Nursing Diagnosis Expected Outcomes
After series of nursing interventions, the client will be able to: Use non pharmacological nausea control techniques such as relaxation, guided imagery, music therapy, distraction, or deep breathing exercises. Patient reports decreased severity of Nausea related to pregnancy nausea Avoid foods and smells that trigger nausea Be educated and assisted about oral hygiene Learn to maintain odor-free environment Be educated and assisted in managing and reducing severity of nausea related to pregnancy Risk for Altered Nutrition: Less than Report understanding of significance body requirements r/t constant nausea as of nutrition to healing process and e/b px not being able to keep her food general health down Show no signs of malnutrition Take adequate amount of nutrients
