dear Mr. Rivera;


Greetings!

At the instance of AGRI-SERVICES PLUS MULTI PURPOSE COOPERATIVE of


Kapitan Pepe, Cabanatuan City, who secured the services of this office, we are writing to demand
payment of the certain amount you have taken for your personal use by maneuvering and tampering
the receipts and other documents of the cooperative regarding the funds of this cooperative in the
amount of SIXTY SEVEN THOUSAND ONE HUNDRED NINETY NINE PESOS (P 67, 199.00)
(Annex “A”). According to them, A Fact Finding Committee conducted the investigation of the said
maneuvering and mishandling of funds and requested you to present your side with the assistance of a
counsel as per attached written invitation to you but you did not respond and until this date you failed
to explain and pay the funds to the Agri- Services Plus Multipurpose Cooperative.

At this opportunity, the undersigned hereby gives you a period of 3 days from receipt of this
demand letter to settle the said amount P 67, 199.00. Otherwise, we will be constrained to file
appropriate legal action (civil and criminal) against you in the proper forum where you will burdened
more by a penalty of 25% of the principal amount if it reaches the court.

In fine, I count on your favorable response in this regard.

At the instance of AGRI-SERVICES PLUS MULTI PURPOSE COOPERATIVE of


Kapitan Pepe, Cabanatuan City, who secured the services of this office, we are writing to demand
payment of the certain amount you have taken for your personal use by misappropriating the funds of
this cooperative in the amount of THIRTY ONE THOUSAND ONE HUNDRED SEVENTY FIVE
PESOS (P 31, 175.00). According to them, despite their repeated demands and follow ups to pay the
said amount you failed to pay and until this date you failed to pay the funds to the Agri- Services Plus
Multipurpose Cooperative.

At this opportunity, the undersigned hereby gives you a period of 3 days from receipt of this
demand letter to settle the said amount P 31, 175.00. Otherwise, we will be constrained to file
appropriate legal action (civil and criminal) against you in the proper forum where you will burdened
more by a penalty of 25% of the principal amount if it reaches the court.
