Script for the First Scenario.

Scene #1
*Dadating si Misis sa ospital na umiiyak at hawak-hawak ang kanyang tiyan dahil sa sakit,
duguan ito.
Patient: Tulong! Tulong!
Nurse: Ano pong nangyari?
Patient: Sobrang sakit na po ng tiyan ko, para pong pinuputol ang katawan ko.
Nurse: H’wag po kayong mag-alala misis. *mapapansin ng nurse na puro dugo na ang damit ng
babae
*Pauupuin ng nurse ang babae sa wheelchair at dadalhin sa isang stretcher
Nurse: Kailan po kayo nagsimulang makaramdam ng sakit?
Patient: Kanina pang alas sais ng umaga
Nurse: Okay po. Kailan nyo po napansin na dinudugo na po kayo?
Patient: Ha?! Hindi pwede! Yung anak ko nurse!
Nurse: Misis, kalma lang po kayo. Tutulungan po namin kayo. Pang-ilang pagbubuntis nyo na po
ba ito? Alam nyo po ba kung ilang buwan na ang pinagbubuntis ninyo?
Patient: Unang pagbubuntis kop o ito at walong buwan na po.
Nurse: Nakakapag-pacheck up po ba kayo buwan-buwan?
Patient: Hindi po.
Nurse: Sa ngayon po, kailangan ko po kayong kumalma at mahiga. Ilang taon na po kayo?
Patient: 23 years old po.

*Kukuhanan ng nurse ang pasyente ng vital signs


Nurse: Misis, kung ire-rate po ninyo yung sakit na nararamdaman nyo, 1 po ay walang sakit, 5
po ay may sakit pero kaya naman pong tiisin, at 10 bilang pinakamasakit.
Patient: 10 po.
*Pagkatapos kuhanan ng nurse ng vital signs ang pasyente, at magsagawa ng assessment sa
pasyente ay dumating na ang physician.

Physician: What is our situation, nurse?


Nurse: Doc, may dumating po na pasyente, 23 years old, gravida 1, para 0, 34 weeks gestation
po. Heart rate = 98 bpm, BP = 104/66 , RR = 22 cpm, Temp po is 38C, she weighs 66kg, her
oxygen saturation is 90%, Clear breath sounds, meron pong third heart sound. Continuous po
ang vaginal bleeding ng patient. Her uterine contractions are short and irregular.
Physician: How about her bowel sounds?
Nurse: Normoactive po doc. Alert and oriented rin po si patient, however, she is crying and
anxious, there is pressured speech.
Doc, may needle and track marks po sa arms ng patient. She also have a pack of
cigarette inside her pocket, and she reeks of cigarette smoke.
Physician: I see. What else?
Nurse: Her pupils are equally dilated and reactive to light. Fundal height is 32 cm. Skin is pale,
cool, and moist, but there are buish circle around the umbilicus upon inspection. She have
normal reflexes.
Physician: how about the baby?
Nurse: The fetal heart tone is 112, the fetal monitor strip showed that there is an increase in
the baseline with mild late decelerations. I think po, these could be a case of abruption
placenta.
Physician: I see. Okay, you did great, where is the patient?
*Pupuntahan ng doctor ang pasyente upang tingnan ang kalagayan nito.

Physician: Kamusta po kayo? Ako si Dr. Valenzuela, ang mangangalaga sa inyo ngayon.
Patient: Doc, sobrang sakit na po ng tiyan ko, humihilab po, parang pinuputol yung buong
katawan ko. Doc, yung anak ko, okay lang po ba siya?
Physician: Huwag po kayong mag-alala, sisiguraduhin po namin na ligtas kayo ng anak niyo.

