1. PURPOSE
This plan is prepared for emergency evacuation in case of any incident or emergency situation
at job site during the construction of Fire Water Storage Tank. This plan is in line with the
site safety requirements and covers detailed steps of response for Emergency response and
evacuation at site.

2. SCOPE
Scope of this document is to set up an emergency response and communications system that
shall apply to any emergency occurring in the Project Area during the Construction of Tank.

3. RESPONSIBILITY
3.1 EMERGENCY RESPONSE TEAM LEADER
a. Inform emergency services if required.
b. Report to Enppi HSE department.
c. Collect roll call result from emergency coordinator.
d. Obtain and provide details regarding the incident.
e. Give instruction for all clear siren.
f. Collect and submit all documents regarding the incident.
g. Implements instructions for corrective actions recommended by Enppi.
3.2 EMERGENCY COORDINATOR:
a. Assign emergency crew members as per their responsibilities
b. Collect row call result and compare with manpower list.
c. Ensure that any further risk is mitigated to prevent loss of life.
3.3 ASSISTANT EMERGENCY COORDINATORS
a. Evacuate the people immediately from site
b. Guide them to assembly area.
c. Make departmental roll call at assembly area.
d. Report to emergency coordinator on persons missing
e. Maintain order and control over employees.
f. Ensure workers do not leave unless authorized by Emergency Coordinator.
3.4 FIRST AIDER
a. Assess the victim.
b. Give proper first aid according to the nature of accident.
c. Safely shift the victim to Ambulance as it will be provided by Enppi.

d. Assist the victim to hospital.
3.5 FIREMAN
a. Fireman will be trained to use fire extinguishers
b. Fireman will extinguish any fires on site
c. After roll call Fireman can be tasked to do checking to find missing workers. Two
Firemen will always conduct such searches together.
d. Every Fireman after such search, will personally report back to the Emergency
coordinator.
e. Act as per emergency coordinator’s instruction
f. Assist nurse to get injured persons on stretcher.
g. Assist the first aider to shift the victim to ambulance.
h. Fireman may instruct any worker to activate the fire alarm.
i. When a fire is detected by a Fire Team member the first priority is to extinguish the
fire.
3.6 RESCUE TEAM
a. The Rescue Team members will be trained to rescue people from height / confined
space, etc.
b. This team shall assembly and report directly to Emergency Coordinator immediately
after roll call.
c. Once the task had been completed the members shall report to the Emergency
Coordinator.
3.7 SPOTTER/ FLAGMAN’S RESPONSIBILITIES
a. Report to emergency coordinator immediately.
b. Wait for the ambulance at the site entrance as directed by the Emergency
Coordinator
c. Guide ambulance to the scene of incident ensuring that ambulance does have passage
clear of excavations, soft ground, etc.
d. Report back to Emergency Coordinator after completion of duties.
3.8 SITE PROJECT ENGINEER
The overall responsible for the implementation of a sound emergency evacuation plan.
3.9 EMERGENCY RESPONSE TEAM
Implement guidelines and procedure specified in this plan.
3.10 STANDBY ATTENDANT
a. Must observe the entry operation from outside the confined space at all times.
b. Keeping rescue equipment available.
c. Staying aware of conditions of employees in the confined space.

4. INTRODUCTION
Confined Space is a place or an area which is not normally intended for human occupancy,
it could be a structure like storage tanks, vessels, vault, deep excavations (4 feet deep) and
other areas in which there are: oxygen deficient atmosphere or presence of combustible,
carbon monoxide and other toxic gases.
4.1 IDENTIFICATION OF EMERGENCY
A situation, which has the potential to be classified and treated as an emergency, may become
apparent in any of the following ways:
a. An accident in which worker(s) has sustained injuries or death or significant property
damage has occurred.
b. An automatic warning device has been activated by fire, smoke, and heat or gas
detection.
c. A fire or gas emission has been discovered and a manual alarm has been activated.
Assuming that the situation is of such a magnitude that the individual who first becomes
aware of it cannot safely provide immediate assistance to all personnel who may be injured
or in imminent danger, then that person’s responsibility is to immediately activate the
sounding of a general alarm (if available) and notification of a more senior person with the
specific location and facts of the incident.
4.2 EMERGENCY EVACUATION PROCEDURE
In an emergency:
When an emergency condition exists or o n hearing of “STOP WORK ALARM”, Every
Supervisor shall ensure that:
a. All works stop at once.
b. All equipment to be shut down.
c. All men are evacuated on foot to a predetermined assembly area in a cross or upwind
direction. A roll call is taken and every man is accounted for.
d. A roll call to be taken and every man to be accounted for. This is to be done by site
Emergency Coordinator.
e. All workers to be sent back to camp, if necessary.
f. No one shall be permitted to return to work until notification has been received from
Enppi that it is safe to do so or until after the “All Clear” is given.
g. A roll call will be conducted by the safety officer and his designated assistants. The
documentation will be maintained by the Safety officer under his custody.

