Dear Ms. Q,

We hereby notify you of the Company’s decision on the official investigation it


conducted in connection with the incident stated in the Notice to Explain dated
21 May 2010.

BRIEF NARRATIVE OF THE EVENTS

On 21 May 2019, you were given a Notice to Explain the alleged serious
violation you committed which arose from a complaint by one pertaining to
the installation of an architectural design, your son and another accomplice
without informing the Corporation and without proper purchase order
procedures imposed by Corporation.

On 27 May 2010, an administrative hearing was conducted where you


submitted your written explanation, which was duly received and noted, and
where you proceeded to explain your side of the story.

Subsequently after the hearing on 27 May 2010, further inquiries were made to
ascertain the veracity of the claims you made during the hearing.

ISSUE

1. Whether you violated the Company’s rules and regulation.

FINDINGS

In the administrative hearing conducted on 27 May 2010, you admitted that:

DECISION

In view of the foregoing and after careful review of all the evidence submitted,
Management finds your continued employment inimical to the interest of the
company for Violation of the Company’s Rules and Regulation.

Thus, Management has decided to TERMINATE, as it is hereby terminating,


your employment effective upon receipt of this notice. Your letter of
appointment dated is hereby irrevocably revoked.
