INTRODUCTION

Colongulo National High School is situated at Purok Sampaguita, Barangay Colongulo,Surallah,

South Cotabato, approximately 100 meters away from National Highway. It is facing the mountainous
area of the Barangay and the mountain range of Lake Sebu, located at the western part of the
municipality of Surallah, approximately five kilometers away from the town proper.
Most of the families of Purok Sampaguita depend on farming and labor works as their way of
living. The community is thickly populated by Antiquenos. The residents of Purok Sampaguita are
God- fearing people. One manifestation of this is the construction of Catholic Church with in the
purok,.
The Purok was established through the generous heart of the late Barangay kagawad Fortunato
Fordan. He donated 2,000 square meters of area , more or less , as a center of Purok, drier,
Samahang Nayon building, basketball court , waiting shade.
To lessen the problem of garbage, the Parents Teachers Association and Purok officials initiated
the program by putting up Material Recovery Facility ( MRF). And in order to subsidize the
sustainability of foods, they encourage the people in the community to plant vegetable garden, trees
and flowers and do recycling activities.

RATIONALE:
Republic Act 9003 otherwise known as Comprehensive Solid Waste Management Program in the
Philippines, an advocacy to awaken and promote the interest of the people to CLEAN and Green
Program of the government.
The aim of this program is to promote “Cleanest and Greenest Community. Evaluation and
monitoring was conducted to ensure the sustainability and implementation of the program. The
Municipal Environment and Natural Resources (MENRO) set a criteria for the Best Community
implementor of solid waste management.
One of the community involvements is an Adopt-a-Purok Program. Households are challenged to
sustain the program because they can benefit primarily from the products of vegetable gardens. To
make the program more meaningful contest on Best SWM implementor was conducted. They are
regularly monitored to set the mode for evaluation and monitoring. Prizes are at stake.

OBJECTIVES:

A. To promote the SWM program and its benefits to the community..

B. To lessen the problem on improper waste disposal in the community.
C. To involve residents in meetings and planning that concerns waste segregation.
D. To promote caring the environment by planting trees, vegetables, flowers, and even in the
promotion of recycling products.

Resolution To Strongly Support the Implementation of Solid Waste Management Program of
Colongulo National High School

Whereas, the Colongulo National High School is implementing the Solid Waste Management
Program based on Republic Act 9003 “ Comprehensive Solid Waste Management in the Philippines.
Whereas, the Parents Teachers Association (PTA) of Colongulo National High School is supporting
the implementation of Solid Waste Management its program and projects especially the Adopt A
Purok program.
Whereas, the community and the parents support the implementation of proper waste segregation in
their respective households.
Now, therefore, Mrs. Hyde Vicente on her motion for unanimous sponsorship be it.
Resolved as it is hereby resolved to Strongly Support the Implementation of Solid Waste
Management program of Colongulo National High School.
Resolved finally to furnish copy of this resolution to the School Principal, Melinda C. Carnecer, SSG
and the Purok President, Mr. Allan Derla for their information.

MEMORANDUM OF AGREEMENT

KNOW ALL MEN BY THESE PRESENTS:

This Memorandum of Agreement is entered into and between by Grade 10- Heron of
Colongulo National High School, represented by,
____________________________________________________, Grade __________________ Adviser,
with office address at Colongulo National High School, Purok Sampaguita, Colongulo,
Surallah, South Cotabato herein referred to this instrument as FIRST PARTY.

AND

MR. / MRS. ________________________________________________, of legal age, married and a

bona fide resident of Purok _____________________, Colongulo, Surallah, South Cotabato
hereinafter referred to this instrument as the SECOND PARTY.

WITNESSETH

WHEREAS, one of the guidelines set by the Clean and Green Evaluation Committee in
School- Based, Municipal and Provincial Level is to extend its advocacy, not only in school
but also in the community by adopting a household and by providing technical assistance
in maintaining its cleanliness and instill core values related to concepts of a cleaner and
greener environment.

WHEREAS, the proper observance of R.A. 9003 is considered to be the best and effective
way to show love of the Mother Earth by encouraging households and store owners in this
barangay, passers-by and visitors and not just a mere compliance for evaluation purposes.

WHEREAS, the SWM Classroom Committee unanimously chose to adopt the household of
Mr. / Mrs. _____________________________________________ for this selfless service.

NOW, THEREFORE, for and in consideration of the following premises and terms of
condition herein set forth, both parties hereby agree the following:

MR. / MRS ___________________________________________Grade ____________ Adviser (1st

Party) shall:

1. Provide four (4) garbage bins properly labeled as to types of wastes at the entrance
or in any conspicuous place within the backyard of the 2 nd party’s residence;

2. Establish vermi beds for vermi culture, and potting media and provide proper
cultural management for various plants planted in the backyard of the of the 2 nd
party;
 3. Conduct clean- up drive operation in relation to the objectives of SWM whenever
necessary; and

4. Keep the records and journal of SWM activity done outside the school.

MR. / MRS. __________________________________________ RECIPIENT (2ND PARTY) SHALL:

1. Maintain the vegetables garden and ensure the sustainability of vermi culture, and
medicinal garden;
2. Segregate waste properly; and
3. Update the 1st party on the development of its project.

This contract takes effect upon signing of both parties and is valid for the period from the
________ day of _________ 2016 until ________ day of ___________ 2017. However, the 1 st
party reserves the right to cancel this agreement in part or in whole for any violation of the
terms and conditions stipulated herein.

In witness, hereof, both parties hereto set their hands and cause this agreement this
