1.0 INTRODUCTION

Project Quality Plan is to describe and set out quantity out quality policies, procedures and practices in
accordance with the requirements for ISO 9001:2015 to maintain an effective Quality Management System
in ABR CONTRACTING COMPANY Projects for Riyadh Project. It is guide of ABR Projects aware of the
commitment and cooperation required to achieve the quality and customer satisfaction.

2.0 GENERAL STATEMENT AND SCOPE


The following is a general description of the Quality Control measures and an overview of the Quality
Control System that the ABR Contraction Company plan to follow and implement in order to conform to the
technical specification, contract requirements and quality standards for the subject project. This plan
provides the mechanism to ensure that the activities affecting quality of the work are accomplished and
recorded in accordance with project specifications and drawings. The project Quality Control plan regulates
inspection, test and other control and identifies personnel and control procedures as part of the proposed
Quality Control organization. This plan shall identify personnel, procedure, control, instructions, tests,
records and form to be used. It is NOT the intent of this manual to preclude the necessity of all individuals
to be professionals and effectively address the documentation of construction inspection in a planned and
orderly manner. The guidelines contained herein must be used in conjunction with the contract documents
and good engineering judgment if satisfactory results are to be obtained.

3.0 RESPONSIBILITIES

The Project director ensures that the required qualities for the construction of the project are fully
complied. He is responsible for the overall project execution, and has the authority to organize and
implement all the activities necessary to achieve efficient and timely completion of the project deliverables.
He shall be responsible for reviewing and approving the preparation, issue, revision and control of QC/QA
program.

The Quality Assurance Department shall ensure compliance with the Project Specifications and see to it
that a Quality Management System has been established, implemented and maintain in accordance with
ISO 9001:2015.

The Quality Control Department shall ensure compliance with the project Specifications and see to it that
Quality Control Works and matters on field has the highest standard of quality achieved by following all
approved and applicable documents. Shall prepare, issue non- conformance reports, revision and control
of QC/QA program.

This Project Quality Plan shall be considered as a LIVING DOCUMENT and therefore will be updated
throughout the duration of the project when needed as necessary and shall be implemented during
the construction phase of the Project.

4.0 QUALITY ASSURANCE MANUAL


4.1 QUALITY POLICY
In a highly competitive market where competence, quality and cost determine a Company’s position, ABR
Contracting Company strives at strengthening customer relationships by providing quality driven services
primarily directed towards customer satisfaction, especially in the fields of Construction, Design and
Supervision.

In order to realize this policy, the Management formulates strategic quality objectives, which are periodically
measured, analyzed and reviewed to ensure their continuous suitability to the business needs. Every
Employee within the organization plays a key role in achieving the set objectives, through innovation and
professional conduct.

A Quality Management System, has been developed to exemplify the Management’s commitment towards
the implementation of this policy. The QMS is designed to permanently:
 Determine customer needs and specify them in the form of defined requirements for the organization,
 Determine, plan, carry out, monitor and adapt the service realization processes,
 Expand the range of corporate experience & expertise,
 Review its performance to streamline internal processes, optimize efficiency and bring forth
improvements.

The Management of ABR Contracting Company is fully committed to ensure that this policy is scrupulously
and efficiently communicated to all levels of the Organization and permanently applied throughout the entire
Company.

4.2 MANAGEMENT RESPOSIBILITY


4.2.1 MANAGEMENT COMMITMENT
The ABR Contracting Company Management, is committed to uphold the Company’s Quality Policy, through
the effective implementation of the Quality Management System.

This commitment is translated through continuous efforts to increase the Customer satisfaction, meet and
often exceed his requirements.

Based on the Company’s short, medium and long-term strategy, Quality Objectives are formalized during
the Management Review Meetings. The Divisions/Departments are then furnished with the proper resources
to meet them. This commitment is evident in the entire Quality Management System.

4.2.2 QUALITY MANAGEMENT SYSTEM PLANNING

Through proper implementation of the Quality Management System, internal QMS audit activities are
organized to be dynamic and responsive to the Customer requirements and Company needs. The audit
results

present a significant input into the structure of the Quality Management System, including the identification
of effective business processes, their sequence and interrelations. Such input may also be the result of
corrective/preventive actions, process capability analyses, measurement & analysis of Key Performance
Indicators, etc...

4.3 RESPONSIBILITY, AUTHORITY AND COMMUNICATION

Every Employee within the organization plays a key role in achieving Customer satisfaction. However, his
assigned responsibilities and authorities are in accordance with his duties. These responsibilities and
authorities are detailed in every Procedure, separately.

The Corporate Management encourages communication across all levels of the Organization. Critical
information shall be shared with all concerned, in order to authorize effective actions, as fast as possible. The
effectiveness of such actions is periodically analyzed.

In order to emphasize this focal point, a documented Procedure “Responsibilities, Authorities and
Communication” is introduced

5.0 RESOURCE MANAGEMENT

5.1 INFRASTRUCTURE

Suitable work environments are created in the offices and at Construction Projects. Occupational Health &
Safety (OHS) working conditions are governed through the “Occupational Health & Safety’ Manual, which is
implemented at Company offices, and construction Projects. Additional requirements are addressed through
specific Safety plans, which are developed as needed, on a Project-by-Project basis.

5.2 RESOURCES

The Company has a strict policy of providing the needed resources, at all levels in the Organization, to ensure
proper realization of the final Product.
The provision of resources is described in the section “Planning for Product Realization”. Additionally, the
following Divisions/Departments ensure the implementation of this policy.

5.2.1 HUMAN RESOURCES MANAGEMENT (HRM)


Provision and management of human resources are described in the following Procedures:
 “Performance Evaluation”
To evaluate and manage employees work performance
 “Recruiting Procedure”
To fill vacant positions through the proper application of the recruitment processes

5.2.2 OTHER RESOURCES

 Technical Support Services (TSS)


As described, previously, in the section “Company Overview’, TSS provides a multitude of services to the
Construction design, supervision, such as transportation, housing, office services, etc.... Documented
Procedures have been established to furnish the Projects with the needed resources, namely:
 “Engineering Missions”
To initiate engineering/design studies
 “Supervision”
To provide standard construction procedure, Quality Assurance, etc... Additionally, the TSSD provides
transportation, housing, office services, etc... To the Company’s Employees.

5.2.3 INFORMATION TECHNOLOGY DEPARTMENT (ITD)

Provides a multitude of computer services to the Maintenance Divisions. Documented Procedures have
been established to furnish the Projects with the needed resources, namely:
 ITD_01 “Computer Hardware/ Software Request”
To initiate the request for Hardware and ready-made Software
 ITD_02 “Computer Equipment Maintenance”
To initiate the request for in-house development of applications
 ITD 03 “Application Software Development’
 ITD 04 “System Data/Backup & Recovery

6.0 PURPOSE & QUALITY OBJECTIVES OF THE PROJECT


6. 1 Have been develop in order to provide a uniform guidelines and standard method for specific
areas of inspection activities.

6.2 Provide procedures and instructions that will be able to help different discipline for installation,
testing, pre-commissioning, examination, troubleshooting, maintenance, repairs and documentation
of the new projects, ongoing projects, and existing projects.

6.3 Develop the Quality Control System and to satisfaction of client’s requirements.

6.4 To guide and familiarize the contractors work including Quality Control for The documentation.

6.5 To implement the Quality Control, Health, Safety and Environmental Standard procedure on
Project site.

6.6 Measure quality performance and identify positive and negative trends.

6.7 Identify and resolve discrepancies relating to contractual project specifications.

6.8 Ensure the Materials installed on site shall be on standard or approved as per specification and
execution work as per approved procedure.

7.0 ACTION (QUALITY SYSTEM ELEMENTS)

7.1 MANAGEMENT AND ORGANIZATION

 The hierarchical structure of the Project Quality Assurance is shown in Appendix. The basic position,
responsibilities and authority related to Project Quality Management detailed below.

7.1.1 Responsibilities

7.1.1.1 Project Director

 The Project Director is responsible for the overall project execution, and has the authority to organize
and implement all the activities necessary to achieve efficient and timely completion of the project
deliverables. He shall be responsible for reviewing and approving the preparation, issue, revision and
control of QA/QC program.
 The Project Director is the one who is primary responsible for the successful execution of the project
which includes the following:

a. Project Time Scheduling

b. Quality Management

c. Safety Management

d. Cost Control

e. Customer Communication

f. In-house Communication

g. Overseeing the smooth flow of all project work

h. Assuring compliance with contract requirements

i. Completion of the Project on time

j. Authorization of Project Correspondence and working drawings.

