FACTS

           Initial Investigation disclosed that Montero Sports (Veh-1) was then


traversing north direction Brgy. Disgrasya this city while the Rusi Motorcycle (Veh-
2) was coming from opposite direction (south bond). The Vec-2 is then on the
process of overtaking and upon reaching the place of incident both vehicle collide.
(Vehicle-1 to its left front bumper while Vehicle-2 to its front wheel). The driver of
the Veh-2 sustained severe injuries and was immediately brought by the
responding Rescue 123 if this municipality at Maayo General Hospital, Digos City
for immediate medical treatment but unfortunately his arm dislocate and also have
some fractures in his  hand. Both vehicle incurred damages of undetermined costs
and it was brought to this office with both driver for proper documentation and
disposition.

REMARKS: P.M., July 15, 2022. It appears in the investigation conducted by the officer-
on-case (PO1 Remiscal Rheymark), that driver of vehicle B is the party at fault for hitting
the left front bumper of vehicle A.

NOTE: Driver of vehicle B was advised to undergo medical check-up.

ACTION TAKEN: Driver of vehicle B was issued T.O.P.

Initial Investigation disclosed that Montero Sports (Veh-1) was then traversing north

then reported and the suspect was immediately apprehended.

1. Reference: Police Blotter Entry No. 2468 September 8, 2021.


2. A theft incident happened on or about 12:55 in the afternoon of September 8, 2021
at Mac Arthur Highway, Digos City. The perpetrator Sandra Te, 27 years old, single,
a resident of the said place was identified.
3. The victim, Laura Sy, 25 years old, who is also a resident and a neighbour of Sandra
Te in Mac Arthur Highway, Digos City after she left her house in the morning for
work she realized that she left her designer bracelet at the top of the table in their
dining area. And when she got home it went missing.
4. After work, Laura then checked the CCTV, the perpetrator stole the bracelet at
about 12:55 in the afternoon and the suspect did not realized that there’s a CCTV in
that part of Laura’s house. She then immediately identified the suspect.
5. Further investigation is still on-going and the motive of the incident is yet to find
out.
6. Other developments regarding the incident shall be submitted as soon as obtained.

1. Reference: Police Blotter Entry No. 01234567 dated 07 February 2022.

2. On June 13, 2022 at around 9 o’clock in the evening a theft incident transpired at
Barangay. Sweto Davao City wherein the victim was, Clint Buotan, 18 years old, single and
a bonafide residence of Brgy. Gubot Davao City. His phone was allegedly stolen by the
identified man named Balmond Fasthand, 20 years old, single and living alone in his place
at Barangay Ilado Davao City. The incident that took place in front at Aling Nasing’s store,
was witnessed by Marie Marites, neighbor of Clint, who bought sardines and egg in the
store.

3. A theft incident transpired at Brgy. Gubot Davao City wherein the victim was, Clint
Buotan. His phone was allegedly stolen by the identified man named Balmond Fasthand,
20 years old, single and living alone in his place at Barangay Ilado Davao City. The incident
that took place in front at Aling Nasing’s store, was witnessed by Marie Marites, neighbor
of Clint, who bought sardines and egg in the store. Afterwards, both of them were shocked

at the rapid pace of the event. They could even identify which corner Balmond went as he
was ran directly towards straight direction. The victim initiate and thought of going to
Balmond’s friend living in the area as he was speculating that the accused might hiding in
their house. However, that was the moment Clint confirmed that Balmond friend was not
aware that the accused was in their Barangay.

4. This report was confirmed by PLT Kid Smith who were the in charge police of the
investigation. Upon investigation, they confirmed that it wasn’t the first time Balmond
made the incidents. The investigation viewed his previous record from different Barangay
and still looking by the Authority.

This pertains to the theft incident that transpired at Mac Arthur Highway, Digos City
perpetrated by James Pepito on September 8, 2021 at 11:30 in the morning.

Report revealed that James Pepito stole big amount of money from a grocery store.

Further investigation disclosed that James Pepito, 25 years old, a resident of Mac Arthur
Highway is an addict and is stealing money. Report also revealed that this is not the first
time Pepito stole big amount of money, he also stole money from his aunt.

Officers from Digos City Police Station have conducted follow-up and further
investigations that resulted in issuance of warrant of arrest.

Other developments regarding the incident shall be submitted as soon as obtained.

door at about 2:30 am, Saturday. Gallaza tried to pacify Armateo, who was under the
influence of liquor but failed until he shot the victim in his left chest using his .357
revolver. Armateo reportedly threatened Gallaza and his family that they will all be killed,
and he would "drink their blood," Corpuz said. The victim was immediately brought to
San Enrique Memorial Hospital in Valladolid City but pronounced dead on arrival by the
attending physician. Recovered from the crime scene were two .45 caliber pistols with
live ammunition and a gun recoil spring while unfortunately the unlicensed .357 revolver
was not found after the suspect threw it into the rice fields. Police said Armateo was
having a personal problem with his wife who is having custody of their children. Armateo
went home in Brgy. Sibucao, San Enrique to attend the court hearing in Valladolid
regarding the charges of alarm and scandal filed against him recently.

