A. 5PM Habit Kontra COVID-19 in Surigao del Sur District Jail Tandag
On 04 March 2021, JO1 Mardenver Balan, under the supervision of SJO4
Rosalito A Reyna, conducted the 5PM Habit Kontra COVID-19 in Surigao del Sur District
Jail Tandag City through disinfection of all contact surfaces of the said jail facility.

B. Bida Ang May Disiplina: Solusyon sa COVID-19 Regional Launching in

Bayugan City Jail
On 04 March 2021, eight (8) POL, two (2) response team member, three (3)
JO1/trainees and six (6) PDL witnessed the launching of Bida Ang May Disiplina:
Solusyon sa COVID-19 Regional Launching via YouTube platform in Bayugan City
Jail.

C. Deployment of Vaccination Plan for BFP and BJMP Visayas and Mindanao
Cluster
On 02 March 2021, eighty-seven (87) BJMP Personnel led by J/SSUPT
JOLLY C TAGUIAM JR, Regional Director of BJMP Caraga and SJO4 Richard C
Recososa, RN, Chief, Health Service Division, attended and participated the
Online Deployment of Vaccination Plan for BFP and BJMP Visayas and Mindanao
Cluster through Townhall Discussion via Zoom Online Platform. Said program was
rendered by the Department of Interior and Local Government (DILG) and the
Department of Health (DOH).

D. VIMS-IR (Vaccination Information Management System) Training rendered
by the DICT
On 03 March 2021, ten (10) BJMP Caraga Health Personnel led by SJO4
Richard C Recososa, RN, Chief, Health Service Division attended and participated
the Training on the usage of VIMS-IR (Vaccination Information Management
System) program, conducted by the Department of Information and
Communication Technology. This program will serve as the centralized online
database system for the National COVID-19 Immunization Program.

E. Collection and Submission of Specimen for Gen-ex and Coordination for
the conduct of PICT and FBS Test
On 01 March 2021, JO1 Jennifer S Jamero, RN, Jail Nurse, collected sputum
specimen for Gen-Expert Testing from two (2) PDL of Bayugan City Jail. The
specimens were then brought by JO1 Angelita N Aranton, response team member, to
the City Health Office of Bayugan.

F. Monitoring of PDL with Newly Initiated Tuberculosis Treatment in Bayugan

City Jail
On 01 March 2021, JO1 Jennifer S Jamero, RN, Jail Nurse, conducted
monitoring of two (2) PDL with newly initiated Tuberculosis Treatment Regimen in
Bayugan City Jail. This was done through weight and vital signs taking, as well as
health history interview for any presence of experienced adverse effect from the TB
Treatment Regimen.

G. Screening of PDL with Presumptive Tuberculosis Manifestations in Surigao
del Sur District Jail
On 02 March 2021, JO2 Diana Christy O Quinonez, RN, Jail Nurse,
conducted a thorough assessment through TB Screening Checklist Form to six (6)
PDL with Presumptive TB Manifestations in Surigao del Sur District Jail Cantilan
for their referral to the Tuberculosis Disease Council (TBDC) of Tandag City.

H. Random Blood Sugar Testing of Two (2) PDL in Bayugan City Jail
On 03 March 2021, Ms. Lourdes Diola, RMT, Medical Technologist of City
Health Office of Bayugan, conducted Blood Extraction for Random Blood Testing
to two (2) PDL suspected of having Diabetes Disease in Bayugan City Jail. This
was done with strict observation of COVID-19 Protocol.

I. Nasopharyngeal and Oropharyngeal Swabbing for Two (2) Insular PDL in
Bayugan City Jail due for Transfer to DAPECOL (BuCor)
On 03 March 2021, Ms. Lourdes Diola, RMT, Medical Technologist of
City Health Office of Bayugan, conducted Nasopharyngeal and Oropharyngeal
Swabbing to two (2) insular PDL of Bayugan City Jail who were scheduled for
transfer to Davao Penal Colony, Bureau of Corrections.

J. Telemedicine Program in Bayugan City

On 02-03 March 2021, JO1 Jennifer S Jamero, RN, Jail Nurse, facilitated the
Telemedicine Session of three (3) PDL with Dr. Jeremiah de Castro of Bayugan
City Health Office. Said session was done through the Facebook Online
Consultation of Bayugan City Health Office.