When driving: Upon hearing the “Stop Work Alarm” or “Disaster Siren”, park the vehicle
at the right side of the road (clear of hydrants, etc.) stop engine, leave the key inside the
vehicle and readily proceed to the nearest assembly point in a cross or upwind direction
and stay in the area until after the “All Clear” alarm is sounded.

In the event of actual emergency, all work permits will be automatically canceled. All
authorized WPR, supervisors will ensure that no activity resumes in the area, unless the
permit is renewed and re-issued by the SA Permit Issuer.
4.3 EMERGENCY ASSEMBLY POINTS
Designated emergency assembly points/safe assembly areas will be identified near the
tank and Locations of the specific assembly areas will be posted and will be informed to all
employees.
If the designated assembly point is not accessible, for some reason (i.e., it is downwind or
in close proximity to the scene of emergency), then personnel must take the safest route
way from the emergency location, towards an alternate assembly point, safe assembly area
or open area in a crosswind or upwind direction. At this point, time is extremely important
that a head count is done to ensure all workers are accounted for.
Each supervisor will ensure all workers are accounted for. Then they will report to AZMWF
management, Enppi representative will be notified that all are present and accounted for.
Drills will be conducted frequently as required in order to acquaint all personnel in case of
emergency. This exercise will be upon Enppi schedule and Al Zamil will participate; result
of the drills will be recorded.
Immediate Evacuation / Site Restriction
All affected employees, immediately upon having been alerted of the emergency using
portable canister air horn, shall stop the work, shut down all equipment and quickly proceed
to a predetermined safe assembly area.
Nobody shall be allowed to return to site until after an instruction from En p p i is received
through.
Access road for equipment or vehicle entry into evacuated areas shall be immediately
provided with watchman to stop them from entering the vicinity.

5. GENERAL REQUIREMENTS
5.1 RESCUE EQUIPMENT
a. Safety Harness.
b. Appropriate Stretcher / Lifeline
c. Appropriate Personal Protective Equipment.
SCBA (provided by Enppi Rescue Team)
d. Man basket
e. Flash Lights and Whistle
f. Mobile Crane and Manlift

5.2.1 Emergency Response Team Leader
Emergency Response Team Leaders the person who reports directly to Enppi and site
Project Engineer. Site HSE officer will be designated as Emergency Response Team Leader.
5.2.2 Emergency Coordinator
a. The Site Engineer has the responsibility of Emergency
Coordinator.
b. Performs key role when an emergency occurs on site.
c. If any accident /incident happened inform Team Leader immediately.
d. Issues instruction to all his co-workers and emergency team members.
5.2.3 Assistant Emergency Coordinators
a. All Foremen will act as Asst. Emergency Coordinator.
b. Assist Emergency Coordinator with any tasks so instructed.
c. In the absence of Emergency Coordinator, the most senior person on site will act as
Deputy Emergency Coordinator.
d. Deputy Emergency Coordinator will have same duties as Emergency Coordinator and
should be prepared for this.
5.2.4 First Aider
a. In all sites we have qualified first aiders but in emergency situation one person should
take only one responsibility at a time so we have a common first aider for all projects.
b. First Aider will provide first aid as required.
c. First Aider will decide on additional treatment required and direct the ambulance
driver to such facility as determined necessary.
d. Once patient had been moved to required destination the First Aider will report to the
Team Leader immediately.
5.2.5 Fireman/ Fire Watcher
Fireman is a person who will be present at the site where hot work activities are going on
to monitor the workplace to avoid any fire incident. In emergency situation all will report
to emergency coordinator while the siren is on, emergency coordinator will give instructions
to all crew members all crew members will act as per emergency coordinators instructions.

5.2.6 Spotter/ Flagman’s
Flagman is a person who is designated at site as a flagman. He assigned duty with the moving
vehicles or equipment. Sometime also utilized to control traffic at construction site. Spotter is
the one who will spot effected area to the ambulance on instruction of emergency coordinator
or his assistance.
5.3 COMMUNICATION DURING ANY INCIDENT
During reporting of an emergency ensure the following information is provided

