Employee Evaluation Form

This employee evaluation form contains sections for employee information, performance ratings in core values and job-specific categories, performance goals, an overall rating, and acknowledgement. The supervisor uses it to evaluate an employee's work quality, attendance, reliability, communication skills, judgment, initiative, cooperation, knowledge of their position, and training over a review period. Ratings are given to indicate if the employee exceeds, meets, needs improvement, or is unacceptable in each category. Goals are then set and an overall assessment is made of the employee's performance level.