7.1.1.2 Construction Manager

 He shall be based at the construction site and be responsible for the execution of the Contract from its
interception through to final hand-over to the client. His duties include but not limited to:

a. Ensure the implementation of the Safety Procedures.


b. Ensure Quality of workmanship as per project Quality Plan.
c. Monitoring and advising on labor, material and equipment required at site and ensuring the
efficient execution of the works.
d. Checking and reviewing all the correspondence and documents to be submitted to the
Engineer.
e. Attending the weekly meeting with the Engineer.
f. Reviewing with the Project Manager and the Site Engineers in weekly basis the site progress
and implement corrective actions to keep the progress as per the contractual schedule.
g. Follow up the Contractual matters with the Engineer.
h. Provide adequate accountancy, administrative, technical and consultative services for the
efficient management of the contract.
i. Monitor the project performance in respect of cost and progress.
j. Motivation of the workforce to derive maximum productivity.
k. Implementation of perfect teamwork.

7.1.1.3 QA/QC Manager

a. Report status of the Quality Management System.


b. Establish, implement and maintain process required for the Quality System.
c. Oversee the Quality Management System.
d. Support the Improvement of the Quality Management System.
e. Inform the top management of any need for improvement.

 The Quality Assurance/Quality Control (QA/QC) Manager shall report to the Project Director and
shall be fully responsible for all Quality Control Works and matters on field and ensure highest
standard of quality achieved by following all approved and applicable documents. He shall be
responsible for the preparation, issue, revision and control of QA/QC program.
 He shall develop and maintain the system for documents control, instrument calibration, joint
inspection with client, audit program, NCR control and subcontractor activities regarding quality.
 The QA/QC Engineer shall be responsible for close liaison with the Engineer regarding all the quality
matters.
 He shall maintain an updated library of all applicable documents, codes and standards related to the
project.
 Conduct periodic audits internal.
 Inspection and Test Plan reports.

7.1.1.4 Site Engineer

 Construction of the works at site as per contract drawings and approved working drawings in a
timely manner.
 Ensure that Quality workmanship is implemented throughout the construction works.
 Coordination of his activities with other Site Engineers.
 Assessing the required resources, labor/equipment for the construction of the site activities which
he is responsible for.
 Coordination with the Technical Engineer/Draftsman for the preparation of working drawings based
on actual site conditions.
 Reports directly to the Construction Manager and coordinates his activities with him.

7.1.1.5 Safety Officer

 Implement the Safety Manual at site.


 Implement practices and procedures to avoid the risks of accidents and injuries.
 Inform all workers of any existing Health and Safety hazards on site.
 Provide training and supervision for all workers to ensure that safe working practices are adhered to.
 Ensure that competent Safety Officer carry out constant safety inspections.
 Maintaining accurate accident, injury and "near miss" reports. A "near miss" is a warning of a
possible accident and therefore needs to be recorded to ensure that it does not happen again.
 Attend the weekly Safety meeting with the Engineer.
 Organize the weekly safety meeting at site.

7.1.1.6 Safety Inspector

 Carry out daily inspection at the worksite, reporting and monitoring of unsafe methods and conditions.

 Record keeper of all injuries, fires, road traffic accident, property damage, dangerous occurrences and
any equipment incidents occurred within the worksite.

 Record keeper of the weekly safety meeting report conducted on site by the Safety Officer including
the list of attendees.

 Understand the client’s safety requirements and the Company Loss Prevention Policy.

 Shall be available on the project site at all times for monitoring and supervision as deemed necessary.

 Will set as a personal example on site at all times.

7.1.1.7 Quality Control Inspector

 Responsible for inspecting all works to insure compliance with the contract plans and specifications.

 The Site QA/QC Inspectors must report immediately to the Project QA/QC Engineer when he
observes any work that fails to meet.

7.1.1.8 Foreman
 Following the labor/equipment in his defined area of activity for the proper execution of the
contract works.

 Assist the Site Engineer in controlling site activities and make sure that work is being done to the
required quality.

7.1.1.9 Laboratory In-Charge

 Perform all daily quality routine tests.


 Ensure that all testing equipment’s are calibrated and functioning as expected.
 Make an inclusive listing and schedule of all quality control testing.
 Report immediately to the Project QA/QC Engineer for any tests that fails to meet.

7.2 Terminology

7.2.1 Client : Refer to the end user of the services offered by an agency,
organization or company.

For RIYADH HOUSING PROJECT- Client is NATIONAL HOUSING


COMPANY
7.2.2 Contractor : Company or person under contact supplying labor and materials
to do a specific job.

For RIYADH HOUSING PROJECT-the Contractor is ABR


CONTRACTING COMPANY

7.2.3 Contract : Information about a business agreement between two or more


parties, concerning their legal obligations. Includes ownership,
depository designations and obligations between parties.

7.2.4 Sub-contractor : To give work under subcontract agreement or to pass on work to


a second person or company under the terms of a subcontract.

Quality : General standard in conformance to specified requirement.


7.2.5

7.2.6 Quality Assurance : Activities, action and procedures performed before and during
execution of the work to guard against defects and deficiencies
and substantiate that proposed construction will comply with
requirements.

The total effort of developing, documenting and implementing


policies and procedures and defining roles and responsibilities in
order to achieve and verify quality in accordance with specified
requirements. The program’s objectives are to achieve Customer
confidence and satisfaction and to assure that the design output,
structure, facility, system or component will perform
satisfactorily in service.

7.2.7 Quality Control: Test, inspection, procedures and related actions during and after
execution of the work to evaluate that actual products
incorporated into the work and completed construction comply
with requirements. Services do not include contract enforcement
activities performed by engineer.

The acts of examining, witness, inspecting, checking and testing


and when necessary revising in process or completed design work
including plan sheets, studies, charts and reports to determine
conformity with contract requirements.

7.2.8 Quality Manual: The documentation of the Quality Policy and Quality System
properly followed and implemented by the company.

The procedures and activities used to implement the Quality

result from activities or processes.

7.2.20 Supplier: To provide or to sell, or make available something that is wanted


or needed by somebody.

7.2.21 Procurement Control Information dealing with the procurement, control, movement
and Management : and management of physical items and services.

7.2.22 Availability Information about equipment, material or services concerning


Requirements : their general availability, according to the type, not tied to a
specific requisition. This availability is both to the site where the
item will be used, and to the warehouse.

7.2.23 Delivery Dates: Information about equipment, materials or services concerning


the date (estimated or actual) at which supplier will deliver. This
is different to the on-site date.

7.2.24 Delivery Notes: Equipment and material availability and price: Information
about equipment, materials and services concerning the
availability from supplier for different types of equipment,
material and service.

7.2.25 Packing List: Information about equipment or materials concerning the


quantity and location of items which are to be sent to another
warehouse, or to be provided when a Purchase Order is to be
implemented, or where a requisition is made based on a planned
or scheduled Work Order.

7.2.26 Shipping Details: Information about equipment, materials or services concerning


details of when and how the item will be shipped from the supplier
to the buyer.

7.2.27 Technical Information about desired or planned equipment, materials or


Requirements: services concerning detailed technical requirements, which can
be compared with the supplier specifications to evaluate fitness
for purpose.

7.2.28 Acceptance Criteria: Specified limits placed on characteristics of an item, facility,


process, or service defined in codes, standards, or other design
documents.

7.2.29 Audit: A systematic, independent and documented activity performed in


accordance with written procedures or checklists to verify, by
examination and evaluation of objective evidence, that
applicable elements of the quality program have been
developed, documented, and effectively implemented in
accordance with specified requirements. An audit differs from
surveillance or inspection activities.

7.2.30 Audit Finding: A deficiency or discrepancy of an item, activity, or document


identified during audit and recorded on an Audit Finding Sheet.

7.2.31 Auditor: Any individual who conducts an audit.

7.2.32 Certificate of A document signed or otherwise authenticated by an authorized


Compliance : individual or qualified party attesting that the materials, items, or
services are in accordance with specified requirements and
accompanied by additional information to substantiate the
statement.

7.2.33 Certification: The act of determining, verifying and attesting in writing to the
qualifications of personnel, processes, procedures, materials or
items in accordance with applicable requirements.

7.2.34 Checklist : A list of items required to be considered as part of a quality


procedure. Serving as documentation of completion, a check
list is a required part of the Project record.

7.2.35 Computer Aided Design Software to support engineering calculations and drawings,
and Drafting (CADD) : used to create plan sheets; in particular, for engineering
calculations and Micro Station for drafting.

7.2.36 Packing List: Information about equipment or materials concerning the


quantity and location of items which are to be sent to another
warehouse, or to be provided when a Purchase Order is to be
implemented, or where a requisition is made based on a planned
or scheduled Work Order.

7.2.37 Corrective Action: Measures taken to rectify conditions adverse to quality and,
where necessary, to preclude repetition or an undesirable
occurrence.
A single drawing or a logical compilation of related calculations,
7.2.38 Document: data, report text, design analyses, specification sections, cost
estimates, meeting minutes, or Project-related correspondence
that describe, define, specify, report or certify activities,
requirements, procedures, or results. The medium can be paper,
magnetic, electronic or computer disc, photograph, master
sample or combination of these.

6.2.39 Drawings: The plans or sheets that form the graphical representation of
the designed and planned work, included in the Construction
Documents.

6.2.40 Existing Condition: The surveys, topographic mapping, utility locations, plans for
existing structures and all other drawings and reports defining
existing physical conditions of a site or facility.
Internal Audit: An unbiased and independent audit of the Quality Control
6.2.41 System being employed by a given Organization in order to allow
self-declaration of conformance.
Documents or records in the highest, most complete form in
6.2.42 Original: which they exist in the active project files. This includes but is
not limited to copies of outgoing correspondence, printed copies
of a published document, original copies of calculations, quantity
take-off and other design analyses, original reproducible
drawings, original resident engineer diaries, and field records.
The engineer, architect, planner, designer, or other person who
6.2.43 Originator: develops a specific document. In the case of drawings, the
Originator is the individual who provides the design information,
sketches, and instructions to the CADD operator.

6.2.44 System (QMS) Plans, procedures, work instructions, and records.

6.2.45 Quality Record: A completed document or record furnishing evidence of


successful implementation of any given aspect of the Quality
Management Program. These records may include any number
of formats or media, such as written reports, electronic media,
drawings, or charts.

7.3 Quality System Outline


7.3.1 General
 The Quality implemented during procurement and construction of the superstructure works of
Riyadh Housing Project is being planned and documented within this Project Quality Plan.
 The QA/QC Engineer will reports directly to the QA/QC Manager.

 All workers and staff involved with the project have a role and responsibilities in the
implementation of the Project Quality Plan.

 The project Quality Control Plan details responsibilities of personnel in relation to the operation
of the Quality System and achievement of the required quality for the work.

7.3.2 Quality Control Activities

 The main objective of the Quality Control is to test, inspects, follow-up procedures, and related
actions during and after execution of the work to evaluate that actual products incorporated
into the work and completed construction activities in conformance with requirements and quality
standards.

 The Project Quality Control Plan details quality control requirements including receiving, In-process
and final inspection and test. The QA/QC Engineer shall complete, as necessary, check lists, test
reports and the like, which record successful completion of the inspection and test requirements.

 To provide formal inspections and test requirements and to identify inspection and test points to

be prepared by ABR.

 Whenever necessary, a non-conformance procedure is to be initiated to advise the Engineer


of deficiencies identified during inspection and testing.

 To ensure that Quality Control is defined as a series of specific activities performed to


provide a reproducible quality product.

7.3.3 Quality Assurance Activities

 The main objective of the Quality Assurance is to ensure that activities, actions, and procedures
performed before and during execution of the work to guard against defects and deficiencies
and substantiate that proposed construction will comply with requirements.

 Quality Assurance will implement procedures to establish a plan and control activities that
eventually contribute to the quality of the workmanship. These procedures provide details of
the activities that shall be used by project team to implement the quality system procedures.

 Quality Assurance will be defined so that a system of general programmatic activities implemented
to ensure Quality Control is performing adequately.

 A complete Quality Assurance Plan encompasses both QA and QC functions and strives to identify
which function is addressed by a specific activity.

7.3.4 Audit Activities

 Audit is a verification activity carried out to assess the operation and effectiveness of Project
Quality Control Plan.

 Quality system element audits are to be undertaken by ABR Project Manager, or his nominee,
who objectively audits and reports on the operating procedures to evaluate their use and
effectiveness.

 Continue surveillance to evaluate the construction process being used and the physical quality of
the work being achieved is generally undertaken by the QA/QC Engineer. Surveillance is
undertaken to verify conformance of the products and processes with specified technical
requirements and method statements, respectively.
7.4 Engineering

7.4.1 Purpose and Scope

 ABR shall implement a system to ensure that those tasks relating to the engineering phase of
the project are performed in accordance with the contract requirements with regards to
procedures, instructions and guidelines.

7.4.2 Responsibility

 Prepare all drawings using the CADD system and provided a digitized type of CADD drawings in
accordance with the requirements of the project.

 Establish, implement and monitor the Quality Assurance program for the work and other
activities as described for the engineering phase of the Project.

 Perform and document all Quality Assurance functions to ascertain that all engineering activities
are performed in accordance with the contractual requirements.

7.4.3 Actions

 ABR shall staff the Project with sufficient engineering and appropriate discipline
representatives to execute the engineering works and achieve all of t h e r e q u i r e d
milestone dates. All work shall be carried out by competent and approved
personnel.

 ABR ensure that the engineering requirements will be fully and clearly defined the Client
supplied documents.

 Engineering shall verify the process, to assure that the requirements for the engineering works
have been achieved, that acceptance criteria have been established and that all relevant reviews
and actions are documented.

7.5 Document Control

7.5.1 Purpose and Scope

 Document control serves as the control point and shall provide for the distribution of all
engineering-related project documents (e.g. specifications, data sheets, requisitions,
procedures and drawings).

 ABR shall implement and maintain a document control system to ensure the following:

a. Documents effecting quality are approved prior to issue for construction.


b. The latest issue of approved documentation shall be used on the construction site.
c. Documents must be completed and should not cause any confusion.

7.5.2 Responsibility

 The QA/QC Engineer is the overall responsible for planning and control of the document control

system.

 The Project/Site Engineer shall be responsible for giving instruction to the CADD Operator for
the preparation of shop drawings and shall review the said shop drawings prior to approval by
the Project Director.
 The Project Director shall be responsible for the submission of requests for
change or clarification to the Engineer.

 The Project Document Controller shall maintain files of original engineering documents and an
updated list of all the drawings and specifications received from Ministry of Housing. All
obsolete documents will be marked superseded and will be retained until the end of the project.
At the end of the project all documents will be returned to the Engineer.

7.5.3 Actions

 Document Control shall initiate, prepare and maintain the various engineering document indexes
for the following items.

a. Project Quality Plan

b. Method Statements

c. Submittals

d. Project Drawing/Specifications

e. Design and Engineering Documents

f. Contract Documents/Drawings

g. Project Construction Time Schedule

h. Quality Records
i. Request for Information

j. Applicable Standards

 Registration of controlled documents upon approval shall be registered using the appropriate
form.

 Registration shall involve recording the following information relating to the document:

c. Current Revision Number
d. Date of Issue

 For the purpose of recording, the following registers shall be used:

 Lists of all documents shall be maintained. The listings include current revision level information
and are available to all associates that need the information to carry out their activities.

 Down level and obsolete documents shall be properly marked to ensure they are not used.

 Current version levels of all documents shall be maintained in areas where the works described
in contract documents are being carried out.

 Quality assurance/control section shall ensure that a complete set of record of contract
documents are maintain on site and are regularly mark with approved amendments to the
contract documents. QA/QC Engineer shall check periodically all the contract documents available
on site office for construction purpose to check its availability and in order to ensure that all
documents used by construction personnel are the latest approved revisions.

 Forms shall be updated whenever required and additional forms shall be included if situation
requires.

7.6 Procurement

7.6.1 Purpose and Scope

 ABRI shall ensure that, only subcontractor / suppliers to provide materials and/or services are
approved by the Engineer.

 The QA/QC Engineer shall ensure that approval from Engineer is obtained for all subcontractor
/ suppliers.

 ABR QA/QC Team will monitor the subcontractor / suppliers to ensure that specified
requirements are being met.

7.6.2 Responsibilities

 Upon receipt of written approval from the Engineer of the subcontract documents, purchase
order of the materials/equipment will be prepared.

 The Quality Control Engineer together with the Project Director shall be responsible for
review of the subcontract documents / purchase orders and verify their adequacy with respect
to the specified requirements prior to their final release.

