    EMERGENCY ITEMS
    Emergency evacuation map posted near work area? Emergency phone numbers posted and known by all? Emergency eyewash and/or shower units accessible? First aid kit available at work site? First aid trained competent person available? BBP kit available/BBP trained individual on site? Fire extinguishers readily available (not blocked)? Fire extinguishers inspected monthly/yearly as needed?

    Safety glasses and/or goggles available + being used? Protective eyewear use is specified in writing? Face shield available for bulk liquid tasks? Grinding? Hand protection used/worn as required? Foot protection worn as required? Hearing protection worn where required? Hard hats worn when falling object hazard is present? Supplies on hand for incidental chemical spills?

    ELECTRICAL SAFTEY ISSUES


    GFCIs used for all portable electrical hand tools? Extension cords rated for hard or extra hard usage? (2 wire ribbon cord is unacceptable for industrial usage) Certified or listed equipment is used per manufacturer? Electrical panels are labeled appropriately? Electrical panel knockouts are in place? Light bulbs for illumination protected from breakage?

    ELECTRICAL SAFETY ISSUES Strain relief integrity for cords and plugs in tact? For extension cords; hard usage cord includes three wire cord marked = S, ST, SO, STO, SJ, SJO, SJT + SJTO Electrical cords inspected & have all prongs in tact? Strain relief in tact for all flexible cords & plug fittings? Portable generators are grounded per NEC requirements? Electric power tools are double insulated or grounded?

    CONSTRUCTION SAFETY & HEALTH ISSUES


    General housekeeping is neat and orderly? MSDS openly available to all employees? Concrete work? Silica dust training documented for all? All hazardous containers labeled appropriately? Lockout/Tagout is being used for appropriate tasks? Hot work permits used for grinding, cutting, welding? Confined space entry work? Check training/permit/etc.

    CONSTRUCTION SAFETY & HEALTH ISSUES


    Flammable liquids are in OSHA/FM metal safety cans? Flammable liquids storage containers labeled properly? Fire extinguisher readily available, accessible + inspected? Impact style air tools have safety clips or retainers on them? Pneumatic power tools have hoses secured? Compressed air used for cleaning limited to 30 psi ? Compressed gas cylinders not in use have caps in place?

    Copy the completed inspection sheet to: If marked"U" for unacceptable or no; list the appropriate corrective action on the reverse side

    CONSTRUCTION SAFETY & HEALTH ISSUES


    Compressed gas cylinders stored secured + upright? Oxygen/acetylene torch units have flash back arrestors? Grinders (portable + stationary) have guards in place? Stationary grinding wheel tool rest is 1/8 inch or less? Stationary grinding wheel tongue guard is inch or less? Grinders are inspected, ring tested + free of defects? Safety glasses + face shield used for grinding tasks?

    CONSTRUCTION SAFETY & HEALTH ISSUES


    Wall openings + floor holes are covered or guarded? 100% fall protection in place above 6 feet in height? Employees operating lifts are trained on the equipment? Fall protect. full body harness+ lanyard used at all times? Excavation? Ladders used > 4 feet deep? Extend 3 feet? Excavation? Protection from cave-ins for >5 feet deep? Rebar caps used for protruding reinforced steel posts?

    CONSTRUCTION SAFETY & HEALTH ISSUES


    Ladders are safe and inspected as appropriate? Stair rails = reqd at 30 change in elevation or 4 risers? Stairs or ladder provided for access points > 19 high? Extension + straight ladders extend 3 beyond landing? Stepladder or commercial stepstool used for high access? Step ladders are only used in open position? Scaffolding = guardrails used? competent person on site?

    CONSTRUCTION SAFETY & HEALTH ISSUES


    Portable circular saws equipped with protective guards? Unsafe hand tools are prohibited? Impact tools, hammers kept free of splinters/mushrooms? Wire rope used for lifting? Deterioration is absent? Web slings used for lifting? Deterioration is absent? Crane use? Written lift plan on file listing load capacities? Hooks used for lifting have safety latch in place?

    CORRECTIVE ACTION PLAN
