JOINT AFFIDAVIT OF TWO DISINTERESTED PERSONS

WE, OFELIA TRINIDAD ZUELA, 74 years old, born on October 13, 1949, Filipino, and
presently residing at Barangay Daungan, Mauban Quezon Province and REYNALDO
BANAGAN PASATIEMPO, 74 years old, born on October 30, 1949, Filipino, presently residing
at Barangay Daungan, Mauban Quezon Province, after being duly sworn to in accordance with
law, depose and state that:

1. We personally know ESTRELLA BETLRAN MANCENIDO (deceased) being


acquaintance for a long period of time;

2. That we declare, confirm and affirm that she died on March 6, 2001 due to Pulmonary
Cardiac Arrest at Barangay Daungan, Mauban Quezon.

3. That she was buried at the catholic cemetery located at Brgy. Mabato, Mauban Quezon
Province.

4. That we have a personal knowledge of the foregoing facts because we were neighbors
and close family friend;

5. We are not related by affinity or consanguinity to the above-mentioned person; and

6. We are executing this affidavit to confirm and affirm the foregoing facts and for whatever
legal purpose it may serve.