5.3.1 Say: “This is an emergency.”


5.3.2 Exact location.
5.3.3 Nature of the emergency/incident.
5.3.4 Report Injury if any
5.3.5 Type of service(s) required (i.e., fire, medical, security).
5.3.6 Give name and badge/ID number.
5.3.7 Repeat above information if ask to do so
5.4 LIFE LINE
If potential exposure in the confined space is acutely hazardous, the employee shall wear
full body harness and wristlets with a lifeline attached or coveralls with built-in harness
and lifeline.
When the shape, size, location or work performed in confined space restricts exit of
personnel, a specified procedure for the rescue of personnel shall be developed during the
planning stage. It is recommended that standby attendant shall secure the free end of the
lifeline to a fixed object.
5.5 RESCUE OF PERSONNEL IN CONFINED SPACES
Should it become necessary to rescue personnel from a confined space, it is essential that
all personnel involved know exactly what to do and what not to do. There have been
several instances where persons trying to rescue an individual in a confined space also became
victims because of failure to follow the proper procedure.
The hazards associated with work in confined spaces include possible exposure to the
following:
a. Oxygen deficient atmospheres.
b. Atmospheres containing flammable gas or dust.
c. Atmospheres containing toxic substances.
A person who is down or who appears to experiencing difficulty while working in a confined
space could be affected by any of the conditions listed above, from a heart attack or other
illness. A rescuer must recognize these potential problems and protect him by wearing
appropriate respiratory and personal protective equipment.
Equipment to affect a timely rescue is located at the entrance of the confined space or is
immediately available to the rescue team. Such equipment includes Self-Contained Breathing
Apparatus (SCBA) or equivalent airline equipment, a harness, rope and other equipment as
indicated.

5.6 COURSE OF ACTION
Upon knowing the situation on the emergency and needs to rescue personnel in the
job site, confined spaces and Work at height:
a. The standby-man stationed outside the confined space must not enter in a rescue
attempt until other help has been called.
b. Do not panic.
c. Hold your breath.
d. Sound an alarm and call for backup help.
e. Check the wind direction by looking the windsock.
f. When back-up help arrived, put on your full rescue unit, get the nearest Self-Contained
Breathing Apparatus that is upwind or cross wind. Unless you are 100% sure that you
know the cause of the incident, you must treat it as if resulted from abnormal atmosphere
and wear SCBA.
g. Put on the SCBA. Making sure you have an airtight face seal and check the air supply
before going to the rescue. Forgetting this crucial step can easily make the rescuer another
victim.
h. Go to the victim and pull him immediately to fresh air upwind or crosswind to a safe
area. In doing so be as gentle as possible so as not to aggravate any injuries the victim
may already have sustained. Use the stretcher if possible.
i. If the victim is breathing; maintain the victim at rest and have trained personnel
administered oxygen to him.
j. Keep victim lying down with blanket or coat. Conserve body heat and do not leave
unattended.
k. If eyes are affected by H2S, wash them thoroughly with clean water, (for slight eye
irritation), cold compresses are helpful.
l. In case a person has only minor exposure and does not lose consciousness totally, it is
best if he does not return to work until cleared by a doctor.
m. If the victim is not breathing, start artificial respiration immediately.
n. Call ambulance& get the victim to the nearest hospital or doctor.
5.7 RESCUE OF PERSONAL FROM WORK AT HEIGHT

Emergency Scenario and Mode of Rescue:

1. Person getting Injured or Fainted on Working Platform inside the Tank:


In order to facilitate rescue, injured/fainted person will be shifted from bracket scaffold platform
into Man basket and further he will be taken out of the tank with the help of the crane.
Once the person is taken out from tank, necessary medical assistance can be provided.

2. Person has fallen from inside platform and in Suspended Position:

In order to facilitate rescue, use ladders to rescue the fallen worker with the procedure outlined below.
1. If the fallen worker is suspended from a lifeline, move the worker (if possible) to an area that
rescuers can access safely with a ladder.
2. Set up the appropriate ladder(s) to reach the fallen worker.
3. Rig separate lifelines for rescuers to use while carrying out the rescue from the ladder(s).
4. If the fallen worker is not conscious or cannot reliably help with the rescue at least two
rescuers may be needed.
5. If the fallen worker is suspended directly from a lanyard or a lifeline, securely
attach a separate lowering line to the harness.
6. Other rescuers on the ground (or closet work surface) should lower the fallen worker while the
rescuer on the Ladder guides the fallen worker to the ground or work surface.
7. Once the fallen worker has been brought to a safe location, administer the first aid and treat the
person for Suspension Trauma and any other injury.
8. Arrange transportation to hospital if required.

3. Person has fallen from outside platform and in Suspended position:


In order to facilitate rescue, if possible, use EWP (Manlift) to rescue the fallen worker from
outside scaffold platform, follow the procedure below.
1. Bring the man lift to the accident site and use it to reach the suspended worker.
2. Ensure that rescue workers are wearing full-body harnesses attached to appropriate anchors in the
Manlift.
3. Ensure that the manlift has the load capacity for both the rescuer(s) and the fallen worker. If the
fallen worker is not conscious, two rescuers will probably be needed to safely handle the weight
of the fallen worker.
4. Position the manlift below the worker and disconnect the worker lanyard when it is safe to do so.
when the worker is safely on the manlift, reattach the lanyard to the appropriate anchor point on
the manlift.
5. Lower the worker to a safe location and administer first aid (by certified person) treat the worker
for suspension trauma and any other injury.
6. Arrange transportation to hospital if required

5.8 REPORTING

Al Zamil should ensure that an oral report is made to ENPPI representative immediately in the
event of any accident incident. Written report will follow including incident details and
investigation.
