Creating an online survey form can help you gain a better understanding of
what makes you gather feedback while processing online survey data can be very
time consuming and boring. Survey data processing consists of designing, data entry
and data capture, data quality assurance and analysis, and reporting and data
tabulation.
This lesson comes with activities designed to determine your progress. You
have to answer each to the best of your ability. Hopefully, after learning this module,
you will be equipped with knowledge and skills in creating an online survey form and
processes an online survey data.

What Is It
Definition of Terms
❖ Survey - to query (someone) in order to collect data for the analysis of some
aspect of a group or area
❖ Online - connected to, served by, or available through a system and
especially a computer or telecommunications system (such as the Internet
❖ Fraud - specifically: intentional perversion of truth in order to induce
another to part with something of value or to surrender a legal right
❖ Forms - a printed or typed document with blank spaces for insertion of
required or requested information
❖ Google Drive - is a file storage and synchronization service developed
by Google. Launched on April 24, 2012, Google Drive allows users to store
files on their servers, synchronize files across devices, and share files.
❖ Google Sheets - is a spreadsheet program included as part of a free, web-
based software office suite offered by Google within its Google Drive service.

personalized survey or quiz. The information is then collected and
automatically connected to a spreadsheet. The spreadsheet is populated with
the survey and quiz responses

Creating an Online Survey Form
There are two ways in creating an online survey. You can create from:

Step 3. Click “New” in the top left hover over More, and choose Google Forms
Step 4. A welcome message will appear just like the next figure. Click Get
Started.

Step 5. A form template opens and there you can add any questions for your
survey.
a. Start with the title of the survey. Click Untitled form in the
upper left corner and type the desired title
b. Check the box labeled “Show progress bar at the bottom of form
pages”, if you like to see the progress bar indicating how much of
your form they’ve completed.

You may also wish to add a theme in your form by doing the following:
All you need is to right-click the thumbnail of the form in your drive,
hover over Open with and click Google Forms, if you want to stop and
continue doing your survey form later.

Step 1. Click New in the top left, and choose Google Sheets, in your drive

Tip: To get rid of this message, you can click Dismiss and continue
editing your spreadsheets.
1. Write brief but clear introduction. What is the survey all about? What is its
objective or purpose? How long would it take to answer it? Where will the data
be used? Will the responses be kept confidential? And other related matters.
2. Design you survey form an attractive, simple, and easy-to-read way using
appropriate colors, font size, and arrangement of the parts of the survey so
that the respondents can easily move from one part to another in a logical
manner.
3. Keep your instructions in answering the questions using short and simple
language.
4. The format of your survey form should be well organized. You may start with
a personal profile of respondent – name, age, gender, locations, and other data
you need. Then, follow this with an introduction, instruction on how to answer
the question, and then the survey questions. Do not make the survey
questions too long as to require many pages. Respondents may lose the
interest to complete the survey.
5. Pilot test your survey. You can ask some people (to act as respondents) to
answer the questions. Find out if there are problems or difficulties in
understanding the instruction and the questions. You can improve the survey
form with the feedback of the respondents.

Advantages of Creating Online Survey
1. Faster
The time span needed to complete an online survey project is on
average two-thirds shorter than that of traditional research methods.
2. Cheaper
Using online questionnaires reduces your research costs. You will save
money on postage and you don't have to allocate time and resources to enter
the information into a database
3. More accurate
The margin of error is greatly reduced with online surveys because
participants enter their responses directly into the system. Traditional
methods rely on the attentiveness of staff to enter all details correctly, and
naturally human error can creep in whenever a person has to perform a
repetitive task.
4. Quick to analyze
The results of the online survey are ready to be analyzed at any time.
7. Easy to style
An online survey is an opportunity to imprint your brand in the user’s
mind and remind them of the benefits you provide
8. More honest
It’s overwhelmingly prefer to complete online surveys rather than take
part in written questionnaires or telephone interviews and usually provide
longer and more detailed answers
9. More selective
With an online survey you can pre-screen participants and allow only
those who match your target profile to complete the survey

1. Survey Fraud
This is the biggest challenge. If your survey is long and/or confusing,
you might get fake answers. Since there is less accountability, chances for
people just hitting buttons to finish are high. Check the questions you use
carefully.
4. No Interviewer
The lack of a trained interviewer to clarify and probe can lead to less
reliable data.

Click “Create Account,” and then select “For Myself.”
On the next page, you provide some information—first and last name,
username, and password—to create your account.
You also have to verify your phone number so Google can make sure
you’re not a bot.
Type the name of your form then type the description of your form.

How to Create a Quiz
Click the “Quizzes” tab, and then toggle on “Make This a Quiz.”

When you are finish, click “Save” to exit the window.
Once you return to your quiz, select one of the questions, and
then click “Answer Key” to edit the correct response, and the weight
each question has in the quiz.
Here’s where you set the correct answer(s), decide how many
points each one is worth, and add answer feedback for the answers to
each question.

Click “Edit Question” to close the answer key and save your changes.
To add more questions to your form, click the plus sign (+).

If you want to add any questions to a different section, it’s simple! Just
drag and drop them between sections. At the end of the section, click the drop-
down menu to choose where the form should direct people next.
To do this, click the three dots at the top of the page, and then click
“Add Collaborators.”

Next, under the heading “Who Has Access,” click “Change”.
Now, you can copy and share the link with anyone you want to
have access to your form.

To do this, select the “Responses” tab, and then click the green
Sheets icon.

Choose the spreadsheet you want from the list of those saved on
your Google Drive, and then click “Select.”

Click the purple page icon.
Click on a template. The form opens in the current tab and saves
to your Drive with all your other forms. If you want to add any questions
or edit any existing ones, the templates are customizable, just like any
other form.

Add the Final Touches
The first tab has a few settings you can enable. From here, you
can collect email addresses and limit each person to one submission.
You can also choose whether respondents can edit their answers after
they’re submitted or see a summary chart at the end of the survey

The “Presentation” tab has settings that show a progress bar that
lets people know how far along they are in the form. You can also shuffle
the question order, show a link to submit the form again (if “Limit to 1
response” is disabled), or compose a confirmation message that
respondents see after they submit the form.
After you finish, hit “Save” to save the changes and return to your form.
To get sharing, open the form you want to share, and then click “Send.”
Choose how you want to share your form from the options at the
top of the pane. From left to right, your choices are: email, direct link,
an embedded link for your website, Facebook, and Twitter.
