AFFIDAVIT
I, EVELYN M. TINGCHUY, of legal age, widow, Filipino and a resident of
Purok 2, Linabo, Malaybalay City, Bukidnon after having been duly sworn to an
oath in accordance with law, hereby depose and say:

1. That I am an owner of a parcel of residential land located at Barangay


Linabo, Malaybalay City, Bukidnon under Transfer Certificate of Title
No. T-117139, more particularly described as follows:

“A parcel of land, Lot 3727, Pls-800 (Lot 13913-B, Csd-101312-007963-


D), situated at Barangay Linabo, Municipality of Malaybalay, Province of Bukidnon,
Island of Mindanao.
Bounded on the NW.,NE., and SE., along lines 1-2-3-4 by Road; and on
the SW.,along line 4-1 by Lot 1393-A, Csd-101312-007963-D), containing an area
of FIVE THIRTY FOUR SQUARE METERS (534 sq. m.)”

2. That the above-described property is my inheritance from my late


parents who died sometime in 1970’s;

3. That I am attesting to the truth and veracity of the foregoing facts;

4. That I am voluntarily executing this affidavit for the purpose of attesting


that the above-described property is my inheritance as a requirement for
loaning the same and for any legal purpose/s it may serve best;

IN WITNESS WHEREOF, I have hereunto set my hand this 23rd day of April,
2018 at Malaybalay City, Bukidnon.