*lalapit ang doctor sa nurse


Physician: Admit the patient for labor and delivery. Assess VS every 10 mins.
CBC with Differential, Platelet Count, Fibrinogen, D-dimer, PT, PTT, Type and
Cross match for 4units of Packed Red Blood Cells (PRBC), Electrolytes, BUN, Creatinine, Glucose,
Toxicology Screen for drug abuse, Urinalysis with dipstick for protein. Non per orem.
Nurse: Noted po, doc.
Physician: bring me the results, STAT.
Nurse: Yes doc.
Physician: I will not be giving the patient any pain medication. Keep the patient at bedrest.
Administer IV of 0.9% NS at 150mL/hour on pump, oxygen 8LMP by mask, urinary catheter for
drainage. Do not perform IE. Monitor the fetus.
Nurse: Yes po, doc. Thank you po.
Physician: Okay. Update me if there are any changes with the vital signs, and bring the lab
result STAT.

*The nurse will then carry out the physician’s order.

*After administering oxygen to the patient, the nurse checked the VS and the fetal heart tone.

Physician: How’s our patient?


Nurse: She is currently stable po, doc. As of now, the fetal heart tone is normal at 130, after
administering oxygen to the mother.

Add interventions, assessment, and evaluation


If profuse bleeding, do IE for cervical dilation, administer oxygen, and position the patient
Last menstrual period
Estimated blood loss
Character of pain
OB WARD
Nurses are given assigned client,
Clinical instructor will endorse to the student, case, complain, attending physician, diagnosis

SCENARIO #2
Narrator: A 23-year-old gravida 1, para 0 female is admitted into the Emergency Department
alone. She was crying and clutching her abdomen. When she arrived, she was complaining of
abdominal pain which she rates 5/10. After being interviewed by the nurse, she reports that
vaginal bleeding began two hours ago and “it has increased in the last 30 minutes.”
During that time, nursing students from TRACE College together with their Clinical Instructor,
came and witnessed the scenario.
Clinical Instructor: Good Morning Nurse JC, we are from TRACE College.
Nurse JC: Good Morning ma’am, sakto po ang dating ninyo, we just had an emergency patient.
She is a 23-year-old gravida 1, para 0. There was a profuse vaginal bleeding, and pain rate of
10/10 at the moment. Her vital signs when she was admitted are Heart rate = 98 bpm, BP =
104/66 , RR = 22 cpm, Temp po is 38C, she weighs 66kg, her oxygen saturation is 90%, Clear
breath sounds, meron pong third heart sound. Her last menstrual period is __________ . We
have already administered oxygen to the patient and a IV of 0.9% NS at 150mL/hour on pump.
As of the moment, she is currently stable. As well as the fetal heart rate which is at 130 bpm. I
need you to look after her and report to me ASAP if there are changes with her vital signs.

Clinical Instructor: Today you’ll be assigned to the OB ward, you will be handling a patient that
was just admitted to the Emergency Department. This

Student Nurse: Okay po ma’am.


Clinical Instructor: Go and check the patient.
Narrator: The student nurse went to patient.

Student Nurse: Good morning ma’am, I am ____________, ako po ang student nurse nyo para
sa araw na ito. I-checheck ko lang po yung vital signs nyo at pati na rin po yung kay baby. Ano
na po ba ang nararamdaman nyo?
Patient: Sobrang sakit pa rin ng tiyan ko. Wala naman mangyayari sa akin di’ba?
Student Nurse: Oo naman po, nandito po kami para po tulungan kayo.
Patient: Maraming salamat. AH!
Student Nurse: Ano pong nangyari? Okay lang po ba kayo?
Patient: AH! Sobrang sakit na!
Narrator: Agad na tiningnan ng student nurse ang vital signs ng pasyente. Matapos makita ang
pagbabago sa kalagayan ng pasyente ay agad na tumakbo ang estudyante sa kanyang clinical
instructor upang ipaalam ang kasalukuyang lagay ng pasyente.

Student Nurse: Ma’am there is a change in the vital signs of the patient, her heart rate has gone
up to 112 bpm, BP of 96/60, and respiratory rate of 26.

Narrator: After hearing this the clinical instructor immediately went to the nurse on duty and
told him the current vital signs of the patient.
