    OATH OF UNDERTAKING

    I, ______________________________, of legal age, single/married, Filipino Citizen presently on


    Detached Service at the National Fire Training Institute, after having duly sworn to an oath in accordance
    with law, hereby depose and state that:

    1. I am a trainee of Fire Arson Investigation and Inspection Course Class Number __________;

    2. I am aware that trainings requirements are embodied in this training program set by the
    National Fire Training Institute in accordance with the Master Training Program of the Institute
    duly approved by the Philippine Public Safety College;

    3. In the course of the training skills, there are unforeseen events that may come along in
    furtherance of the training for it involves the following performances that require physical exertion
    as part of the training package, to wit;

    3.1 Agility/Physical Fitness Test;


    3.2 Reveille and physical conditioning.
    3.3 Structural Plans Evaluation and Review
    3.4 Mechanical Plans Evaluation and Review
    3.5 Electrical Plans Evaluation and Review

    4. The students/trainees are required to undergo Educational Tour training as a part of the
    curriculum before graduation;

    5. Despite of the Physical Exertion required, I hereby submits myself to undergo training as part of
    the curriculum of the Institute without mental reservations ;

    6. I was not forced nor coerced to undergo the above-mentioned training;

    7. That I will be held liable in case of any violations may I be done of committed during the entire
    duration of the course in accordance with the NFT Training Guide.

    8. That I attest to the veracity and truthfulness of the foregoing statements;
