1. GENERAL
This requisition covers the minimum requirement for Mechanical Construction Work &
Commissioning Work (hereinafter called the Work) to be performed and completed by Subcontractor
for Project as specified in cover sheet.
The Subcontractor shall perform and complete the work in strict accordance with the requirements
contained in this Requisition.
The Subcontractor shall be responsible for all work items required to perform and complete the Work
in accordance with the requirements of this Requisition and the related documents described in
Reference List Documents and Drawing (Attachment-#2 ). The equipment that will Commission
shall refer to Equipment List of Mechanical (BOQ) Attachment-#3.
The Subcontractor shall provide materials, labor, facilities, equipment, consumable, testing and
inspections and all other items and services necessary to complete the Work at his own cost and
responsibility.
The following items shall be included in the Subcontractor’s scope :
Detail of Scope of Direct Work shall refer to Attachment-#1 Scope of work & supply.
Subcontractor shall provide some indirect work required to perform the Work at his own
cost and expenses.
The indirect work shall not be limited to items specified in Attachment-#1 Scope of work
& supply and Subcontractor shall verify any indirect work required.
2.3. Material
The material supplied by Subcontractor shall fulfill the requirement, specification, and
quality, well documented and traceable. If the material does not meet the requirement,

In the ADHI’s request, the Subcontractor shall provide his manpower on Pre-commissioning
and Commissioning Stage. The number, qualification and assistance duration of this stage
shall be as ADHI’s request. The man power shall be completed with needed tools,
apparatus,
etc.
3. REFERENCES DOCUMENTS
The contract documents (ADHI with PT. PLN) shall form a part of the requirements of this
Requisition.
4. CONFLICT
In the event of conflict amongst the Contract Documents, the matter in conflict shall be submitted
immediately by Subcontractor to ADHI KARYA for determination and such determination by ADHI
KARYA shall be final, provided, however that the more stringent requirements shall govern

Subcontractor shall provide all of direct manpower including unskilled labor, skilled labor up to
foreman that are needed to perform and complete the Work.

The manpower mobilized by Subcontractor shall have sufficient skill in accordance with his
specialization or job description. If in the opinion of ADHI, any manpower is deemed
unacceptable or has no skill in accordance with his job description, Subcontractor shall replace
him with acceptable and capable manpower immediately.
Subcontractor shall have dedicated superintendent and if necessary, they shall advise to ADHI’s
superintendent for clarification or meeting with client.
Subcontractor shall provide all of safety accessories, necessities, insurance and others for his
manpower.
Subcontractor shall complete his manpower with necessary certificate related to his job
description and in accordance with the law or regulation.
6. CONSTRUCTION EQUIPMENT
For Construction Equipment that are not provided by ADHI, Subcontractor shall provide some
construction equipment that are needed for execution and completion of the Work. Subcontractor
shall propose the construction equipment for ADHI review and approval.
Subcontractor shall provide the construction equipment including its accessories, operator,
consumable, spare parts, maintenance, permission and others related to the operation of the
construction equipment at his own cost and expenses.
The Subcontractor shall include a procedure related to the construction equipment inspection,

operation and maintenance in his submitted construction safety program. It shall be consistent
with the manufacturers' recommendation and ADHI requirements and be incorporated after
Subcontractor's proposal reviewed by ADHI.
Operator of all cranes and other heavy equipment shall be qualified to operate the assigned
equipment in accordance with government regulation. Subcontractor shall provide some licenses
required for operator at his own cost and expenses.
The construction equipment to be used will be subject to inspection before works starts under the
contract. If in the opinion of ADHI, any of equipment is found unsuitable and it would interfere
with desire results, the Subcontractor shall replace them with suitable equipment immediately.
7. SCHEDULE
The Subcontractor shall perform and complete the Work in accordance with “Construction
Master Schedule“.
Based on the Construction Master Schedule and construction milestone required, the
Subcontractor shall prepare and submit a Construction Detail Schedule.
If in ADHI's evaluation the Subcontractor's work performance does not meet the schedule, the
Subcontractor shall make necessary efforts to recover the progress schedule in Subcontractor’s
cost.
The Subcontractor shall be responsible during the execution of the work to ensure the specified
quality of the work.
The Subcontractor shall perform all testing and inspections to conform the specified quality of the
work in accordance with Project Specification, drawings and standard code.

The testing and inspection shall be conducted with or without ADHI’s witness (or the Client)
depending on its importance in accordance with ADHI’s Field Quality Control Procedure.
Details of the testing and inspection procedures shall be established by the Subcontractor and
submitted to ADHI for approval prior to start the Work.
Result of the testing and inspection shall be recorded by the Subcontractor and reported to ADHI
(and Client).
The Subcontractor shall perform field-testing and inspection in accordance with the specifications
and drawings for the items directed by ADHI. The Subcontractor shall be responsible to provide
all apparatus, tools and materials for required testing and inspection.
Cost and expenses related to testing, inspection, recording and reporting including apparatus,
tools and materials shall be Subcontractor's responsibility.
The Subcontractor shall propose Safety Program that will be applied in the performance of the
Work for ADHI review and approval in accordance with ADHI’s SHE Project Plan. All of the
Subcontractor’s staff and labor shall aware of the Safety Program. The Subcontractor shall
provide some equipment, tools and apparatus needed to support the Safety Program.
The Subcontractor shall pay attention to the security of his properties (material, construction
equipment, tools, etc.). The Subcontractor shall assign his staff to in charge fully in security and
make coordination with ADHI’s security. Any loss of the Subcontractor‘s properties is the
Subcontractor’s responsibility.
The Subcontractor shall protect his manpower, material including received ADHI supplied
material, construction equipment, tools, and other properties from weather, damage, accident,
loss, thievery, etc. by providing necessary protection.
All cost and expenses related to the safety and security program shall be the Subcontractor’s
responsibility.
The Subcontractor shall prepare and submit the execution plans and procedures for the work in
accordance with the job specification, local law and regulation, HSE aspect and ADHI’s instructions to
obtain ADHI’s approval prior to commence the work.
Choice of method is not the Subcontractor own decision matter but ADHI's policy, which will
incorporate Client's opinion.
Subcontractor shall use Indonesian products as much as possible. If there is any material or
equipment that has to be imported, the Subcontractor shall obtain prior approval from ADHI before
expediting the product.
Subcontractor is also required to use local labor force to the maximum extent possible.
The surplus / scrap material shall be returned to ADHI’s warehouse or storage at the time requested
by ADHI in accordance with ADHI’s surplus material handover procedure.
Construction garbage shall be disposed by the Subcontractor to the area designated by ADHI.
All cost related to the return surplus material and garbage disposal shall be the Subcontractor’s
responsibility.
During the execution of the work, ADHI and other Subcontractors will be working simultaneously at the
job site. In such event the Subcontractor shall keep a harmonious relationship with other and shall
comply with ADHI’s instructions or arrangements that will be given to the Subcontractor to minimize
any delay of the work as well as harmonious relationship with community. The Subcontractor shall not
claim any additional cost that incurs as a result of such instructions or arrangements.
The Subcontractor shall make whatever arrangements that are necessary to become familiar
with all existing and anticipated conditions and matters that might in any way affect the cost or
the performance of the work.
Any failure to duly investigate the job site, existing facilities or the foregoing conditions shall not
relieve the Subcontractor from his responsibility of successfully performing the work.
