    MACDEE CHOREOGRAPHY

    DANCE GROUP PERFORMANCE BOOKING AGREEMENT

    This agreement ("Agreement") is mutually agreed upon by MACDEE CHOREOGRAPHY ("Artist")


    and__________________________, representative of _________________________ ("Groom/Bride")
    on the _________________ (date of Agreement).

    1. SERVICES TO BE PROVIDED BY ARTIST


    a) Artist agrees to provide dance group performance at the wedding party ("Event") located
    in_____________________ (city and state/country) on_________________ ("Date") between the hours
    of _________________ (start time) and _________________ (end time) ("Time").

    b) Artist agrees to provide a sober, (i.e. not intoxicated or inebriated by alcohol, narcotics and/or
    otherwise), responsible, conduct, in his deployment of his services such that there is always a high degree
    of professionalism at all times.

    2. SERVICES TO BE PROVIDED BY THE GROOM / BRIDE


    a) The Groom/Bride agree to provide access to or a copy of the wedding video for purposes of marketing
    and future business clientele reference.

    3. COMPENSATION OF ARTIST
    a) The Groom/Bride shall pay Artist the sum of $______usd ("Fee") for the rendering of service(s)
    hereunder, of which a deposit of $________usd fee as a good faith non-refundable deposit before the
    start of rehearsals in order to secure booking engagements.

    b) The Groom/Bride agrees to provide transportation, for Artist. If Artist handles any transportation costs
    of such nature, the Groom/Bride agrees to promptly reimburse for the reasonable costs of such
    transportation, which in this particular case amounts to $______usd.

    c) The Groom/Bride shall pay Artist the remaining Fee no later than one (1) week before the event.
    4. CANCELLATION
    a) In the event that the Groom/Bride cancels the Contract with at least 30 days prior notice from its
    scheduled Date and Time as detailed hereunder, no refund of any monies paid in advance to Artist
    shall be made and the balance of the monies due to the Artist shall be waived.

    b) In the event that within 30 days of the Event, as detailed hereunder, the Artist cancels the contract
    or fails to perform the contract for any reason including Acts(s) of God and the amount paid will
    be refunded.

    c) Notice of cancellation in advance shall be deemed received only upon direct voice contact
    between
    the Artist and the Groom/Bride. In the event that this is not possible written communication should be
    sent.

    5) MISCELLANEOUS

    a) This Agreement may be changed only by mutual agreement of authorized representatives of the
    parties in writing.

    b) This Agreement constitutes the entire agreement and understanding between the parties with
    respect to the subject matter hereof.