 The QA/QC Engineer shall be responsible for the evaluation of the subcontractor or supplier's
approach to the control of quality, prior to placing of the purchase order.

7.6.3 Actions
 Purchasing Documentation

 The QA/QC Engineer, in coordination with the Project Director shall check all purchasing
documentation prior to dispatch to proposed subcontractors / suppliers.
 The delivery of materials or equipment being purchased shall have the following information:
a. Description of the goods, materials or equipment.
b. Quantities
c. Technical requirements
d. Required delivery date.
e. Formal Quality Assurance/Quality Control requirements,
f. Identification, packaging, handling and delivery conditions.
g. Documentation (e.g. drawing, test certificates, O&M manual, etc.)

7.6.4 Materials Receiving Documentation

 The Material Receipt Form shall be the formal material receiving documentation. The receiving
department shall include all original copies of shipping documentation with the Material Receipt
Form.

7.6.5 Arrival of Materials/Equipment at Project Site

 ABR field personnel will be responsible, upon notice of shipment by the supply contractor, for
knowing all pertinent shipping information. The notification of shipment shall be documented
by ABR forwarding the supply contractor's notice.

7.6.6 Selection of Subcontractors / Suppliers

 Vendor/subcontractors will be involved in the project only after the approval of the Engineer.
 Prior to select a vendor/subcontractor, checking of previous experience, works performed,
quality control system, capability to execute requested works, shall be done.

7.6.7 Verification of Purchased Materials/Equipment

 Upon award of a subcontract / purchase order of materials/equipment, the QA/QC Engineer,


with the Project Director, shall plan and coordinate the implementation of a program to ensure
that specified requirements are being complied.

 The quality and arrangements for verification may be changed depending upon the performance
of the subcontractor I supplier.

7.6.8 Control of Procurement

 The Procurement Department is responsible for the purchasing of all items (with the exception
of some local consumables, spare parts etc.) is controlled in our Head Office in Riyadh. The
activities will be carried out at the Head Office in accordance with the Head Office procedures.
 Before issuance of any Purchase order, it is necessary to issue Material Approval Request
(MAR) to the Engineer. After the approval the material can be ordered.

7.7 Materials: Inspection, Receipt and Control

7.7.1 Materials/Equipment Receipt Procedure

 Purpose and Scope

 This section provides direction for the receipt, handling, and storage of permanent plant
materials and equipment.
 The purpose of this section is to provide basic guidelines for acceptable site practices utilized in
material receiving, handling, and storage procedures to ensure the quality integrity of
permanent plant materials and equipment.
 Detailed instructions and procedures for receiving, handling, and storage of permanent plant
equipment and materials are contained in the following sources. These sources provide the
specific details and criteria for acceptable receiving, handling, and storage practices:

7.7.2 Responsibilities

 Checking of materials/ equipment arrived at site shall be carried out by the site Warehouse
In-Charge assisted by the Quality Control Inspector as necessary.

 The QA/QC Engineer shall be responsible for the verification of certification supplied with the
items.

 The Quality Control Department shall ensure that all the materials delivered to the site are
complying with the Contract Requirements and as per approved sample.

7.7.3 Actions

 Immediately upon delivery of materials/equipment, the items supplied must be checked and
inspected by the Quality Control Inspector using the Material Receipt Inspection Checklist. He
shall notify immediately the Project Director and / or the QA/QC Engineer for any deficiencies.
 The completed Material Receipt Inspection Checklist together with all required certification
associated with the items shall be forwarded to the QA/QC Engineer for verification of
compliance with the specified requirements.
 Reports are to be issued to the Engineer by the Project Director regarding deficiencies
identified upon receipt or while in the custody of ABR.
 In the event of a Non-Conformance being issued against material, this material will be tagged by
means of a red tag/label stating "HOLD DO NOT USE".

 In the event of a deficiency being found, ABR QA/QC Engineer shall be informed immediately
for the decision as to whether a non-conformance report should be raised.

 Material Inspection Reports are to be transmitted to ABR QA/QC Engineer for additional
verification of compliance with the specified requirements.

7.8 Inspection and Testing

7.8.1 Purpose and Scope

 ABR shall implement inspection test activities for products resulting from manufacturing and
construction processes.

 The requirements shall include activities associated with incoming inspection (on material and
products supplied by subcontractors and suppliers), in-process inspections (on the use of
these materials and products following incoming inspection, but prior to final inspection) and
final inspection for acceptance.

7.8.2 Responsibilities
 Project personnel at the construction works (including the Quality Control Inspectors, Field
Staff, Foreman and Surveyors) and subcontractor I supplier shall be responsible for the
implementation of request for inspection, quality control activities, detailed inspection and test
requirements.

 The QA/QC Engineer shall be responsible for establishing hold I witness points to ensure
verification of compliance as shown on Inspection and Test Plans. The Inspection and Test Plans
are to be submitted to the Engineer for their comment approval and for the Engineer to identify
their hold I witness inspection test requirements.

7.8.3 Inspection and Test Status

 The status of inspections and tests shall be maintained at all stages of the construction and
installation process. Such identification ensures that only materials, which have passed the
required inspections and tests is accepted for use.
7.8.4 Actions

7.8.4.1 Incoming Inspection

 For deliveries direct to the site warehouse, the QA/QC Engineer or Quality Control Inspector
shall ensure that all components and materials to be incorporated to the works have been
inspected prior to use in accordance with inspection requirements. For this purpose the
QA/QC Engineer or Quality Control Inspector shall complete the Material Receipt Inspection
Checklist.

 For deliveries direct to the work place, the Site Engineer or the Foreman shall ensure that the
Delivery Description correctly represents the components and materials being delivered and
that the quantity is correct.

 Using the appropriate inspection and test plan the QA/QC Engineer shall compile and check
all inspection and delivery records and ensure that the components and materials delivered
to the site warehouse, and directly to the work place, comply with the specified requirements.

7.8.4.2 In Process Inspection

 Where nominated in the inspection and test plan the foreman, surveyor, QA/QC Engineer,
Quality Control Inspector, and Subcontractor I Supplier shall carry out inspections and I or
arrange for testing as may require by registered testing laboratories at the required test
frequency.

 Where required, the Inspection Report formats shall be included in the Inspection and Test
Plan and shall be used to record the details of these inspections.

 If a checklist is received for completion during the inspection, the person nominated in the
Inspection and Test Plan shall complete the checklist and submit it to the QA/QC Engineer.

 The Engineer shall be notified of impending Hold I Witness Points using the "Inspection Request"
form at least within 24 hours in advance. As required by the specification the form shall be

submitted in advance of the impending Hold I Witness Point Inspections and will nominate the
date and time of the activity.

 The signature of ABR Quality Control Inspector is required to release relevant hold I witness
points shown on the Inspection and Test Plan and also the signature of the Consultant's
Quality Control Inspector to release relevant hold I witness points.

 The "Inspection Request" (IR) shall be submitted to the Client, for advice and action. Work shall
proceed unless ABR is advised otherwise.

 Where necessary, Non-Conformance Reports shall be issued by the QA/QC Engineer.

7.8.4.3 Final Inspection

 Upon completion of a section of the work or part of the work by another subcontractor, the
QA/QC Engineer shall inspect the work and complete a Punch List for that particular section of
the work. The Punch list shall be used to record and eventually sign off all outstanding items
and remaining defects.

 In connection with inspection of the physical works, the QA/QC Engineer shall verify
information and review all associated conformance records, including all records produced by
inspection and test activities, and any Non-Conformance Reports that may have been generated.

7.9 Inspection, Measuring and Test Equipment

7.9.1 Purpose and Scope

 ABR shall establish and maintain a system to ensure that all identified measuring and test
equipment being used, shall be calibrated with equipment traceable to project standards.

 To provide a standard calibration procedure to all types of testing equipment.

7.9.2 Responsibilities

 The QA/QC Engineer shall be responsible for the assessment and identification of inspection,
measuring and testing equipment that requires formal calibration.

 The QA/QC Engineer shall be responsible for the establishment and maintenance of the
calibration record.

7.9.3 Actions

 The Contractor shall ensure that all test equipment used for measuring testing shall be fully and
satisfactorily calibrated.

 Prior to use, the equipment must be checked by the QA/QC Engineer to ensure that its
capabilities are compatible with the intended application.

 Equipment shall be ensured that it is calibrated prior to use on the Project.

