    Rule II Sec. 6a Application (Notarized)

    Rule II Sec. 6b Supporting Documents


    1. Registration of Business from DTI or Securities and Exchange Commission;
    2. Barangay Business Clearance;
    3. Building Permit from the LGU concerned or lease agreement for building;
    4. Mayor's/ Business Permit;
    5. Fire Safety Inspection Certificate;
    6. Supply contract/agreement/certificate from supplier; and
    7. List of Vehicles (model, make, plate number and capacity)
    Rule II Sec. 7 Reportorial Requirements
    A. Annual Reports
    1. List of facilities and capacities, including transport vehicles with plate
    numbers;
    2. List of suppliers and volumes; and
    3. Inventory of LPG cylinders
    B. Monthly Reports
    1. Supply;
    2. Demand/sales/consumption/thru put or refilling volume; and
    3. Inventory;
    C. Special Reports
