    AFFIDAVIT OF LOSS

    I, SHELLA MAY A. PESCO, of legal age, Filipino, married and a resident of Combado,
    Maasin City, Southern Leyte, Philippines, after having been duly sworn to in accordance with
    law, depose and say:

    1. That I am a lawful holder of the following identification cards, debit and credit cards:
    a. Postal Identity Card No. PRN I66180701950
    b. UMID ID No. CRN-0111-4269685-0
    c. BDO debit card with account no. 006130292553
    d. Union Bank debit card with account no. 1094 2678 7800
    e. BPI Savings debit card with account no. 5736001995
    f. BPI Amore credit card

    2. That on November 21, 2022, my wallet was stolen, along with the aforesaid IDs, ATM
    cards and credit card;

    3. That I have exerted my best efforts to locate and recover them but despite diligent
    search made, my efforts proved futile. I now consider the same irretrievably lost;

    4. That the said ids and ATM cards have not been confiscated by any officer of the law or
    by the issuing agency and/or banks due to violation of any law, rule or regulation;

    5. That I am executing this affidavit to attest to the veracity of the foregoing, and for
    whatever legal purpose this may serve.