 As minimum all survey equipment used on the Project shall be calibrated prior to being used on
the works and subsequently at the frequency detailed in the Calibration Record.

 Documentation verifying the calibration shall be maintained in the record.

7.10 Non-Conformance Control

7.10.1 Purpose and Scope

 A Non-Conformance condition exists when any part of the construction, material or


equipment do not conform to the requirements of contract documents, approved submittal
or design drawings.
 The purpose of this procedure is to provide a vehicle for the resolution and correction of any
items, procedures, materials, or equipment identified as not conforming to project documents.

 Surveillance reporting shall be used as a mechanism to highlight procedures and/or items of work,
that if continued shall be deemed as not conforming to project documents.

 The non-conforming control mechanism does not apply to quality related matters, which can
be resolved through normal working channels.

7.10.2 Responsibilities

 The Project Manager or his designee shall determine compliance with corrective action
requirements and identify the need for follow-up action. ABR shall review the NCR log weekly
and shall submit a copy of the NCR to the Engineer.
 The QA/QC Engineer shall be responsible for recording and processing the identified non-
conformance.

 All Project staff employed by ABR shall be responsible for the identification of all kinds of works,
items, procedures, materials or equipment not conforming to subcontract documents.
 QA/QC Engineer will install a red tag when an item is reported as non- conformance to stop
the works, which will be replaced by the green tag after Engineer’s approved disposition. The
green tag will be removed only by QA/QC Engineer when the NCR is closed out or a Non
Conformance Closure (NCC) is being issued by the Engineer.
 NCR's shall be numbered sequentially and a log maintained identifying the following:
a. NCR number

b. Originating Discipline

c. Date initiated

7.10.3 Actions

 When a procedure or an item of work has been identified, during the regular monitoring
activities of the QA/QC Inspector and prior to completion of the work, as not complying with
the Project documents, the QA/QC Inspector shall advise the QA/QC Engineer.

 The QA/QC Inspector involved shall initiate and issue a surveillance report to the relevant
discipline site engineer.

 The surveillance report shall highlight the problem area and shall suggest a possible method of
corrective action that may be taken to avoid a non-conformance recurrence.

 The surveillance report shall also indicate a reasonable time span during which the corrective
action should be completed. In addition the surveillance report shall also advise that should
the work not be completed within this time span, that the surveillance report shall be
immediately raised to the level of a non-conformance.

 Where a non-conformance has been identified, the Project staff member responsible shall
advise the QA/QC Engineer.

 The QA/QC Engineer shall initiate a non-conformance report for internal NCR.

 The original non-conformance report shall be returned to the QA/QC Engineer and the details
available shall be entered in the non-conformance log register. At this stage, details of the
area, originator and description shall be entered in the log.

 The Project Director shall issue instruction to implement the required corrective action upon
receipt of the non-conformance report with an approved resolution.

 The QA/QC Engineer shall sign the original non-conformance report and a copy of which shall
be forwarded to the Engineer upon completion and approval of corrective action.

 The original non-conformance report shall be reviewed by the QA/QC Engineer and the
details entered to update and closeout the non-conformance report log, for that particular non
conformance report.

 In the event of a subcontractor's failure to comply with the corrective action requirements of
an NCR either unsatisfactory work or failure to meet the time requirements, the QA/QC
Engineer on behalf of the Project Director will prepare formal correspondence with a copy
of the original NCR attached, advising the subcontractor of his failure to meet the corrective
action requirements.
 Letter will establish a date by which the corrective action must be satisfactorily completed by
the subcontractor and state that in the event of failure to satisfactorily complete the corrective
action by this date, others will perform the corrective action work with all associated costs
back chargeable to his contract.

 Copy distribution of all such follow-up NCR correspondence shall be the same as the original
NCR and shall be filed in the appropriate NCR file.

7.11 Corrective & Preventive Actions for NCR

7.11.1 Definitions

 Non-Conformance – non-fulfillment of a requirement of the Project standard.

 Corrective Action – is an action taken to eliminate the cause of a detected non-conformance.


 Preventive Action – is an action taken to eliminate the cause of a potential non-conformity or
avoid repetition of the same non- conformance.

7.11.2 Responsibilities

 It is the responsibility of QA/QC Engineer to prepare and issue a non- conformance report
(NCR) on detection of a non-conformance including, where appropriate, as a consequence of
an environmental complaint.

 It is the responsibility of all workers to bring suspected non- conformances to the attention
of the QA/QC Engineer.

 Construction Manager and Site Engineers will comply with all corrective and preventive actions
prescribed. In exceptional cases, they should inform the QA/QC Engineer of reasons why
actions cannot or will not be taken, for further consideration and decision.

 The QA/QC Engineer will establish and maintain a reporting and record keeping system for non-
conformances, corrective and preventive actions.

 Non-conformances, corrective and preventive actions will be reviewed through the


Management Review process.

7.11.3 Procedures

 By whichever means a non-conformance is identified, the underlying cause/s of the non-


conformance must be investigated.

 Appropriate and timely corrective action must be taken according to the nature of the non-
conformance.

 Preventive action, such as implementing modifying or enforcing procedures or controls, will


be taken to avoid repetition of the non-conformance, or prevent a potential non-conformity
from occurring.

 Any corrective or preventive action taken to address the causes of an actual or potential non-
conformance must be appropriate to the magnitude of problems and commensurate with the
environmental impact encountered.

 The QA/QC Engineer will implement and maintain a system of reporting and record keeping for
non-conformances, corrective and preventive action.

 Any changes to the environmental management procedures as a result of corrective or


preventive action will be recorded.

 The non-conformance report form will detail the nature and scale of the non-conformance,
propose corrective and preventive actions, as appropriate, include references to procedure
number, date and include timescales, where relevant.

7.11.4 Non- Conformance Tags

QA/QC Engineer shall use the non-conformance tags as follows:

 When an item found as non-conformance, related work shall stop and reported to Project
Director. After NCR, install issuing Red tag at non- conformance location and notify the Consultant
immediately.

 Red tag is place prior to Engineer’s disposition.

 After approval by Consultant’s disposition, QA/QC Engineer will remove Red tag then install
Green Tag. The Green tag allows to proceed the works.

 Red tag is place prior to Engineer’s disposition.

 After approval by Consultant’s disposition, QA/QC Engineer will remove Red tag then install
Green Tag. The Green tag allows to proceed the works.

 QA/QC Engineer will remove green tag when the NCR is close out by him and the Engineer.

7.12 Handling, Storage and Preservation Procedure of Materials

7.12.1 Purpose and Scope

 Plan and implement a system to ensure that material storage, handling and preservation is
controlled to prevent misuse, damage, deterioration, or loss.

 Material handling practices shall be applied to ensure a minimum possibility for deterioration
of materials and components by physical damage, or loss of shipping and storage protection
provided by the vendor or supplier.
 Normal handling of materials is under the direction and responsibility of the receiver who will
take all due precautions to minimize damage to material.

7.12.2 Responsibilities

 The Construction Manager and Warehouse man shall be responsible for the monitoring of
handling, storage and presentation of materials during construction activities.

 The QA/QC Engineer shall ensure that special care in handling of materials storage and
presentation requirements are included in the subcontract document / purchase orders.

 The Warehouse Man will check for receipt, storage and issuance of all materials and
equipment and the preparation of required record.

7.12.3 Maintenance of Storage Areas

 The warehouse shall maintain a storage map indicating the current status of storage areas. In
the event that a subcontractor is being hired, he shall maintain his assigned storage areas in
accordance with the following criteria:
a. Access should be controlled.

b. Cleanliness/good housekeeping shall be applied.

c. The condition of the storage area shall be monitored.

d. Fire protection will be compatible with materials stored and type of storage.
e. Sources of contamination, such as eating and drinking in areas, rodents, and flooding,
shall be controlled.

7.12.4 Material Control

 The Material Control at site shall be carried out in such a way to keep the materials in good
order, and issue them at the required quantity and at the required time in good condition, to
preclude any delays in construction schedule.

 Major objectives are:

a. The status of outgoing and incoming materials can be immediately ascertained.

b. Material shall be stored properly.


c. Excess material in the field shall immediately be returned to the warehouse for
proper receiving, storage.

d. Upon completion of each construction job, surplus materials shall be handled in

accordance with the project requirements.

7.12.5 Identification and Traceability of Materials

 From receiving until incorporation into the works, materials must be handled, stored, erected in
such a manner to allow an easy identification and tracking.