Police Captain Eduardo Corpus, police chief of San Enrique said they are waiting for the
result of the paraffin test, expected to be out today, before the filing of the case. Police
investigation showed that Armateo, armed with two .45 caliber pistols, shouted bad
words against their family and forced his way into the house of Gallaza, by destroying the
door at about 2:30 am, Saturday. Gallaza tried to pacify Armateo, who was under the

4. In reference to the predecessors of this report, the narrative started with a


drunk Federito Armateo breaking into the Gallazas’ residence at approximately
2:30 am of October 10, 2001. Indication of his forced entry was emphasized by
a broken door. The occupants of the house went to investigate and found an
intoxicated Armateo armed, threatening them. Peter Gallaza and his family
tried to negotiate but Mr. Armateo sustained hostility. Peter Gallaza killed his
brother-in-law as his last resort via gunshot wound on the latter’s chest. Mr.
Gallaza then disposed the weapon used (a .357 revolver) by throwing it to a
rice field. On the other hand, the victim (Federito Armateo) was brought to a
hospital (Salvador Benedicto Memorial Hospital) located in La Carlota City but
was declared DOA (dead on arrival).

5.
A. Upon receiving the report via call, we dispatched a team to investigate and find
sufficient grounds.

B. Items recovered were a gun recoil spring and two (2) .45 caliber pistol. A
paraffin test was conducted.

6. The investigations found that:


-Armateo was a member of the PNP Special Action Force assigned in Iligan city,
ranked as a Patrolman.
-Armateo had issues with his wife regarding child custody
-Armateo was recently charged with alarms and scandal prior to this incident.
-Armateo went far to elaborate his threats against the Gallaza family, stating
that he will drink their blood upon their slaughter.
-The weapon used by Mr. Gallaza wasn’t recovered and is nowhere to be found.
-The paraffin test proved positive of Mr. Gallaza’s use of firearms during the
incident.
-There is sufficient ground for HOMICIDE.

V. SYNOPSIS OF OPERATIONS:

7. In the campaign against illegal drugs being implemented in Barangay Zone


1 of Digos City. especially at the Mac Arthur Highway intersection in the
same barangay. This led to the arrest of one RAUL MENDES, 28, a single
resident of Barangay Zone 1 in this city, and the confiscation of the
following items/evidence from his custody and control: three (3) hot sealed
transparent plastic bags containing white crystalline substance thought to
be "shabu," two (2) pieces of 100 pesos' denomination marked money, and
one (1) piece of aluminum foil. The Comprehensive Dangerous Drug Act of
2002 (CDDA), which amends R.A., is said to have violated Sections 5, 11, and
12 of Article II. A complaint known as I.S. No. 6425 was made against the
suspect RAUL MENDES before the prosecutor's office. 66K-66666, IV-66-
TVX-66. By signing a waiver in accordance with Article 125 of the RPC, the
suspect exercised their right to a preliminary inquiry and is still in police
custody.

8. That last November 18, 2021 said arrested suspect was committed to Digos
City Reformatory Center while awaiting trial with criminal case # 66766
and 12345 under presiding Judge, Judge CHEASTY SANCHEZ, Regional Trial
Court Branch 4, Sta. Ana, Digos City.

The whole staff's unwavering assistance and support for one another
demonstrated admiration for the organization's commitment to completing
its purpose, as well as goals.

VI. RESULT OF OPERATION:

9. That sometimes in the morning of September 8, 2021, a confidential


informant appeared at our office and reported to P/LT JONATHAN GORDO,
Officer of the Day that a certain “RM“ is engaged in selling illegal drugs
(shabu) at Barangay in this city. Moreover, our informant relayed to us that
he (informant) and certain “RM” agreed to meet each other at or about 7:45
in the evening of same date at the crossing road going Mac Arthur Highway,
this City purposely to buy stuff.

10. Acting on the information, P/LT GORDO brief the INVEST /AIDSOFT
personnel at our Office regarding a plan for Buy-Bust operation against
“RM”. During the briefing P/LT GORDO, relayed the information which he
obtained from the Brown the marked money consists of two (2) pieces of
One Thousand peso (Php1,000.00) bills with serial numbers Df666666, and
HJ777777 which PMSg Brown marked his initial “X” on each bill and signed
in a acknowledgement receipt. While PSMS Marshall Mathers III was
designated as arresting officers and the other members serve as back-up.
PMSg Brown and PMSg Mathers III decided their pre-arranged signal that
once PMSg Brown will turn ON and OFF the flashlight of his cellphone it
means the transaction had already consummated and in that instance I will
rush towards them and arrest the suspect “RM”;