K. 5PM Habit Kontra COVID-19 in Surigao City Jail
On February 26, 2021 to March 5, 2021, J/CINSP ROBERTO C GOTICO,
Jail Warden of Surigao City Jail (SCJ) together with SJO2 Laurence S Ortega, Jail
Nurse, implemented the “Daily 5 o’clock Habit Kontra COVID-19” program as part
of the STOP COVID-19 INITIATIVE PROGRAM through clean up and disinfection
of all contact surfaces of the jail facility.

L. 5PM Habit Kontra COVID-19 in Surigao del Norte District Jail Annex Dapa
On 27 February 2021 to 05 March 2021, J/CINSP RAUL C BADILLO, Jail
Warden, Surigao del Norte District Jail Dapa, enjoined the conduct of the “Daily 5
o’clock Habit Kontra COVID-19” program as part of the STOP COVID-19
INITIATIVE PROGRAM through clean up and disinfection of all contact surfaces
of the jail facility.

N. Triage Screening for Changing of Guards in Butuan City Jail Male
Dormitory
On 28 February 2021, sixteen (16) incoming Personnel on Lockdown (POL)
of Butuan City Jail Male Dormitory were subjected to Rapid Testing in the BJMP
Caraga Regional Office COVID-19 Testing Area duly conducted by JO1 Jessie Z
Doysabas, RMT, Regional Medical Technologist. Subsequently, they then
underwent Triage Screening conducted by SJO3 Florefe M Badillo, RN, Jail Nurse,
in Butuan City Jail Male Dormitory.

O. Removal of Surgical Staple Wires from one (1) PDL in Surigao del Norte
District Jail
On 03 March 2021, in Surigao del Norte District Jail, JO1 Khimkem
Labucay, RN, Jail Nurse, successfully removed the attached surgical staple wires
from the healed incision of one (1) PDL who previously underwent Appendectomy.

P. Consolidated Activities of Jail Nurse in Surigao City Jail

On 02-09 March 2021, respectively, SJO3 Laurence Ortega, RN, Jail
Nurse, conducted the following health related activities in Surigao City Jail:
a. Temperature Check of twenty-five (25) Lockdown Personnel;
b. Temperature check of fifty (50) PDL on Quarantine;
c. Health Status Monitoring of five (5) PDL

Q. Fasting Blood Sugar Monitoring to PDL in Bislig City Jail
On 07 March 2021, JO1 Cris Ann S Rosil, RN, Jail Nurse, conducted
Fasting Blood Sugar (FBS) Testing to two (2) PDL and one (1) personnel in Bislig
City Jail.

R. Rapid Antibody Test and Rapid Antigen Test of Incoming Lockdown

Personnel in Bayugan City Jail
On 08 March 2021, six (6) incoming lockdown personnel of Bayugan
City Jail underwent Rapid Antibody Test (RAT) and one (1) underwent Rapid
Antigen Test in Bayugan City Health Office.

S. Physical Conditioning Activities for PDL through Sunning and Morning

Exercise in Cabadbaran City District Jail
On 05 March 2021, JSI ANDRES T ESPINOZA JR, Warden, enjoined the
conduct of Physical Conditioning activities for PDL through Sunning and Morning
Exercise Activity in the Basketball Area of Cabadbaran City District Jail. A total of
sixty (60) PDL were able to benefit from this activity.

T. Physical Conditioning Activities for PDL through Sunning Activity in Surigao
del Norte District Jail
On 02-09 March 2021, respectively, JCI JOEL C CALIMBO, Warden,
enjoined the conduct of Physical Conditioning activities for PDL through Sunning
Activity in the Basketball Area of Surigao del Norte District Jail. A total of one
hundred two hundred (200) PDL were able to benefit from this activity. This activity
was supervised by JO1 Nove Gee Coniato, Alternate WDO.

U. Telemedicine Program thru Facebook Messenger

On 07 March 2021, JO1 Cris Ann S Rosil, RN, Jail Nurse, facilitated the
Virtual Medical Consultation conducted by Dr. Xelca Mae B Barbosa-Bechayda of
Hinatuan District Hospial via Facebook Messenger to thirteen (13) PDL and one
(1) Lockdown Personnel in Bislig City Jail.

V. “Tutok Gamutan sa TB sa Panahon ng Pandemiya”
On 07 March 2021, JO1 Cris Ann S Rosil, RN, Jail Nurse, conducted the
“Tutok Gamutan sa TB sa Panahon ng Pandemiya” to the three (3) PDL-TB
patients in Bislig City Jail through direct observation of TB Medication
Administration and provided nursing intervention thru assessment and health
monitoring.