 Each material shall have a unique identification number/code. Under these unique numbers /
codes, all related documents and records shall be retained on files. The materials will be
stored/handled/incorporated into the work using this number/code.
7.12.6 Actions

 The QA/QC Engineer shall check that special handling, storage and presentation requirements
are included in subcontract documents / purchase orders.

 Storage areas shall be organized to maintain proper segregation of materials.

 Items should contain purchase order identification for retrieval and inventory control applicable
to the nature and use of the material.

 Bins rack and restricted access area shall be provided to segregate material by usage, jurisdiction
and classes.

 Electrical and mechanical materials should be separately stored.

 Floor storage shall be laid out in a given pattern to facilitate traceability of items and according
to storage records.

 The material receiving report shall designate which of these areas the material
is to be placed as it arrives at the job site.

 Materials found to be defective or badly damage during delivery on site should be returned
back to supplier.

 Material received at the job site shall be recorded on the material receiving report in
accordance with accompanied documents.

7.13 Quality Records Management

7.13.1 Introduction and Purpose

 Records are an important organizational asset; they provide the primary route for evidence
based verification and traceability, and demonstrate compliance with client requirements
while also proving the efficacy of the quality management system.

 Quality records are to be labeled and indexed for ease of retrieval and for proper referencing.

7.13.2 Application and Scope

 The scope of this process encompasses all records generated by the quality management system
as well as records generated by sub-contractors or other third parties. Records must be
controlled so the information they contain is readily accessible, legible and suitably maintained.
This procedure works in conjunction with the Control of Documents Procedure.

7.13.3 Requirements

 This procedure applies to all quality management system records and is to be followed by all
personnel where appropriate. Quality records are generated and maintained by the project
document controller responsible for their creation.

7.13.4 Quality Records

 A project must establish and maintain records, which provide evidence that the
product/workmanship has been inspected and/or tested. These records shall show clearly
whether the product/workmanship has passed or failed the inspections and/or tests according
to defined acceptance criteria. Where the product/workmanship fails to pass any inspection
and/or test, the procedures for control of non-conformance, shall apply.

7.13.5 Process

7.13.5.1 Identification of Quality Records

 Quality records are maintained to attest to the implementation of the quality system.
Records are stored as secured computer files for electronics data and in designated filing
cabinets to prevent deterioration and damage for hard copies. Such records are easily
accessible for use and are made available for review upon Consultant or audit request.

 The following documents are acceptable records:

a. Submittal and Quality Forms


b. Reports
c. Minutes of meetings
d. Computer files

 Archival documents and data retained for legal or knowledge preservation purposes or both
are suitably identified. All records must contain sufficient data to attest to satisfactory
completion of the recorded activity and at minimum, must be signed and dated by the
individual responsible for completing the record.

7.13.5.2 Protection, Storage and Retrieval of Quality Records

 Quality records exist in either hard copy or electronic formats. Hard copy records are stored
where they are protected from physical deterioration, loss and damage due to environmental
conditions.

 Electronic back up data and are stored in a lockable, fire resistant cabinet.

 Quality record is also labeled and indexed for ease of retrieval and for proper referencing.

 All record cabinets, containers, and devices are clearly marked and labeled to identify their
contents. Records are indexed and grouped for expedient retrieval. Records must not be
stored on personal storage drives or files.

 The retention period for quality records is determined by the client and contractual
requirements or legal considerations, etc.

7.13.5.3 Disposal of Quality Records

 Upon expiration of the retention period, Document Control will dispose of such quality records
in an appropriate manner. Confidential records are shredded.

7.13.6 Responsibilities

 The QA/QC Engineer shall plan in establishing and maintaining a filing system for the
collection and storage of all documents generated by the Quality System.

 The Project Document Controller will maintain at site all generated quality records and compile
complete close-out documents.

7.13.7 Actions

 Documents shall be generated in compliance with the activities described in the Project Quality
Plan. These documents are collectively termed the Quality Records and shall be retained and
filed.

 The QA/QC Engineer shall collate and maintain the Quality Records in an indexed and readily
accessible filing system.

 Records shall be stored in chronological order commencing with the oldest and progressing
through to the most recent.

 The Quality Records to be collected are stored and shall include, but shall not be limited to,
the following:
a. Test Certificates
b. Checklists from Inspection Activities
c. Non-Conformance Reports

7.14 Quality Audit Program

7.14.1 Objective

 The objective of the Quality Audit Program is to monitor the Quality Control Plan to ensure that
the plan is operating efficiency and properly.

7.14.2 Responsibilities

 Audit Report shall be generated by the QA/QC Engineer to document all discrepancies found
during the period of the audit.

 QA/QC Engineer shall check all the time effectiveness of quality assurance/control program
and shall act for implementation of all provisions stipulated by the program.

 The Project Director or QA/QC Engineer may initiate audit of quality assurance/control
program.

7.14.3 Audit of QA/QC Program

 The quality audit shall consist of audits that shall review the following:

a. Non-conformance Reports
b. Non-conformance Report log
c. Project Quality Control Plan
d. Document Control
e. Test Procedures

 A copy of the Audit Report shall be filed in the Quality Assurance Office, for review by the Project
Director.

 Follow-up action, if required, shall entail subsequent periodic reviews of those items noted
to ensure that the corrective actions have been properly implemented.

 Implementation and effectiveness of quality assurance/control program shall be checked


periodically to ensure its compliance with project requirements and if its provisions are strictly
followed.

7.14.4 Quality Control Audit Schedule

 Quality control audit is done annually or in the case may be when there is a breakdown of the
construction works that might cause delay and affect the project time schedule.

7.14.5 Actions for Project Internal Quality Audit Plan

 QA/QC Engineer prepares the internal quality audits plan at the beginning of the year taking
into consideration the case and importance of each activity and covering all the activities
affecting quality at least once a year.

 The Project Director approves it, and then a copy of the plan will be sent to concerned section.

 The Audit Team shall study and review all the documents related to the audit subject.

 Each Auditor will record non-conformities that he will detect during the audit.
 After the audit, the Audit Team will have a meeting to discuss notices and non- conformities
appeared during the audit.

 The QA/QC Engineer will make a report and submit it to Project Director for approval.

 The Project Director approves the audit report and copies will be distributed through QA/QC
section to concerned sections to carry out corrective actions.

 The Project Director is responsible to initiate a corrective and preventive action required and
QA/QC Engineer follows up the action for each non-conformity and follow-up corrective actions
execution.

 Internal Quality Audits results and all of its reports shall be maintained.

7.15 Independent Testing Agency/Laboratory Roles and Responsibilities

7.15.1 Objective

 The independent testing agency/laboratory is to perform a specified testing where under the
Contract.

7.15.2 Quality Assurance

 Independent Testing Agency/Laboratory which may be employed by ABR in the event of a


specific testing required, shall be an agency authorized to operate in the Kingdom of Saudi
Arabia.

 Independent Testing Agency/Laboratory shall maintain a full time specialist or staff to review
the required services regularly.

 Calibrated equipment shall be used for the required tests.

7.15.3 Qualifications and Procedures

 Tests and inspections shall be conducted in accordance with specified requirements and
if not specified, in accordance with applicable standards of the American Society for Testing
and Materials and other recognized authorities as approved.

 Submit written description of testing laboratory giving qualifications of personnel, laboratory


facilities and equipment.

 The testing laboratory shall be approved by Engineer and registered to perform inspection
and certification services as an approved inspector.

7.15.4 Testing Agency/Laboratory Responsibilities

 Comply with specified standards and any "special inspections" whether or not such inspections
are specified in the project specifications.

 If the material furnished and/or work performed fails to meet requirements of contract
documents, laboratory inspector shall promptly notify both the Contractor and the Engineer
of such failure.

 Coordinate ahead of time with the Engineer when samples will be taken.

 Provide qualified personnel at site. Cooperate with the Engineer and ABR in performance
of services.

 Perform specified sampling and testing of products in accordance with specified standards.

 Ascertain compliance of materials and mixes with requirements of Contract Documents.

 Perform additional tests as may be reasonably required by the Contract.

 A representative of the testing agency who has reviewed and is familiar with the project and
specifications, if necessary shall attend Quality and construction meetings as he may be
required or requested to address quality control issues.

7.15.5 Testing Agency/Laboratory Reports

 Testing Agency/Laboratory Reports shall include but not limited to the following:
a. Date of Issue.
b. Project title and number.
c. Name, Address, Telephone number of testing agency.
d. Dates and Location of Samples and Test or Inspections.

test pre-requisites have been met, that adequate calibrated instrumentation is
available and used and that the necessary inspection is performed.
ii. Revision Control: ABR shall revise each ITP and obtain approval for such revisions from
the Engineer as needed throughout the work.

B. Submittals

1. Prior to the start of the applicable work, A B R shall submit for review and approval
the detailed Inspection and Test Plan (ITP) for all construction processes.
2. ABR shall submit in a separate submittal, for approval by the Consultant, all Inspection
and Test Plans required and shall be in accordance with relevant provisions of the
Project Specifications.

C. Inspections Procedures
1. Respective site engineers for each discipline i.e., Civil, Mechanical and Electrical; shall
prepare his respective areas or structures to be inspected in accordance with latest
approved drawings and the related applicable project specifications.
2. When the site engineer of a discipline estimated to be sure that his work is complete
and ready for inspection he will submit the Request for Inspection (RFI) to the QA/QC
Engineer.
3. QA/QC Engineer shall come to the area along with site engineer and together check
the work against approved latest drawings, project specifications or any approved
applicable documents.
4. If the QA/QC Engineer finds some items of works still need correction he will instruct
the concerned site engineer to make the necessary corrections.
5. The QA/QC Engineer re-checks corrected items after a reasonable time; and if the result
of corrections is already satisfactory, he will sign the RFI and submit it to the Consultant,
at least within 24 hours in advance from the stated inspection time already shown on
the RFI.
6. The QA/QC Engineer shall wait for the Consultant at the work site at the time stated
on related IR, keeping with him all the necessary latest approved drawings and other
approved applicable documents.
7. As soon as the Consultant arrives at work site, QA/QC engineer shows him the structures
to be inspected as well as the concerned approved drawings.
8. The Consultant and Project QA/QC Engineer shall conduct a joint inspection for the work
as per submitted RFI.
9. If the Consultant approves this work as per submitted RFI, ABR shall proceed with the
next stage of construction activities as planned.
10. If the Engineer writes some remarks as correction needed or not accepted, ABR will
make all the necessary corrections and shall submit a new RFI when the work is ready
for inspections and shall notify the Engineer at least within 24 hours in advance prior to
next inspection.
11. This procedure shall continue until the work become satisfactory and acceptable to the
Engineer.

D. Testing
1. General

Testing shall be performed either by A B R laboratory on site or by independent laboratory


if required.

2. Construction Inspections and Test Records

 Shall submit test reports to the Consultant.

 Identify inspection and test status.

 The status report shall identify all required inspection and tests for each system when it is
scheduled, when it was completed and any non-conformances discovered during the test or
inspection.
 Inspection status reports shall be kept current and shall be available for Consultant’s review
at all times.
3. Contractor’s Responsibilities
AB R CO NT RA CTI N G COMPANY shall:
 Include procedure for the control of inspection, measuring and testing equipment. Specific tools
and equipment shall be identified to make and perform tests in the field with instructions for
their use, calibration and storage. Identify, maintain, control, adjust and calibrate tools,
gauges, instruments and other measuring and testing devices used for activities controlling
quality. Calibration shall be performed at established period.
 Audit and document the validity of previous inspection and test results when inspection,
measuring and testing equipment is found to be out of calibration
 Give the Testing Agency/Laboratory sufficient notification to arrange for the taking of adequate
samples of materials proposed for the use of Works or for the performing of a specific tests
when required.
 Arrange for Agency/Laboratory access to the site or location where samples are to be taken
or specific tests performed.

4. Mock-Up Requirements

 Mock-up is a sample of works or practice of Method Statement to be approved by the


Consultant before proceeding work.

 Construction of the mock-ups will be programmed as approved by the Engineer.

 Approved mock-ups will establish the minimum acceptable quality of workmanship and will
serve as the standard by which subsequent work will be judged as acceptable. Resubmit or
amend mock-ups until approval is given by the Engineer. Remove mock-ups that are
designated as unsuitable. Do not proceed with work until mock-up has been approved.

 Retain each approved mock-up until receipt of Engineer’s instruction to remove or to


incorporate it in the works.

 Locate mock-ups where directed by the Engineer.

10.0 CONTROL OF SUBCONTRACTOR QUALITY SYSTEM


A. Scope

This procedure details the minimum quality requirements that suppliers and sub- contractors
to ABR must maintain should they wish to supply materials/parts which may be fitted into
deliverable products.
B. General

 The Quality System requirements must be met by sub-contractors and suppliers.

 ABR aim is to procure items and services from sources where Quality is assured.

 Suppliers/Sub-Contractors with Third Party Certification of their QMS, from an approved


Certification Body, will be preferred.

C. Code of Conduct for Suppliers

 Suppliers / sub-contractors must comply with all national and other applicable laws and
regulations.

 Expect its suppliers/ sub-contractors to make proper provision for the health, safety and
welfare of visitors, contractors & customers. Suppliers / subcontractors are encouraged to
implement a health and safety management system.

 Suppliers / sub-contractors should appreciate the impact their business has on the
environment and adopt appropriate practices and procedures.

 Sub-contract activities shall be conducted in accordance with all relevant HS&E laws and
regulations.

D. Quality Assurance

 Quality Management System (QMS) Requirements

 Suppliers / Sub-contractors shall have a demonstrable Quality Assurance system, or be able to


show that actions have been put in place to achieve such requirements.

 Suppliers / Sub-contractors shall appoint a person with sufficient authority to make the
final decisions on all Quality related matters.

E. Contract Review

 Parts/materials purchased by the supplier are to be checked against the drawing, specification,
standard to ensure conformity.

 Parts manufactured or processed, tested and inspected by the supplier are to be checked to
ensure conformity.

 The supplier shall ensure that outsourced vendors shall meet the requirements of the process
specification.

F. Documentation Control

 The supplier shall operate an effective documentation control system to ensure that the
correct issue of controlling drawings and specifications are used.

G. Quality Records

 Quality records associated with manufacturing and all aspects of Traceability shall be
maintained.

 Records shall be stored in a manner that makes them readily available at the request.

 Archived records to be stored in a manner to prevent damage or loss.

 Archiving of records, which are consigned from hard (paper) copies to microfilm/microfiche
or electronic media (scanned images etc.), must be carried out in accordance with appropriate
standards. A system for reviewing these must be maintained by the suppliers / sub-contractors.

H. Material Control

 Where suppliers and sub-contractors use other suppliers and sub-contractors in fulfillment of
orders, then purchase orders on these second tier suppliers and sub- contractors must flow
down the applicable purchase order requirements.

 Suppliers and sub-contractors shall have a process for the approval and on-going evaluation of
their own sub-tier suppliers and sub-contractors and the status of such suppliers shall be
communicated to relevant personnel.

 Sub-tier Vendors may be subject to Quality Audit by the Suppliers/Sub- Contractors to ensure
compliance with requirements.

 All raw materials must be obtained with appropriate material certification to allow traceability
back to source. The information detailed on this certification is to be verified against the
definitive standard.

I. Handling, Storage and Preservation

 On receipt of parts from a vendor, the supplier should have a receiving goods procedure.
All parts should be checked in accordance with this procedure prior to parts being confirmed as
acceptable for use.

 Materials must be clearly identified and segregated.

 The Supplier/Sub-Contractor shall include within any planning documentation, provision

for the prevention, detection and removal of any foreign objects.

 A process for the identification and disposition of perishable items with shelf life limitations
shall be in place.

J. Initial Sample Inspection Report

 For items other than those identified as controlled parts, Sub-contractors shall submit an
Initial Sample Inspection Report (ISIR) with the first delivery of any item and subsequently, when
changes or repairs to tools, fixtures used in the manufacturing process occur, to permit to
evaluate any influence of the change on their own products. The report should provide
information on the reason for change and the detail of the change.

 The reports may be prepared using on the suppliers own format if all information detailed on
the drawing is addressed, including notes and further processes.

K. Fabrication, Manufacturing Control and Traceability

Manufacturing, assembly and inspection/test records shall be generated (if applicable) to allow
the configuration history to be known at any point in time particularly at the point of release.
Where the supplier utilizes fixtures and tooling for the manufacture, test or inspection of
items, a register of these tools is to be maintained, ensuring correct use and that they are
maintained and calibrated in accordance with the requirements.
Manufacturing Controls / Traceability will allow for the following:

1. Identification to be maintained throughout the products life.

2. Batch Traceability, back to the parent raw material.

3. The identity of Sub-assemblies shall be traceable, to and from, its parent assembly.
4. Parts shall only be procured from Quality Assured sources where Traceability is
maintained.
5. Original manufacturer certification shall be provided when requested.

