We would like to offer our sincere appreciation to everyone who helped us with this report. First
and foremost, we want to express our gratitude to our Almighty God for providing us with
wisdom and guidance. We were on the point of giving up until God extended his hand to us and
walked beside us all the way to the end. This Weekly Accomplishment Report proved to be a
challenging task.
We'd like to thank everyone in this section for their effort, patience, and hard work. Nothing
would be possible without each other's willingness to persevere. Furthermore, we want to show
our gratitude to our family, who has provided as a source of motivation for us and has always
supported us to make the best of any situation. Our deepest appreciation for always being there
for us, and for your everlasting love and unending physical, emotional, and spiritual support.
We would also want to express our gratitude to our professor, Mr. Ralph Anor, for guiding us
through the Documentation and resolving our concerns despite the fact he was teaching in a
new teaching system as result of the pandemic is really appreciated. Furthermore, we value
everyone's patience and would want to use this opportunity to thank the entire class for
entrusting some working students with their learning this semester.
Finally, we'd like to thank Arian Kyle Bagaporo and John Marco Dador, representatives of our
subject committee, for aiding us in the production of our Weekly Accomplishment Report, for
guiding and reminding us throughout the process, and for responding to our concerns.
We are really grateful that we managed to complete our accomplishment report within the time
given by our professor.
This report cannot be completed without the effort and cooperation. We hope our

TO TEACHERS: Everything you've done for us has been much appreciated. You shared what
you knew with us in order to teach us. Thank you for not giving up on us, despite the fact that
you were exhausted at times. In these uncertain times, everyone is going through a difficult
mental or physical phase, and we all need someone to offer us hope and encouragement to
keep attempting to normalize the situation. Today, I'd want to express my gratitude for being the
backbone of the educational system and for continuing to put in long hours, especially in these
trying times. Even though it becomes overwhelming for you on a regular basis, we are happy to
have your presence to assist us through this scenario.
Hats off to you for constantly lifting our spirits and showing us how to overcome obstacles
without giving up.

TO STUDENTS: Students' lives are the happiest in a person's life. During this time, we must
remember the three D's of success: dream, discipline, and dedication. Dedication is the answer
to almost all problems in life. It has many meanings, but they all point to a single devotion to
something in life. The same can be said for our students' lives: if we put more emphasis on
dedication, we will be able to achieve our future goals. Dedication is the path to happiness. It
will help you stay consistent in your pursuit of your goal, and if you lack dedication for your
work, you will abandon it too soon.

