MAXILLARY INJECTIONS
-
- MIDDLE SUPERIOR ALVEOLAR BLOCK
- Anesthetizes maxillary premolar, mesiobuccal root of max 1st molar (occurs
in 28% of cases)
- Symptoms include tingling and numbness of the upper lip
- Pulp with buccal periodontium and gingiva
- Injection site: height of the max. mucobuccal fold superior to the apex of the
second premolar
-
- POSTERIOR SUPERIOR ALVEOLAR BLOCK
- Anesthetizes maxillary molars
- Symptoms include inadvertent anesthesia of mandibular nerve and structures
associated with it due to location (lateral to posterior superior alveolar nerve)
- Pulp with the buccal periodontium and gingiva.
- Injection site: height of the mucobuccal fold, above the apex of the 2nd molar,
and distal to the zygomatic process.

-
- INFRAORBITAL BLOCK
- Anesthetizes maxillary anteriors and premolars, and mesiobuccal root of the
1st molar.
- Symptoms include tingling and numbness of the upper lip, part of the cheek
(medial), side of the nose, and lower eyelid
- Pulp with labial/buccal periodontium and gingiva.
- Injection site: height of the maxillary mucobuccal fold superior to the max. first
molar.
-
- NASOPALATINE BLOCK
- Anesthetizes the anterior portion of the hard palate (and the associated
palatal periodontium and gingiva of the maxillary anterior teeth.
- Symptoms include: Numbness of the anterior hard palate.
- Injection site: height of the maxillary mucobuccal fold superior to the max. first
molar.
- Pulp with labial/buccal periodontium and gingiva.

MANDIBULAR INJECTIONS
- INCISIVE BLOCK
- Anesthetizes mandibular anterior teeth and premolars.
- Symptoms include numbness and/or tingling of the lower lip.
- Injection site: anterior to the mental foramen, inferior to the apices of the
mandibular premolars, with the needle facing posteriorly.
- Pulp with the facial periodontium and gingiva
-
- INFERIOR ALVEOLAR BLOCK
- Anesthetizes an entire half of the mandible including anterior and posterior
teeth & their associated periodontium and gingiva, excluding the area
anesthetized by the buccal block. Includes the tongue and labial lower lip.
- Symptoms include numbness or tingling of half of the tongue and floor of the
mouth, as well as half of the lower lip.
- Injection site: into pterygomandibular space, until the mandibular ramus is
contacted
LOCAL ANESTHESIA STUDY NOTE - Alma Fuentes
- Pulp with the facial/buccal & lingual periodontium and gingiva (excluding that
of the molar teeth), soft tissue.
-
- BUCCAL BLOCK
- Anesthetizes the buccal periodontium and gingiva of the mandibular molars.
- Symptoms include: absence of pain during dental procedures.
- Injection site: into the buccal mucosa that is the most distal and buccal to the
distal of the mandibular 3rd molar.
-
- GOW GATES BLOCK
- Anesthetizes the mandibular teeth to the midline and associated lingual
periodontium and gingiva, associated facial periodontium and gingiva of
mandibular anterior teeth and premolars to the midline within one mandibular
quadrant and possibly buccal periodontium and gingiva of the mandibular
molars
- Symptoms: numbness of the lower lip, buccal and temporal regions, as well
as delayed action of the LA (it takes a while to take effect.)
- Injection site: anteromedial border of the neck of the mandibular condyle,
inferior to the pterygoid muscle and lateral to the medial pterygoid muscle and
medial to the mandibular ramus. On the buccal mucosa, on the medial
surface of the mandibular ramus, distal to the height of the mesiolingual cusp
of the second molar (7).

-
- VAZIRANI-AKINOSI BLOCK (CLOSED-MOUTH)
- Anesthetizes mandibular teeth to the midline and associated lingual
periodontium and gingiva, associated facial periodontium and gingiva of the
mandibular anterior teeth, and premolars to the midline within 1 mandibular
quadrant and possibly the buccal periodontium and gingiva of the mandibular
molar teeth
- Symptoms: increased ability to open mouth in client w/ trismus, numbness of
the above tissues. Transient facial paralysis may occur.
- Injection site: buccal mucosa between the medial surface of the mandibular
ramus and the maxillary tuberosity w syringe barrel parallel to the occlusal
plane.
