After the conduct of investigation, the company shall prepare and submit work accident
report using the prescribed form (WAIR). Moreover, other work accidents resulting to
disabling injuries such as Permanent Partial Disability and Temporary Total Disability
shall be reported to the DOLE Regional Office within 30 days after the date of occurrence
of accident using the DOLE prescribed form (WAIR).
All near misses shall be recorded and reported. A system for notification and reporting
of work accidents including near misses within the company shall be developed and
reviewed by the OSH Committee as necessary.

12.0 Dust Control and Management
Activities such as building of temporary structures and lifting and operation of
electrical, mechanical, communications system and other requirements
Dust Control Procedures:
1. Regular Cleaning: Establish a cleaning schedule to ensure that dust is regularly
removed from work areas, equipment, and surfaces. This includes dusting,
vacuuming, and using appropriate cleaning agents.
2. Ventilation Systems: Install and maintain effective ventilation systems to control
dust levels and improve air quality within the facility. Regularly inspect and clean
ventilation filters and ducts.
3. Personal Protective Equipment (PPE): Provide appropriate PPE, such as masks or
respirators, to employees who are exposed to high levels of dust during their work
activities.
4. Dust Collection Systems: Implement dust collection systems or use equipment
with built-in dust collection mechanisms to minimize the release of dust particles
into the environment.
5. Employee Training: Conduct regular training sessions to educate employees about
the importance of dust control, proper cleaning techniques, and the use of PPE.
