    PHARMACIST AFFIDAVIT

    I, RANJITH KUMAR P, S/o Pethan. A, residing at No. 1/180, Murugesan Nagar, Neelankarai Link Road,
    Thoraipakkam, Chennai - 600097, declare and solemnly affirm the following to the Licensing Authority, THE
    ASSISTANT DIRECTOR OF DRUG CONTROL, ZONE-IV, CHENNAI-6 that I am a registered pharmacist, having
    registered as pharmacist with Tamil Nadu pharmacy council with Registration No. 24016 A1 and renewed up to 31st
    March 2020.
    I am employed in M/s ASTRA PHARMACY” situated at Ground Floor in Exg. ASTRA SPECIALITY HOSPITAL at
    Plot No: 707 & 724, Tsunami Colony Bypass Road, Nookampalayam Link Road, Off OMR Road, Semmancheri,
    Chennai – 600119 as a full pharmacist from 16/8/2018 onwards and I am not employed anywhere else.
    I was not previously worked in any firm/concern.
    I will look after the day to day activity of the business of the said firm & I am fully aware of the provisions of Drugs
    and Cosmetics Act and I am responsible for any contravention thereof. I will maintain the sales bills for drugs sold on
    prescription & maintain all the record and registers of the firm as stipulated under the Drugs and Cosmetic Act 1940
    and rules 1945.
    In case I leave the said firm I will inform the licensing authority of my resignation and this affidavit to be treated as
    cancelled from the date of resignation.
