FORM C1 – CAPABILITY AND EXPERIENCE

Bidder shall complete the below questionnaire relative to its capability and provide evidence in the terms of
previous projects where such services and capabilities have been delivered and/or provide samples or
reference/contact details relative to the demonstration of such capabilities. Bidder shall also state clearly in the
response to this questionnaire whether the delivery of the Services has been done directly by the Bidder using in-
house resources or is typically outsourced to subcontractors or consultants.

In relation to the evidence (project examples or references), Bidder shall limit its response to four (4) pages of A4
paper.

FORM C2 - SUBCONTRACTORS

Bidder shall provide its responses to the below questions relative to the selection and nomination of key
Subcontractors or Suppliers whose are planned for this scope of works. For each listed or named Subcontractor
or Supplier, the Bidder shall demonstrate to the satisfaction of the BEJV that the Subcontractor or Supplier has
the relevant financial, technical capabilities and experience to perform the Works or Services to be
Subcontracted. The answers to the following questions shall not exceed four (4) pages of A4 paper.

FORM C3 - CONTRACT SCHEDULE

Bidder shall submit a Preliminary Contract Schedule in the format specified below. The Preliminary
Schedule shall clearly demonstrate the critical activities for delivery of the Works and illustrate the
Bidder’s understanding and ability to sequence and the Works to meet the Contract Milestones.

Bidder shall identify at least three major problem areas that may affect successful contract completion
and Bidder's proposed resolution of each problem should it occur. Bidder’s response shall not exceed
3 pages of A4, showing the risk, mitigation plans and contingency planning allowances within the
schedule to manage the risks on delivery of the Works.

Bidder’s Preliminary Schedule for its work activities and deliverables shall include the following
attributes, as a minimum:

● Start and finish dates for each major activity included in Bidder’s scope of work.

NOTES:

This form is for bid evaluation purposes and will not be a part of the contract. However, BEJV may, if
the Preliminary Schedule is compliant with the ITB Documents requirements, incorporate the
Preliminary Schedule into the Contract as the baseline Schedule for execution of the Works.

FORM C4 - PROJECT ORGANIZATION AND WORK PLAN

Part 1: Project Organization

Bidder shall attach an organization chart showing key personnel, including numbers, titles and
categories of home office and field personnel. Bidder shall also provide a written description of the
organization, defining lines of authority / responsibility / communication and the overall working of the
organization with particular emphasis on Home Office / Site interfaces and the procedures for
monitoring and controlling the Works. Response to not exceed four (4) pages of A4 (A3 may be used
for the Organization Chart)

Part 2: Work Plan

Bidder shall attach a narrative, not to exceed four (4) pages (A4), describing its plan for conducting
the Work under this contract. This plan should be developed in conjunction with the preliminary
schedule, organization chart, resumes, personnel tabulation (staffing plan by month for each required
position), equipment list and proposed suppliers / subcontractors to knit this information together and
explain the management and flow of work to contract conclusion. Additionally, Bidder to confirm it has
the staffing available to execute as planned.

The Plan shall include but not be limited to the following activities:

SAMPLE ONLY
● Compliance with Company Procedures;
● Detailed methodology (including any baseline studies proposed);
● Access to work sites;
● Access to stakeholders;
● Execution of the work; and
● Staffing resources available.

Bidder shall, in addition to the above narrative provide a specific Work Plan that shall identify the
resources (plant, labour and facilities) to be mobilised for performance of the Work together with a
specific schedule describing the mobilisation activities. This should include:

● Mobilisation of Key Personnel


● Mobilisation of Equipment (if intended)
● Use of Temporary Offices and Accommodation (if intended)
● Stakeholder engagement activities

NOTES:
This form is for bid evaluation purposes and not intended to be a part of the Contract. However,
BEJV may elect to incorporate it into the Contract if during bid clarification/negotiation meetings, the
Plan is finalized and agreed to as the baseline for execution of the Works.

FORM C5 - RESUMES OF KEY PERSONNEL

Bidder shall provide a resume for each key person to be assigned to the contract for each position
listed in the following table that meets or exceeds the requirements specified therein.

Each Resume shall be provided corresponding to the named individuals for the Key Positions listed
and each Resume shall not exceed two (2) pages of A4 size paper highlighting relevant experience
on similar projects.

roles (maximum 1 page A4)

Key personnel will be listed in the Terms & Conditions Clause titled "KEY PERSONNEL", restricting
their replacement without BEJV’s consent.

FORM C6 - QUALITY PROGRAM

Bidder shall provide a summary description of the Quality Assurance / Quality Control (QA/QC)
Program to be implemented in performance of the Work. This summary shall include information on
the organization of the program, including the following and be limited to four (4) pages of A4:

● Authority and responsibility of all involved personnel in the QA/QC Process.

● QA/QC Manager/Engineers reporting to the Project Manager

● Administrative policies and procedures to be used in carrying out the program to demonstrate
how all work activities performed by the Bidder will conform to the applicable quality and
contract requirements.

● ISO 9001 certificate and its Scope of application (or applicable Third Party certifications)

NOTES:

This form is for bid evaluation purposes and will not be a part of the contract. Final program will be
approved as submitted under the contract requirements.

FORM C10 - EXCEPTIONS AND DEVIATIONS

Bidder submits herein any exceptions and deviations it wishes to make to any part of the Invitation to
Bid (ITB), taking care to refer precisely to which part of the ITB Documents the exception or deviation
is being made. All exceptions and deviations shall be submitted in the form of amendments to the ITB
Documents and each must be listed separately. No exceptions or deviations to the ITB Documents
shall be recognized unless expressly set forth herein and all other terms and conditions of the ITB
Documents remain in full force and effect except as modified herein. If no exceptions or deviations
are proposed by Bidder, it shall so state below. Failure to complete this form may be cause for
rejection of Bidder's proposal.

FORM C11 - SAFETY PROGRAM

Bidder shall also submit the following Safety and Health data for evaluation:

1. Safety Program: A copy of Bidder’s written Safety Program or Plan. Depending on scope,
bidders can submit a sample Occupational Safety and Health Plan to satisfy this requirement.
(Refer to Article 6 and Annex 2 of Regulations on the Minimum Safety and Health
Requirements for work on Temporary and Mobile Construction Sites).

2. Safety and Environmental Program Accreditation: A copy of Bidder's 3rd Party safety and
environmental accreditation (if any, e.g., ISO 45001, ISO 14001).

3. New Hire Orientation Program: A copy of Bidder’s written Safety Induction Program.

4. Risk Assessment: Provide a sample risk assessment and safety statement that meets the
requirements of Article 11 Law on Safety and Health at Work and Rulebook No.
20090020009 on the Manner of Preparing a Safety Statement, its Content, as well as the Risk
Assessment it should be based on.

5. Expert Advice: Provide details of your inhouse or external Occupational Safety Expert and
Authorized Health Institution resources. (Articles 3, 17 and 18 of Law on Safety and Health
at Work)

6. Safety Performance (Article 36 Law on Safety and Health at Work)