6. Traceability is maintained if sub-tier vendors / outsourced processes are used.

L. Inspection Control and Calibration

1. Where a key characteristic are identified on the controlling document, it shall either
subjected to 100% inspection. Each measurement taken shall be recorded and
submitted for each manufacturing batch, unless otherwise agreed by Quality
department.
2. All Test and Measurement equipment used for verification of parts must be
encompassed within the suppliers / sub-contractors Calibration system. The system
and records for internally calibrated items must demonstrate that the calibration is
traceable.
3. Shall be notified immediately of any instances that come to light where non-
conforming product may have been inadvertently supplied without notification.

4. Inspection stamps/signatories shall be used to indicate that all Quality and design
requirements have been met. The inspection and test status of items shall be
maintained throughout production and installation to ensure that only items, which
have passed inspection and/or test at previous stages of operation, proceed for further
work operations.

M. Non-Conforming Items

 Notification of non-conforming material, the supplier /subcontractor shall take those


steps necessary to determine if other batches or related parts or processes already
delivered may have been affected and advise accordingly.

 The Supplier/Sub-Contractor shall write a response to any reject, advising of


containment actions from receipt of reject note with closure of the reject. The
response should be completed on the Reject Note defining:

a. Remedial action on the product


b. Root cause

c. Corrective and preventative measures stating date of implementation, supplying


evidence of these measures i.e. procedure changes, Quality Alerts, etc.

11.0 OFF-SITE WORK CONTROL


A. Request to Work off-Site

 The request to work off-site should include the following information:

1. Description of the responsibilities to be performed by the worker.

2. Length of time proposed to work off-site.

3. Resources needed to complete the work.

4. Assessment of negative impact on worker’s absence from the project construction site.

B. General Guidelines

1. Off-site work may be granted for both usual, day-to-day responsibilities and special
projects provided that they met the criteria for productivity and necessity.
2. Workers must come to an agreement with their superior about the type of
responsibilities would qualify for off-site work in advance so as to facilitate faster
approval when situation arise.
3. Off-site workers should be accessible and respond to contacts from their superior and
others related to their approved responsibilities in a reasonable time frame.

4. All safety precautions must be observed for the duration of the off-site work.

12.0 CONTROL OF SHOP DRAWING DEVELOPMENTS

 Shop drawings reflect the contractor’s understanding and interpretation of the construction
documents for the fabrication and installation of the various components used in the
construction of the project. These drawings are detailed to indicate the sequence of
assembly of various components of the project, and its final form or position within the project.
The following procedures are sequence of shop drawing developments:

1. Shop drawings are being prepared by the AutoCAD draftsman.

2. Project Discipline Engineer will check the shop drawings in compliance to the project
requirements and submit to the project document controller.

3. The Project Document Controller will assign a unique reference number in sequential
manner and revision number. He will encode to the system according to the data
given. Format for submittal should be as per the relevant Drawing Submittal
numbering system as mentioned in item 3.4 of Numbering Guidelines.
4. The Project Director will review and sign transmittal ensuring all required
information has been included. Then he will issue transmittal to relevant party.
5. Upon receipt back from the Engineer either approved or with comments, the
Project Document Controller will stamp with date and time received and distribute as
required. Then he will update the Drawings Transmittal Register log. Lastly, he will send
confirmation of receipt to the Engineer.

13.0 REPORTING
A. Quality Reports

1. Quality reports are to be submitted to the Engineer regularly in weekly and


monthly basis.
2. Routine quality inspection tests are to be submitted in daily basis.

14.0 QUALITY MEETINGS


A. Management Review Meeting

The quality assurance team will meet monthly throughout the duration of the project with
the management representative for the express purpose of assessing the suitability,
adequacy and effectiveness of the quality management plan.
Management representative is responsible for the planning of Management Review Meeting
at least once a year. This is after completion of each internal quality audit cycles according
to procedure of internal quality audits.
After completion of one quality audit cycle covering all sections, the QA/QC Engineer
prepares a report about the quality systems in the project including:

1. Effectiveness of the quality system

2. Weak points of the system

3. Proposals for improvement

This report will be submitted to the management representative to be reviewed before


discussion in the management review meetings.
The Quality Management Review Meeting will be held as follows:

15.0 MAINTENANCE, CALIBRATION PLAN & PROCEDURE

A. Purpose

The purpose of this Standard Operating Procedure (SOP) is to provide a standard


calibration procedure and standard calibration for the calibration of all types of testing
equipment.
B. Procedure

1. Operate the equipment in accordance with the relevant Operating Instructions.

2. Perform the equipment calibration according to the equipment specific SOP.

3. Fill out form “Calibration Record” sheet with the following information:

a. Equipment name/description and ID number


b. Name of manufacturer
c. Current location
d. Installation date
e. SOP number
f. Date of out-of-use
g. Date of last calibration

4. Evaluate if “Suitability Sticker” has been compromised.

5. In cases where the Suitability of equipment has been compromised due to the re- calibration,

the technician will deface and cross-out the “suitability Sticker” and attach a note stating
“Performance Verification Invalidation Due To Re- Calibration”.

6. The performance of the equipment only where necessary shall be repeated prior to routine
testing use.

7. Calibration procedures shall be appropriate for the intended use of the equipment and shall
provide criteria for determining if calibration is satisfactory.

8. Manufacturer operating manuals shall be consulted to determine the correct calibration


interval. Equipment which requires calibration shall not be used if satisfactory calibration
cannot be achieved or the calibration date has passed.

C. Re-Calibration

 If the calibration does not conform, repeat the procedure.

 If the second calibration does not conform, clean and check the equipment according
to the manufacturer’s instructions. Then repeat the procedure.

D. Documentation

 Document calibration results as per “Calibration Record Sheet” will serve as guidance.
E. Labeling

Testing equipment requiring calibration shall be recorded or coded to indicate the


calibration status, including the date when last calibrated and the due date for
recalibration, when practicable. Alternatively, equipment calibration status may be
identified in an associated record to indicate the status of calibration.
F. Maintenance

1. Testing Equipment shall be maintained at all times.

2. Out of Service Testing Equipment

Testing Equipment that has been subjected to overloading or mishandling, gives suspect
results, or has been shown to be defective or outside specified limits, shall be taken out of
service. It shall be isolated and/or clearly labeled (Out of Service – Do Not Use) to prevent
use until repaired and shown by calibration or test to perform correctly.
3. Quality Control Checks

Quality control checks may be carried out at appropriate intervals to verify


that testing equipment is functioning as expected. The procedures for quality control
checks shall be included in the technical procedure for which the equipment is being used.

4. Correction Factors

Where calibrations give rise to a set of correction factors, the section shall ensure that
software is updated with these correction factors.

17.11 Training Program


17.11.1 A training program shall be employed at the job site covering orientation, indoctrination, quality
control procedures and methods and supplementary quality control technical training.

17.11.2 Introduction

The QA/QC Assistant Group shall be responsible for each newly assigned Quality Control
Engineer with sufficient orientation and indoctrination training to assurance an understanding of
the following topics.

The organization, purpose and objective of the quality control manual. The application and use
of the Project Inspection Plan and Procedures.

Project special provisions contained in the technical specification applicable to a particular phase of
work.

This indoctrination shall be completed within four (4) weeks of assignment to the project and
before the Quality Control Engineer is certified to perform verification inspection on project.

17.11.3 Procedures and Methods Training

At least quarterly, a one hour minimum training session with a predetermined agenda shall be held
by the Supervisor. These training sessions shall be attended by Quality Control Engineers involved
with the activities being discussed shall consists with work operations in-progress. Subject to be
discussed shall include the implementation of quality control inspection plans and procedures,
documentation requirements, applicable codes and standards, processing of non-conforming
items, records keeping and other similar quality control related topics.

Supplementary Quality Control Technical Training and Supplementary Quality Control Training Aids
shall be prepared by Quality Control Engineers. These Training Aids shall include General
Instructions in the application of established Quality Control Inspection Plans and Procedures as
well as training in various inspections activities.

17.11.4 Training Records

The Supervisor shall be responsible for maintaining a file of the quality control training sessions
attended by each Quality Control Engineer. These records shall show the number of training
sessions attended, the subjects covered in each session, the name of the instructor who led each
training session, and the date the training was received.
