    AFFIDAVIT OF TWO DIS-INTERESTED PERSONS


    We, AUREA C. FAGEL, widow, of legal age, Filipino Citizen, with residence
    and postal address at #03-B Abellera St., Middle Quezon Hill, Baguio City, and
    EMILIA B. FELIX, of legal age, Filipino Citizen, with residence and postal
    address at #03-B Abellera St., Middle Quezon Hill, Baguio City, Philippines
    after having been duly sworn to in accordance with law hereby depose and
    state under oath, that:

    1. We personally know MARTIN C. BALINGIT Jr., who died on April 21,


    2022 at Datakan, Kapangan, Benguet;

    2. Said person is a member-consumer of the Benguet Electric Cooperative


    (BEENCO), however, in his records with the said office, his last name
    registered is “BOLINGIT”;

    3. His correct last/family name is “BALINGIT” and this spelling he’s been
    using since his lifetime until his death. Maybe when he applied for
    BENECO membership, either it was him who inadvertently wrote his
    name incorrectly or it was the BENECO personnel who made the
    inadvertence;

    4. With the foregoing circumstances, the name “MARTIN C. BALINGIT,


    Jr.” and “MARTIN C. BOLINGIT, Jr.”, pertains to one and the same
    person;

    5. We know the above facts and circumstances as we are long time friends
    of the aforementioned person;

    6. We are executing this affidavit to attest to the truth of our foregoing


    statements and for all legal intents and purposes it may serve.

    IN WITNESS WHEREOF, we have hereunto set our hands this 6th day of March,
    2023 at Baguio City, Philippines.
