This document provides a checklist for a client to identify their personal goals for therapy. It is divided into 5 sections: coping with specific problems/symptoms, interpersonal goals, improving well-being, orientation in life, and self-related goals. Each section lists broad categories and specific goals that the client can mark if they want to pursue them in therapy. Space is also provided for clients to write any goals not on the list. At the end, clients are asked to write out their top 5 goals and describe how each fits into their life situation and importance. Goals Checklist
Setting goals is an important part of psychotherapy. That’s why we’d like to learn about your
personal goals for therapy.
To help you formulate your therapy goals, we’re providing the list below. The list is divided into
five sections. In the left column, we’ve listed broad content areas. In the right column, we’ve
listed specific therapy goals.
 Carefully read through the list and mark each goal you’d like to pursue in therapy.

 If you have a specific personal goal that you can’t find in the list, please describe it in your own
words on the lines provided.

 At the end of the list, please describe how each goal fits in with your specific life situation, and
indicate the importance of each goal.

Coping with Specific Problems and Symptoms
