Cover Sheet – Contains name of the business, names of principal(s), address and phone number of
business, e-mail and web-site addresses, name and address of person who wrote the plan.

Statement of Purpose and Executive Summary – An operating and policy guide for your business. If
funds are needed, state how will they be used and what effect these funds will have on the business.