We accomplish this semester because of our hard work and also to our professor who guide us
and help us to understand this subject. To our student council that we guide us as a freshman.
We have a good role model that we can follow the steps to our Program Head Mr. Fritz A. Perez
and Ms. Mikee S. Marbacias our Program Coordinator. To our NSTP Professor Mr. Ralph C.
Anor your passion for your student in making us to learned is what we are today because of your
continuous effort to direct us toward the right path. As our professor, you have always
encouraged your student that we will do better in exams. We are encouraged and want to be get
encouraged by you always. You are an incredible professor with high knowledge of both wisdom
and great studies. You have a great sense of mind which makes studies so fun and easier. Your
motivation and inspirational words have made us learned our lessons. Thank you for allowing us
to question without judging and for having growth mindset with us. School, colleges and
universities don't achieve acclaim because of their historical buildings but because of the
professors like you Sir Anor you are excellent mentor. Thank you for an adventure semester.
And to my fellow students, I would like you to us great all for passing this challenging semester
and thank you for the participation of each and every one in our daily groupings even though we
are in Online class we always find a way to participate to our daily workloads.
INTRODUCTION
This accomplishment report talks about the summary and performance for this semester. It also includes the
process of weekly activity and weekly documentation on how our class going thru from start to the end.
During our first week of online meeting with our professor Mr. Ralph Anor. He discusses about the rules and
regulation for our subject and some technical problem specially the poor internet connection but as a student
we try our best to understand all the important lesson for us to learn more about our topic. In the other week
professor Mr. Ralph Anor told us to do some documentation about our weekly meet by group that there will
always be a group that will summary all the important things we do in that specific day the time in and out
the activities, quizzes, presentations, pictures and lessons that professor Mr. Ralph Anor discuss for our
weekly documentation. In this whole semester we tackled about NSTP (national service training program)
the vision and mission the important meaning and its different component, laws and how this subject will
help us as a student.
OBJECTIVES
The primary objective of the NSTP subject is to promote our role in nation-building. As such, it
aims to encourage us to become civic and/or military leaders and volunteers whom could be
called upon by the nation in cases their services are needed. Another objective in this NSTP
subject is to gain knowledge that will benefit us in the future, knowledge not only about the
course subject matter, but also more things about volunteerism and serving the country. The
NSTP aims to promote and integrate values education, transformational leadership, spirit of
patriotism and nationalism and sustainable social mobilization for youth development, community
building and national security. It is a program where we aim to develop our physical, moral,
spiritual, intellectual, and social well-being. We want to learn on how to be a good leader and
how to be a good student and to enhance our skills in basic leadership.
Learning NSTP-LTS could give hope and inspiration to the children who are not privileged to
have a quality education. NSTP served as an eye opener to us students, as we become aware of
the current situation of our country which is poverty. We want to be knowledgeable at enhancing
civic consciousness and defense preparedness by developing the ethics of service and
patriotism while undergoing training in any of its program components. We want to set one’s
sight on being some service-oriented and responsive students who will be of help to the
locality/community. We are objected to enhance our basic leadership so as to prepare for the
future because we are not only going to teach inside the classroom but also to help, volunteer
and lead in the country in times of need.
ACCOMPLISHMENT
For my first year as a freshman, this subject and semester had been a swift but tough ride. In
general, NSTP taught me about its goal and relevance, as well as its three components. I
completed numerous activities in this subject and completed them on time. And the most
important thing is that I learned how to be a good citizen. The National Service Training Program,
is of great importance to being a college student. It helps us to be open to the things around us.
It enhances our morals and values and promotes our holistic development. We've tackled a lot of
things in a span of five months. and in those months, we knew more things on disasters and
calamities such as flooding, typhoons and earthquakes. We learned how to extend our hands to
others through practicing leadership and disposal management. we helped, not only our
fellowmen, but ourselves as well for we were able to enhance our core values and build our
character.
My whole semester wouldn’t be that successful if I didn’t experience a roller coaster ride. In that
ride, I experienced cramming due to many activities given, there are times we have a freedom to
pause for a moment from the activities and projects. But thankfully we have a professor that
could lighten up our heavy topics and schedules that motivates us to do our tasks. The discipline
that our professor gives makes us more responsible students and that is how my whole semester
has been successful.
PROBLEM MET
The first issue we encountered was the limited capacity of our classmates' internet connections,
as some of us only used mobile data, making it difficult to keep up with the lessons our teacher
was discussing, as well as a problem with their microphone that caused them to not understand
our teacher during attendance. Second, the usage of Zoom meetings since some of us had
difficulty using it and it is only restricted to 40-minute meetings, so when Sir Anor didn't notice the
time left, the conversation was abruptly cut off. The third issue is that several of us have reported
that our activities are not uploading or loading properly but have yet to be uploaded to our
Facebook page. Fourth problem was our classmates who doesn't participate in every group
activity. The fifth problem was the students who understand poorly about the discussion of Sir
Anor causing them lack of attention during meetings. Also, the distraction causing by the
environment that us, students, are into that causes to loss focus during meeting. Lastly, the
boredom of every student when they don't like the lesson that our teacher discussing.
SUGGESTED SOLUTION
Our suggestion to this kind of problem that we encountered is to save money for your online
class if you are using mobile data for your load and we suggest this sim card called “DITO” your
100 pesos is enough for 1 month since it has 10GB and it also has fast connection for you to
keep up with the lessons and discussions. Second is the time limit of the zoom meeting when the
40-minute call has finished. Sir. Anor will send a new link for us to continue and join his
discussion. Third is the uploading of activities to our Facebook. If we can’t upload our activities,
we ask a favor to our classmate that can upload our activities or reports. We suggest that when
Sir. Anor wants to upload activities or reports to our Facebook page, you can upload earlier
before the deadline. Sir. Anor also gives us time to do our activities and report. In the fourth
problem we suggest that if one of your group members is not cooperating or who doesn't help
you in any activities and report you should remove him or her in the list so it won’t be unfair to
those members who help you do the task that your professor gives you. For the fifth problem we
suggest that if you poorly understand or can’t catch up with your professor’s discussion don’t be
shy or hesitate to ask questions to your classmates or to your professor for clarification. Also you
can study the lesson again so you at least might understand a little. If you encounter distractions
in your environment, nicely tell them that you have a discussion that is on- going, you can also
go to a place that is quiet so you can understand the teacher and the lesson clearly. And if you
feel bored just take down some notes that will help you focus more on the discussion.

Detailed Report: Detailed Content On our first weekly report, this documentation will
tackle more about the journey of Saint Vincent College of Cabuyao. It's not quite hard to
understand and to know how the SVCC started from a little school into an Academy. The founder
it self of SVCC which is Mr. Simeon and Mrs. Marina Chavez have a best and quality insights for
us as a student. They build the SVCC for those Cabuyeño children who have eager to learn
without spending a lot of money. Because Mrs. Chavez believe studying doesn’t need to be too
expensive. On our discussion, it illustrates on us how far the SVCC has come after two decades
of service in the community. The SVCC continues to expand their property due to ensure the
comfort area and safety of its ever growing population. SVCC also offer over twelve courses
such as: - Bachelor of Sciences in Informational Technology - Bachelor of Sciences in
Criminology - Bachelor of Elementary Education, Generalist - Bachelor of Early Childhood
Education - Bachelor of Secondary Education - Major in English - Major in Filipino - Major in

Detailed Report: Details contend on our second week report, are documentation about
the introduction of NSTP law. First we tackled what is NSTP, what are the components of NSTP.
(NSTP)Law or National Service Training program / RA9163 is for tertiary level of
student. Amending to the purpose Republic Act no 1706 and other purpose was enacted last
January 2002 to expand. And the 3 components are ROTC, or
Reserve Officers training corp. It is tackled about the study of military, police
man Inc. About L TS, or Literacy training service it is studying about the
volunteers students who was teaching like teachers officers or others that
helping youth society and the last components I CWTS or civic welfare training service program.
Here is the details of Civic Welfare Training Service (CWTS)
at this program about designed to enhance the civic consciousness of the
students by developing the value of service and commitment for the welfare of
life all members of the community. NSTP - CWTS 102 (practicum) is intended
for intensive community exposure for thorough implementation of community
plans and project with will develop the student’s value of commitment and
resourcefulness. CWTS, an NSTP component makes us realize that it is not
about fulfilling our academic requirements but serving while we are still
students, usually for the community welfare which we all belong in. Community is a group of
people, interconnected with each other and are living in the same
locality. We can’t just invade a community and do whatever we can. We must
first make a community profile. It is a comprehensive and structured set of
information describing the characteristics of a community. It is important that we know where we
are going. We must also identify what help to give, one that would be beneficial to all.

Detailed Report: This documentation will focus on the rules and policy in taking our
prelim examination. The
first thing that we do is to join the zoom meeting for clarifications regarding the prelim exam. All
in all, we are fifty-three (53) in NSTP Class, Sir Anor confirmed that there are three (3) students
who don't have a permit Sir Anor clearly said that they should leave the meeting immediately
because they shouldn't take the exam for today since we are implying the "no permit no exam
policy" in his class but he will give a special exam which is next week once they complete the
payment for they're prelim. Then Sir Anor started discussing the rules in taking his prelim exam
which is if the answer is correct but the system takes it as wrong we shouldn't worry because
he's
going to check it one by one to avoid confusion for others then if we cannot type our answers
completely we should type the main answer or the first word. Sir Anor stated that if we didn't
attend his class for today's schedule we are no longer have the chance to answer his exam even
if
we have a permit we should follow the "no attendance no exam". Then later on the discussions
went well and that's the time when Sir Anor gave us the class code he noted that we shouldn't
send the class code in our subject group chat he also said that we have limited time to answer
the exam which is one hour and twenty minutes only. That's the start when we open our
Edmodo app to start answering our exam. The prelim exam contains 12 questions it involves an
essay and multiple choice. Then while we are answering our exam there's one student who
sends
the code to our subject group chat which is against the rules in short even we answer the exam
our score is still recorded as zero. Sir Anor has the right to give us zero because at the beginning
of the discussion he already said the things that we shouldn't do while answering our prelim
exam.

limitations, and life motivation. It will also alter your mental condition. Because you have a good
decision to make, self-awareness will help you to be in a good mood. It will assist you in
increasing your self-awareness. They will argue that you cannot be an effective leader unless
you can develop your self-awareness. And in every decision you make, put God first and let God
lead you. The next topic is citizenship training. This lesson will help you develop your character

INTRODUCTION:
Detailed Report: We tackled this week about drugs, what is a drug? Drugs are such kind of
medicine that a stressed or a bored person can take every time. Drugs have many variants,
cannabis, heroine, ecstasy, depressants, stimulants, hallucinogens inhalants and many more.
Some of these drugs are legal on some countries like the cannabis or we also known as
marijuana, it can use as a treatment for Alzheimer's disease, Appetite loss, Cancer, Crohn's
disease, Diseases effecting the immune system like HIV/AIDS or Multiple Sclerosis (MS), Eating
disorders such as anorexia, Epilepsy, Glaucoma. Medical marijuana uses the marijuana plant or
chemicals in it to treat diseases or conditions. It's basically the same product as recreational
marijuana, but it's taken for medical purposes. But there are also negative effects of using
marijuana if it is not necessary. Over using of marijuana can cause long-term or possibly
permanent adverse changes in the brain. See how it may affect in our mental health if we don’t
use it properly. That’s why there are a lot of teenager now a days that has no paths in life
because they are addicted to marijuana which leading them to waste their lives. Why do people
use drugs? Other people, teenagers see lots of people consuming various substances. They see
their parents and other adults drinking alcohol, smoking or vaping and sometimes, trying other
substances. Also, a teenager’s social scene often revolves around drinking and using marijuana.
Sometimes friends urge one another to have a drink or smoke pot, but it’s just as common for
teens to start trying a substance because it’s readily available and they see all their friends
enjoying it. In their minds, they see drug use as a part of the normal teenage experience. Escape
and self-medication, when teens are unhappy and can’t find a healthy outlet for their frustration
or a trusted confidant, they may turn to chemicals for solace. Depending on what substance
they’re trying, they may feel blissfully oblivious, wonderfully happy or energized and confident.
The often rough teenage years can take an emotional toll on children, sometimes even causing
depression, so when teens are given a chance to take something to make them feel better, many
can’t resist. For example, some teens abuse prescription medicine to manage stress or regulate
their lives. Sometimes they abuse prescription stimulants to provide additional energy and the
ability to focus when they’re studying or taking tests. Others are abusing prescription pain

relievers and tranquilizers to cope with academic, social or emotional stress. Boredom, teens
that can’t tolerate being alone had trouble keeping themselves occupied or crave excitement are
prime candidates for substance use. Not only do alcohol and marijuana give them something to
do, but those substances help fill the internal void they feel. Further, they provide a common
ground for interacting with like-minded teens, a way to instantly bond with a group of kids. And
Rebellion, different rebellious teens choose different substances to use based on their
personalities. Alcohol is the drug of choice for the angry teenager because it frees them to
behave aggressively. Some teens abuse prescription medicine to party and get high.
Hallucinogens are also escape drugs, often used by young people who feel misunderstood and
may long to escape to a more idealistic, kind world. Smoking cigarettes can be a form of
rebellion to flaunt their independence and make their parents angry. The reasons for teenage
drug-use are as complex as teenagers themselves. And last is lack of confidence. Many shy
teenagers who lack confidence report that they’ll do things under the influence of alcohol or
drugs that they might not otherwise.
This is part of the appeal of drugs and alcohol even for relatively self-confident teens; you have
the courage to dance if you’re a bad dancer, or sing at the top of your lungs even if you have a
terrible voice, or kiss the girl you’re attracted to. And alcohol and other drugs tend not only to
loosen your inhibitions but to alleviate social anxiety. Not only do you have something in common
with the other people around you, but there’s the mentality that if you do anything or say anything
stupid, everyone will just think you had too many drinks or smoked too much weed. There are
some ways to prevent using of drugs. First is get involved in some distracting activity. Think of
some activities that can help you to lose your focus on drugs or stop you for trying it. When you
are busy on other activities you can no longer think of trying drugs because you do not have time
for it. When you engage in other activity you take your mind out of using drugs. Some examples
are doing a exercise, reading a movie and readings. Next is talk it through, cravings are likely
going to be a part of experience when you are recovering. If you're used to use drugs the
moment you have a yearning for them, it may be hard to say no to cravings. Discovering an

effective distraction for cravings can be a challenge but with help of your friends or family you will
be able to manage those cravings. Talking about what you feel when you are having cravings
can help you feel more in control. It is great to have a companion when you're quitting from
drugs, your love once will do. Next is urge surf, the practice of urge surfing is a mindfulness
technique, which means it involves bringing your full attention to the present moment.
Mindfulness practice which has been shown to reduce feelings of anxiety, ease depression
symptoms, increase self-esteem, and develop greater personal awareness. Urge surf is a
technique that can be used to avoid acting on any behavior that you want to reduce or stop. The
last one is the challenge and changes your thoughts, you may have quit drugs months ago, but
you still find yourself craving from it. It's common for former users to become triggered by the
time of day, places, activities, emotions, and other people who smoke, but challenge yourself
that it would not be helpful to you if you continue using drugs. Remind yourself all the negative
effects of it.

Introduction:
Detailed Report:
We tackled this week is all about the 1987 Constitution of the Philippines and all the other past
constitution that was being declared. Constitution is a system for government or what we call
‘law’. The Constitution is the supreme law of the Philippines it explains the rights of the country.
The purpose of the constitution is to give sovereignty to the state or the government to protect
peace and freedom also to make the country stable. The kinds of Constitution was also being
discussed first is the origin and history it is needed to consider the past or previous constitution.
Second its form how was it form if we trace wherein and it is still included in terminologies. Third
is amending them the reason why it is amended to have a heavy punishment and it is
accordingly to our generation. Statute is a law that Legislative Department passed which is from
the Congress and the House of Representative. The Congress reviews what the House of
Representative because they make the law. The Law in the Philippine is not equal why?
Because it’s the reality if you don’t have money or connection from them they will not help you.
The Constitution has six constitution since the proclamation of Independence on June 12, 1898.
The 1899 Malolos Constitution the First Republican Constitution in Asia during the presidential of
Emilio Aguinaldo the separation of the church and the state and had civil rights. The 1935
Commonwealth Constitution presidential of Manuel Quezon it was being Used till 1946 with the
help of America in 1899-1935 the Philippine had the freedom. The 1943 Constitution presidential
of Jose P. Laurel this constitution was being used by many president. And the three Branches of
Government the Executive, Legislative and Judicial Department was being powered. Also it has
twelve articles and the Preamble. 1973 Constitution or Martial Law presidential time of Ferdinand
Marcos the time that a president can rule for 6 years and can be unlimited nominated. 1986
Constitution or 1986 Provisional “Freedom Constitution” or the temporary freedom. 1987
Constitution or the present constitution that we used presidential time by Corazon Aquino. Sir
Anor also asked us to recite the preamble and explain what it is. A preamble is the introduction of
the Institution what are the contents and it is determined.

Introduction: The cabinet of the Philippines, members cabinet of the Philippines and
officials’ cabinet level-rank.
Detailed Report:
Our lesson is all about the cabinet of the Philippines, cabinet members, and cabinet-level
officials. The cabinet is an advisory body composed of the heads of the 22 executive
departments, They are nominated by the president and then presented to the commission on
appointments a body of the congress of the Philippines that confirms all appointments made by
the head of state, for confirmation or rejection. It is an organization that aims to improve the
country, help those in need and guide every Filipino. After the discussion, Sir Anor assigned us
an activity in which we must conduct research or gather information about the executive branch,
legislative branch, and judicial branch. And we need to find or search their duties,
responsibilities, and functions of the president, and vice president of the Philippines, the
governor and vice governor of Laguna, as well as the mayor, vice-mayor, and barangay
chairman in every location. And also include the 24 senators in our senate as well as each
representative of 307 committees district. The judicial branch, which includes the Supreme
Court, Court of Appeals, Sandigang Bayan, Court of Tax Appeals is the final component of our
activity. Before we leave in our zoom meeting Sir. Anor stated that we must be creative and
include images of those that participated people in this activity, as well as we need to upload and
post our work on Facebook on our page in the National Training Service Program.

Detailed Report: The time in and time out of this documentation are dated on the 18th
day of November 2021 because we are unable to document the time in and time out on the day
of the pre-final exam since our group misunderstood what weekly report we will do. But we
manage to make a way to make this weekly report by capturing the time in and time out on the
said date below the picture and make the detailed report based on what we remember on the
day of the pre-final exam. The time in was 2:49 because sir Anor told us that he will be
discussing by 2:00 pm and the time out was 3:49 pm it is an early out since he only gave us what
to do on the accomplishment report. This documentation focuses on the rules and policy in
taking our pre-final examination. The first thing that we do is to join the zoom meeting for
clarifications regarding the pre-final exam. All in all, we are fifty-three (53) in NSTP Class, Sir
Anor confirmed that there are three (3) students who don't have a permit so Sir Anor clearly said
that they are prohibited to take the exam for today since we are implying the "no permit no exam
policy" in his class but he will give a special exam which is next week once they complete the
payment for their pre-final. Then Sir Anor started discussing the rules in taking his pre- final
examination which is to be careful about the symbols and spellings we are going to type because
if it has symbols or wrong spelling, the system or NEO-LMS will incorrect it. Sir Anor stated that if
we didn't attend his class for today's schedule, we are no longer have the chance to answer his
exam even if we have a permit because the rule was, we should follow the "no attendance no
exam". The discussion was short because Sir Anor just gave the instruction and what time we
will be taking the exam. He also said that we have limited time to answer the exam which is
starting from 2:20 to 3:30 in the afternoon total of one hour and ten minutes only. That's the start
when we open our NEO-LMS account to start answering our exam. The Pre-final exam contains
19 questions that can be answered by true or false, multiple choice, matching type and fill in the
blanks.

INTRODUCTION:
DETAILED REPORT: Today, Sir Anor checked our Accomplishment report, but
before that, Sir Anor did not check the attendance because he went downstairs to check
something and said he would be back in 5 minutes, and after 5 minutes of waiting, Sir Anor
proceeded to check our accomplishment report and said he would check the attendance later
after checking the accomplishment report.
After that, Sir Anor goes over the file and sees some errors, such as the margins, font style, font
size, spelling, and class photographs. Some parts are incorrect and empty, and he edits it so that
we can learn how to do it correctly and easily. He teaches us some steps to make it more
detailed and presentable, and after he checks and edits those pictures, he notices that the

incorrect and that it would create some changes when we edit it because once we add some
photographs and a detailed report, the other page will become sloppy and disordered. He added
that we should do it organized and edit it week by week
rather than leaping onto another page. After that, one of my classmates inquired about the
weekly report, and there was an issue and misunderstanding. And as a result of what happened,
our Zoom meeting is ended unexpectedly

capital letters and we can only take the test once for various reasons, specifically speaking to
avoid cheating. The examination only last for an hour, finish or not finish the examination will
end. Yes our prof is strict, but this is for us, we have to be well disciplined and productive about
the time given to us. Cheating will lead to zero points, we have another important policy, “No
permit no exam”. The given examination may contains fill in the blanks, enumeration, multiple
choice, matching type and essay