11. That at around 7:45 in the evening of same date, coordination with the
PDEA was made and thereafter PMSg Brown and PMSg Mathers III together
with the informant proceeded to the target area earlier of the (informant
and “RM”) agreed meeting time while the others personnel went behind.
There at a minute after PMSg Brown noticed a male person walking
towards them and the confidential informant told him that it was target
alyas “RM” the informant called the said person by mentioning his name
“RM” and introduce PMSg Brown as a buyer of stuff (shabu) then a certain
“RM” asked PMSg Brown how much will he buy? PMSg Brown said four
hundred pesos (Php400.00) only at the same I handed to him the said
marked money and insert his hand to his right pocket and pick one item:
one (1) piece small size heated sealed transparent plastic sachet containing
white crystalline substance suspected shabu with one (1) piece of
Aluminum Foil and handed to him. After scrutinizing the said item and after
sensing that it is a shabu, immediately, PMSg Brown performed the
prearranged signal before PMSg MarshalL Mathers III rushed towards them
and identified himself as police to alyas “RM” No resistance coming from
the suspect during the conduct of arrest and subsequently was apprised his
constitutional rights.

12. A case for VIOLATION OF SECTION 5, 11 and 12, Article II of Comprehensive


Dangerous drug act of 2002 (CDDA) amending R.A. 6425 was filed before
the prosecutor’s office against the suspect RAUL MENDES docketed under
I.S No. IV-66-TVX -66K-66666. Suspect availed of their rights for
preliminary investigation by signing a waiver under articles 125 of RPC and
remain at the police custody.

13. That last November 18, 2021 said arrested suspect was committed to Digos
City Reformatory Center while awaiting trial with criminal case # 66766
and 12345 under presiding Judge, Judge CHEASTY SANCHEZ, Regional Trial
Court Branch 4, Sta. Ana, Digos City.

VII. CRITIQUE AND OBSERVATION:

14. That the members of Digos City Police Station 1 (Tres de Mayo), DCPO
Headed by PCPT JESTER SANCHEZ GORDO under the leadership of PCOL
LUKE JIMENEZ, City Director, DCPO showed teamwork, alertness and
bravery in the performance of their respective duties, likewise loyalty to the
services that outmost carried-out in the whole troops. Thus, Officer of this
DCPS1, DCPO really achieved its mission and responsibilities as law
enforcers and guardians of Peace.

VIII. RECOMMENDATION:

15. Strongly recommend that officers and personnel be awarded with PNP
Efficiency Medal to bolster their Morales as a rewards of their devotion to
Duty and loyalty to the service for lasting peace, being the members of the

V. SYNOPSIS OF OPERATIONS:

6. The Philippine National Police primarily responsible of peace and order Serve
and protect the lives and property of the populace and rights of an individual. As

servant of the public places to attain its common goal in most religious and
dedicated services towards people around us. In view of the successful mission
conducted on time and day of duties, P03 Rheymark Remiscal, and its personnel
pledge deeply concerned to encourage more improvement and positive action
Barangay Permibikil, Davao City. That resulted to the arrest/apprehension of
certain BALMOND FASTHAND, 21 years old, single, resident of Barangay Ilado, and
confiscated from his the item/evidence; super ultra android phone. A complaint for
Theft under section 81 of Republic Act 10951, which amended article of Revised
Penal Code was referred against the suspect for the inquest proceedings at the
office of Philippine National Police.

VI. RESULT OF OPERATION:

7. That at around 8:30 in the morning of 19 June 2022, CLINT BUOTAN, 18 years
old, Filipino, single, son of Miya Badlungon, and MIYA BADLUNGON, 41 years old,
Filipino, married, house keeper, mother of Clint Buotan, residents of barangay
Sweto, Davao City personally appeared at the  barangay Permibikil, Davao City  
Police Station  to report a theft incident that transpired at around 10:00 in evening
of 05 February 2022 at Aling Nasing’s store located at barangay Permibikil, Davao
City and identifying Clint Buotan as victims. 

8. That Clint Buotan called the police office where the case was filed. He cited the
great possibility of the whereabouts of the accused.

9. That after receiving the report, we immediately proceed in the reported place
and saw the accused named BALMOND FASTHAND.

10. That during the interview on Clint Buotan, he clarifies about the night that the
incident happened and took place. He confirmed that Balmond Fasthand was the
culprit in the case.

11. That after hearing the information disclosed by the victim, the police
immediately invited Balmond Fasthand to go with them at the police station for
interrogation and clarification which the accused followed straight away.

12. We   execute   this   affidavit   to   attest   to   the   veracity   of   the


aforementioned   facts   and   to   support   the   filing   of   appropriate charge/s in
court against the arrested suspects.

VII. CRITIQUE AND OBSERVATION:

13. That the members of Davao City Police Station 1, by PO3 Rheymark Remiscak
together with PO1 Victor Basco, and Jerome Girona., DCPO showed teamwork,
alertness and good effort in the performance of their respective duties, likewise
loyalty to the services. Thus, the officers really achieved its mission and
responsibilities as law enforcers and guardians of peace.

VIII. RECOMMENDATION:
