The document provides instructions for a clinical skills evaluation on urinary catheterization, outlining 19 steps of the procedure including preparing supplies, positioning the patient, inserting the catheter, inflating the balloon, collecting a urine sample, and documenting the procedure. It also lists components to evaluate such as introducing oneself to the patient, explaining the procedure, and ensuring privacy.

Not done Incomplete Correct (0) (1) (2) 1 Student greets and introduces self to patient. Confirms patient identity. 2 Student explains procedure, obtains consent and notes any allergies to cleansing solution. 3 Ensures Privacy and asks for Chaperone.
4 Gathers Supplies (Urinary Catheter, Urine Bag,
Cleansing Solutions, Lubricant/Local Anesthetic, Sterile Gloves, Sterile Drape, Urine Collection Container etc.) and Washes Hand. 5 Ensures the correct sized catheter is not expired and checks adequacy of the balloon tip. Explain how to determine correct size for patient age group. 6 Draws required amount of Water for Injection into syringe. Check Catheter specification – commonly 10ml.
Draws Local Anesthetic into another syring.
7 Clamps the end of the catheter to prevent leakage.
8 Positions patient, wears sterile gloves and puts drape
around the penis/vulva. Explain positioning of male vs female. 9 Student cleans genitalia, moving from cleanest point outwards. Avoid use of methylated spirit (irritant). Males: Retracts foreskin, holds penis below glans and cleans outwards in a circular fashion from the meatus to the base of the Glans. Repeats two more times. Female: Uses non dominant hand to expose meatus and cleans from front to back (Left side, Right side and center) 10 Inserts Anesthetic Gel into uretheral meatus (if sterile). Apply on the Catheter if not sterile. 11 Asks patient to bear down and advances catheter. Full length of the catheter should be inserted. 12 Student places the end of the catheter into the urine container and releases clamp to collect urine sample. 13 Student inflates the balloon and verifies retention gently 14 Student attaches urine bag to end of catheter and hangs below the level of the bladder to facilitate gravity drainage 15 Tapes the tube to patient’s thigh
16 Thanks patient, provides gauze and allows patient to
dress up. 17 Cleans up area and washes hands. 18 Does a Marcoscopic examination of the collect urine sample, labels sample and expresses need to take it to the lab for MCS and Biochemistry 19 Documents Procedure (Date, time, indication, size and type of catheter, amount of water for injection instilled into the balloon, sample macroscopic findings, approximate amount of urine in bag etc.) Comments:
