Oral Health Survey Report: Patient Name Age Gender Decayed (D) Missing (M) Filled (F) Individual DMFT Index

This oral health survey report summarizes the results of examining 5 family members to assess their oral health status and needs. The DMFT index was calculated, finding an average of 3.6, indicating a moderate oral health status. While some teeth were decayed or missing, most teeth remained intact. Regular dental visits were not common. The survey could be improved by examining more community members, but focusing on family was preferable due to the pandemic. Overall, the oral health of the subjects was found to be moderate based on the results.

ORAL HEALTH SURVEY REPORT

1. Statement of the purpose of the survey

 The aim of this survey is to know and assess the current oral health status of
some of my family members and its future needs for oral health care.

2. Materials and Methods

 The target area is in Brgy. 669 Zone 72, Padre Faura St. Ermita, Manila. It is an
urban community where informal settlers are rampant.
 The method that I used was to directly ask them questions that is in the OD form
and check their tooth by using a mouth mirror and explorer.

3. Results
 Here below is the tabulated data of DMFT result of some my family members.
The data shows that among the 5 subjects examined, 6 are decayed, 9 are
missing, and 3 are filled. The gathered data revealed that there is 3.6% DMFT
rate which is moderate.

4. Discussion and Conclusions

 After I gather the data and checking their oral cavity most of them don’t
regularly visit the dentist for a dental checkup. Most of them their last treatment
was oral prophylaxis which I’ve done inside the CEU clinic infirmary during my
Clinic 1 periodontic clinical requirement. The reason why they don’t go to the
dentist is because they don’t feel any discomfort or pain
 In terms of the short comings in this survey. It is better to actually make a survey
to the community to really know the status of their oral health. But since there is
still virus it is best to just stick with your family and from there you can help
them by giving small treatments like oral prophylaxis and others.

5. Summary or Abstract
 Based on the results of the gathered data. The oral health status of my 5 family
members is moderate. There are some tooth that are decayed and filled, but
majority of their tooth are still intact.