W. Physical Conditioning Activities for PDL through Sunning Activity in

Surigao del Norte District Jail
On 07 March 2021, respectively, JCI JOEL C CALIMBO, Warden,
enjoined the conduct of Physical Conditioning activities for PDL through Zumba
Dance in the Basketball Area of Surigao del Norte District Jail. A total of fifty (50)
PDL were able to benefit from this activity. This activity was supervised by JO1
Nove Gee Coniato, WDO.

 X. Alay sa Puso Program
On 11 March 2021, BJMP Caraga led by J/SSUPT JOLLY C TAGUIAM
JR, along with the Health Service Division headed by SJO4 Richard C Recososa, RN,
joined hands with Barangay Doongan Local Government led by Hon. Gilberto Enriquez,
with the Department of Health HRH, in order to render a Free Medical Outreach Program
to the residents of Brgy Doongan, Butuan City. This activity was held at the covered court
of the said barangay, wherein, a total of eighty-six (86) residents were duly catered and
served to, most of whom are pregnant mothers.

Y. Community-based Rehabilitation Program
On 14 March 2021, SJO4 Richard C Recososa, Chief, Health Service
and Welfare and Development Divisions, conducted a Community-based
Rehabilitation Program Session through Sharing and Values Restoration Activity
to fifty (50) Drug Surrenderees in Bingkilan, Butuan City.

On 15 March 2021, under the supervision of SJO4 Rosalito A Reyna,
Warden, SJO3 Rhodora S Gamale, Chief Nurse, conducted posting of BIDA
Solusyon Fliers and IECs in Surigao del Sur District Jail.

AA. Rapid Antigen Test for Incoming Lockdown Personnel of Surigao del
Sur District Jail Tandag
On 1 March 2021, through the coordination efforts conducted by SJO4
Rosalito A Reyna, Warden, nine (9) incoming personnel of Surigao del Sur District
Jail Tandag underwent Rapid Antigen Testing at the quarantine facility of Tandag
City in Brgy. Buenavista.

BB. Rapid Antibody Testing and Triage Screening of Incoming Duties in

Cabadbaran City District Jail
On 15 March 2021, JO1 Jessie Z Doysabas, RMT conducted Rapid
Antibody Test to seven (7) Incoming Personnel on Lockdown (POL) of
Cabadbaran City District Jail through the use of Rapid Antigen Kits with due
observance of COVID-19 precautionary protocol.

District Jail Tandag
On 03 March 2021, SJO4 Rosalito A Reyna, newly installed SDSDJ
Officer-In-Charge of Surigao del Sur District Jail Tandag, underwent Rapid Testing
(RAT) at City Health Office of Tandag City. The outcome of the said test was
NEGATIVE.

DD. Receipt of Rapid Antigen Test Kits for Surigao del Norte District Jail
On 16 March 2021, JO1 Deonil Moreno and JO1 Junel Semine
received one (1) box containing twenty-five (25) Rapid Antigen Kits donated by the
Local Government of Claver, Surigao del Norte. This was made possible through
the coordination efforts conducted by JO1 Nove Gee Coniato.

On 24 March 2021, five (5) BJMP Caraga personnel assigned in Surigao
del Norte District Jail Dapa Annex successfully received the first dose of
AstraZeneca Vaccine in Siargao District Hospital.

On 26 March 2021, JO1 Carla Tosca Erika V Guittap, RN, Jail Nurse,
conducted treatment monitoring and health teachings to seventeen (17) PDL under
TB Treatment in celebration of the World TB Day.

XX. COVID-19 Vaccination (1st Dose) of Nineteen (19) Personnel in Bislig

City Jail and Surigao del Sur District Jail Bislig
On 25 March 2021, nineteen (19) personnel of Bislig City Jail and Surigao
del Sur District Jail Bislig underwent COVID-19 Vaccination (1st Dose) of
AstraZeneca Vaccine.

P 500.00 for the purchase of the said prescription.

Information Drive on Colorectal Cancer to fifteen (15) PDL in Bayugan City Jail.

On 28-31 March 2021, JO1 Carla Tosca Erika V Guittap, RN, undertook
to conduct Temperature Monitoring of six (6) Lockdown Personnel in Butuan City
Jail Female Dormitory.

On 22 March 2021, SJO3 Laurence Ortega, RN and JO1 Nathaniel Simon
G Bonotan, RN conducted Triage Screening to eighteen (18) incoming Lockdown
Personnel in Surigao City Jail.
