HAIR&MAKE UP SERVICE CONTRACT

MAGIC GLAM BYJRM

This make up SERVICE CONTRACT is made and entered into as of (date)


__________ between (Client) _________________________and (Makeup Artist)
_________________________ Upon signing this contract, it is legally binding and both
Makeup Artist and Client as follows;

1.Makeup Artist Obligation to provide the service hereunder is subject to change and/or
unavailability of the artist as a result of sickness, accidents, acts of God and other
reason beyond the Makeup Artist’s control.

2.Client agrees to notify Makeup Artist of any allergies he/she have toward any
cosmetics and/or chemicals. Should a reaction occur, requiring medical attention or not.
It is agreed that the Makeup Artist is not legally liable or responsible for injury and/or
ailment of any kind.

3. POLICY BOOKINGS to secured a date a client required a deposit at least


20% or 40%. This deposit is not refundable and non- transferable. This deposit will be
put toward the client’s said event. The remaining balance will be due on dates and said
event. Accepting forms of payment is Palawan Express, Cebuana Lhuillier.

4.PAYMENTS are to be paid before/after Makeup Service. No personal cheques or


credit card accepted.

a. Fee arrange for Makeup service;

●Additional Head ₱_____________(Per Head).

5. HEAD COUNTS will be paid also regardless of the number of heads presented.
If clients add another head it will be counted also but the price is in said

6. POSTPONES/ CANCELLATION If the client postponed or cancelled the


event due to natural resources or other causes once the contract has been signed on,
50% of the total contract price will be charge.
7.TRAVEL FEES is not handled by Makeup Artist, it should be the client’s handle
depends on location and additional of transportation to get on location as said.

8.AIRFAIR/ACCOMODATION all cost are paid by the client this is


reasonable for said event and location.

⃰ADDITIONAL NOTES;
●The client’s must be ready for hair and makeup.
●Food will be shouldered by the client.
●Payments are not refundable.
●Transportation is not handled by Makeup artist.
