MFO Description of Accomplishment

Presentation of Outline Submitted to the subject coordinator within five (5) working
days from the beginning of each moth a weekly plan for each
subject handled that will commence at the start of the school
year.
Class Attendance Submitted to the principal on or before the first Friday of May
and November each year an evidence showing that the faculty
member had conducted classes in each subject for at least 100
hours in a school year for every subject handled.
 Punctuality Submitted to the Principal an evidence on or before the tenth
working day of April and November of each year showing that
the faculty member is able to meet 100% of classes within 5
minutes after the scheduled time had began and dismisses class
on time all throughout the semester.
Student Evaluation Submitted to the principal an evidence on or before the tenth
working day of April and November or each year showing that
the faculty member is able to administer at least ten (10)
quizzes for the whole semester to each of his classes.
Student Performance Returned students’ test papers, both periodic and quizzes, as
Feedback and evidence by the students’ signatures on the acknowledgement
Transparency sheet within ten (10) working days after the day of
examination.
Revision of Syllabi/
Subject Guides

Grade Submission Submitted to the principal, through the Class Adviser, reports
of rating 10 working days from the last day of every periodical
test all final grades of all students in all subjects handled.
Syllabus/ Subject Guide Was able to cover all topics which were supposed to be

Other Academic Tasks Was able to perform during “Light a Tree” program sponsored
(Accreditation, by the City Government.

Reflections:

Being a teacher is not just a job, it is a responsibility and obligation. As a teacher we

should treat our students as our own children. We should give our best to teach them
lessons not only lessons came from subject we are teaching but also in real life we have
experienced.
