09/07/2021 Pre- operative D> Into OR a 26 year old female per wheelchair accompanied
9:00 pm Preparation-Cesarean by NOD,with an informed consent for surgery was signed and

A> Ushered to OR table placed in supine position ; bolster was


9:15 pm Anesthesia Induction positioned under the patient’s right hip to offset abdominal
weight and thus reduce uterine pressure on the vena cava;
safety strap was secured and foley catheter was inserted; blood
pressure cuff, thermometer, ECG electrodes and a pulse
oximeter were placed on the patient. initial vital signs taken as
follows: BP 119/45 mmHg, CR 82bpm, RR 20cpm, SPO2
100%

A> Skin preparation solution was applied from midchest to the


pubis and to the sides of the patient all the way down to the
9:30 pm Skin preparation and operating room table as far as possible; and circulator shaved
drapping the area with disposable clippers.

A> The skin was incised via a midline vertical incision; skin
and adipose tissue were retracted, incision was carried to the
level of the fascia; posterior fascia was bluntly dissected from
9:50 pm Operation started the rectus abdominus muscle and secured with two Kocher
clamps; small transverse incision was made in the lower uterine
segment and carried bilaterally with Lister bandage scissors;
head was delivered by arching the baby’s torso toward the
mother’s abdomen; umbilical cord was clamped with two
Mayo clamps and then cut with Lister bandage scissors; and
then neonate was then passed to the awaiting neonatal team.

A> Uterine incision was closed in two layers using 0 synthetic


absorbable suture; hemostasis was achieved by use of the
electrosurgical unit; fallopian tubes and ovaries were placed
back into the peritoneal cavity; paracolic gutters and cul-de-sac
were cleaned of any remaining clots and blood; and lastly the
abdomen was then closed
10: 30 pm End of operation
R> The patient tolerated the procedure well and was sent to
recovery in stable condition.

09/08/2021 Postoperative Care D> Patient received with two liters of oxygen, and vital signs
2:00 am were monitored.

8:00 am A> Patient was restricted to bed rest for the first 24 hours
postoperatively; foley catheter was discontinued 24 hours
following surgery; dressings were removed and changed 48
hours after surger; prochlorperazine and metoclopramide was
prescribed as needed for nausea; and meperidine was
prescribed as needed for pain.

R> Verbalized relief of discomfort. Able to nap during the


10:00 am shift afterbreastfeeding the infant. No signs of infection and
hemorrhagingnoted.
