NCM 118 COMMUNITY HEALTH NURSING RELATED LEARNING EXPERIENCE WORK GUIDE

Introduction

Community organizing for community is a vital part of professional nursing practice. Embedded in
the process are dynamic activities and programs nurses can formulate in order to help the community to
become empowered and attain progress by helping them help themselves. The practice of dole-out
system has not been very effective in helping the communities attain resilience and self-improvement.
This causes beneficiaries to become very dependent on external aid. The guiding principle of Lao Tze
has been very effective in empowering the people in Bicol University College of Nursing extension
services helping the communities in Legazpi City. The extension program helped established successful
extension services promoting health, nutrition and environmental health. Thus, helping the people in the
community attain optimum health for the residents of the communities.

This related learning experience work guide will help you in the application of the theories,
concepts and principles to actual practice. The activities you will prepare from this module will help you
formulate program, projects and activities vital in community health nursing process. Your future
professional practice in public health will be very challenging and as future nurses and frontliners, BUCN
needs to mold you become very efficient, effective and competent health care deliverers. Community
development depends on healthy residents that will be able to organize themselves so that they will be
able to help you, public health care givers, develop sound programs and activities relevant to health. You
can become their agents of change.

Situation

As newly hired public health nurses by the Department of Health- Region V, you are assigned to
a far-flung community 60 kilometers away from Daraga, Albay. You will need to endure 15 kilometers
every week to be with the community at least once a week delivering basic health care services. The
community is in the shoreline west of Albay. The total number of the population is 2,400 and the number
of families is 840 with the number of households of 760. There are 14 pregnant women and most of the
men in the community are fisher folks earning from an average income or daily wage of 80 pesos per day.
There are many children ranging from the age of 7 to 12 and there are many youths too. Most of these
youths are female within the age ranging from 17 to 19. Male teenagers are helping with sea product e.g.
dried fish and making “binuro” or patis from this products. During a check-up, there were increasing
numbers of adults and elderlies having hypertension and EENT problems. Most of the children have skin
diseases and evidence of parasitism. The residents access their drinking water to a nearby spring and the
household have their own deep well for hygiene and sanitation purpose. The barangay local government
unit is organized however they have poor access in terms of health and social services. The barangay
has an elementary school, a mini chapel and sari sari stores are all around the barangay. They have 5
puroks and they are well represented with their purok leaders. The health and nutrition committee are not
so well organized and the health workers are not properly trained to handle health and nutrition matters in
the community. They still believe in hilot as their way of delivering babies. They also believe in “Tawas” or
“santigwar” as their way of alternative medicine. Different species of herbal plants are available in the
area however the residents do not know how to properly utilize them. They have variety of vegetable and
fruit plantation and piggery are all around the household and different animals are in the area e.g. native
chicken, fowls for sports, goats and dogs are seen along the road sides. There was a case of rabies and
measles in the area. Garbage is not properly dumped and it scattered along the road corners. Several
children are observed to be malnourished, with Eczema and have cough and colds. No Covid infection
are noted in the area but, fever affects some children and several of them have tooth decays (dental
caries). The teenagers are noted to have Tinea Flava and several teenage are pregnant (aged 17-19).
The overall situation of the place is peaceful and the people are seemed kind especially to health
workers. There is no available electricity but cellular phones and signals are present in the area (SMART
and Globe). The nearest market, health clinics and access to food and medicine is 8 kilometers away
from the adjacent municipality. You are assigned to help the community especially in uplifting and
improving their health and nutrition and promote environmental health.

With these community profile and situation. Work on the following exercises:

Exercise

Identify health risks and prepare the following:

1. As a group prepare a proposal for projects or activities that can be developed to address potential
health risks (choose from the adult, pregnant, youth or children). When preparing a proposal, you
can develop multi-approach for several diseases so you can create a program or projects, e.g.
medical, nutritional and dental outreach program, feeding program etc. Or you can prepare a
training program for the barangay health workers or series of mother’s class for the pregnant and
lactating mothers. Please prepare information education material also as a group in relation to the
potential topics to be discussed. The proposal, health education or health activities and IEC will
be presented every Wednesday 8:00 to 11:00AM.
2. During the coordination or collaboration with other agencies who can help you with your proposed
activity, prepare a coordination letter (individually) and a sample memorandum of agreement (as
a grou) with the agencies. You can develop multi-sectoral approach e.g. tripartite agreement.
3. Prepare individually a FNCP and learning insight for the week’s activities. This is to be submitted
4 days after the RLE.
4. The group must also submit an accomplishment report 4 days after the duty, these reports will be
compiled along with the FNCP.

A. Professional competence - 50%


B. Requirements - 30%
C. Personal attributes - 20%
